The Orphanin FQ / Nociceptin receptor as a novel drug target in psychiatric disorders.

Almost 10 years after the discovery of Orphanin FQ / Nociceptin (OFQ/N) a large number of synthetic small molecule agonists and antagonists have been developed and tested in various physiological assays. Together with the academic work on the physiological functions of OFQ/N, we now have compelling evidence that this neuropeptide system might represent a valuable target for the development of drugs treating a variety of psychiatric disorders. Most prominently, the anti-stress and anxiolytic effects of OFQ/N agonists have been investigated although clinical trials have not yet been launched. Ot

Suicidal behavior in patients with adjustment disorders.

This retrospective study characterizes the suicidal behavior in 119 patients with Axis I adjustment disorders as assessed by psychiatrists at the University of Virginia Hospital. Results indicated that 72 patients (60.5%) had documented suicide attempts in the past, 96% had been suicidal during their admission to the hospital, and 50% had attempted suicide before their hospitalization. The most commonly used method of suicide attempts was overdosing. Of the sample group with suicide attempts in the past, 67% had Axis II diagnoses of borderline personality disorder and antisocial personality di

What risk factors tell us about the causes of schizophrenia and related psychoses.

Genetic, epidemiologic, and molecular studies concur that liability to schizophrenia is transmitted through the inheritance of a number of genes of relatively small effect, some of which are shared with other psychoses. Each of these susceptibility genes causes minor deviations that are relatively innocent in themselves, for example, increased lateral ventricular volume, schizotypal personality, or subtle cognitive difficulties. However, when an individual is unlucky enough to inherit several of these traits, their cumulative effect, often compounded by environmental hazards, propels that pers

Psychiatric comorbidity differences in male and female adult psychiatric inpatients with substance use disorders.

To examine gender differences in the co-occurrence of DSM-III-R axis I disorders and axis II personality disorders in young adult psychiatric inpatients with substance use disorders (SUDs), a consecutive series of 70 inpatients (33 men and 37 women) with SUD were reliably assessed with structured diagnostic interviews. Higher rates of dysthymia and eating disorders were observed in SUD females and higher rates of cluster A personality disorders were observed in SUD males. No gender differences were found for depression or anxiety in our SUD inpatients; these findings contrast with gender ratio

Dissociation, somatization, substance abuse, and coping in women with chronic pelvic pain.

OBJECTIVE: To examine the relationships between histories of sexual or physical abuse and current reports of dissociation, somatization, substance abuse, adaptive coping, and maladaptive coping strategies among chronic pelvic pain patients. METHODS: Using a structured interview, we assessed sexual and physical abuse and somatization. The Dissociative Experiences Scale was used to assess dissociation, and an abbreviated version of the COPE scale was employed to assess adaptive and maladaptive coping strategies as well as substance abuse. Participants included 46 women with chronic pelvic pain.

Psychiatric disorders and crime: a study of pretrial psychiatric examinations.

A study of 50 consecutive individuals referred for pretrial psychiatric examination, presumably because of previous psychiatric hospitalization (82 percent), indicated that antisocial personality, alcoholism, or drug dependence was present in 80 percent. Schizophrenia or bipolar affective disorder was seen in about a third of the subjects, usually associated as well, however, with antisocial personality, alcoholism, or drug dependence. There were no significant differences in index crime between those with or without schizophenia bipolar affective disorder. It is concluded that the latter cond

Injection and non-injection drug use and infectious disease in Baltimore City: differences by race.

PURPOSE: The current study examines differences in the prevalence of biologically-confirmed hepatitis C virus (HCV), HIV, and coinfection between Black and White adult cocaine/heroin users across three drug use subgroups identified in previous research (Harrell et al., 2012): non-injection smoking crack/nasal heroin users, heroin injectors, and polydrug injectors. RESULTS: 59% of the 482 participants in the study were male. Significant race differences emerged between drug use subgroup memberships. Non-injection smoking crack/nasal heroin users were predominantly Black (75%), while heroin inj

The role of illicit substance use in a conceptual model of intimate partner violence in men undergoing treatment for alcoholism.

Illicit substance use (ISU) predicts intimate partner violence (IPV) above and beyond alcohol use and other known IPV correlates. Stuart and colleagues (2008) provided evidence for a theoretical framework by which ISU contributes both directly and indirectly to IPV. We sought to replicate and extend their findings using data from 181 married or cohabiting heterosexual couples in which the male had recently begun a substance abuse treatment program and met criteria for alcohol dependence (97%) or abuse (3%). Using SEM, we found that (a) Stuart et al.'s model provided a good fit to the data; (b)

Infrequent illicit methadone use among stimulant-using patients in methadone maintenance treatment programs: a national drug abuse treatment clinical trials network study.

We sought to determine the prevalence, patterns, and correlates of past-month illicit methadone use and history of regular illicit use among stimulant-using methadone maintenance treatment patients. We obtained self-reported information on illicit methadone use from 383 participants recruited from six community-based methadone maintenance programs. Overall, 1.6% of participants reported illicit use in the past month, and 4.7% reported a history of regular use. Younger age and history of outpatient psychological treatment were associated with increased odds of past-month illicit use. Illicit me

A 12-year follow-up of a treated cocaine-dependent sample.

The study examined long-term outcomes (mortality, substance use, mental health, employment, criminal involvement) among a cocaine-dependent sample. This 12-year follow-up study, conducted in 2002-2003, updates information obtained at intake and two face-to-face interviews conducted in 1990-1991 and 1991-1992 among 321 male cocaine-dependent veterans admitted to drug treatment in 1988-1989. At the 2002-2003 follow-up, 28 had died and 266 were interviewed. A mixed model examining the longitudinal relationships demonstrated that treatment was associated with lower levels of cocaine use over the 1

Gender differences in crack users who are research volunteers.

This study compared gender differences in a non-treatment sample of crack cocaine users interested in participating in a research study on addiction. Data was collected from initial telephone screening interviews of women and men responding to cocaine research recruitment in a midwest urban environment over a two-year period. Female respondents (n = 88) were age- and race-matched with men interviewed over the same time period, for a total sample size of 176. Mean age of the female sample was 33 years and the majority were African-American. Basic demographics were similar for both genders. Resp

HIV-transmission knowledge in drug users from outpatient facilities in Spain. A national survey.

Knowledge of AIDS and its transmission was studied in patients undergoing drug-dependence treatment for opiates and/or cocaine. The study area included all of Spain and was carried out via a questionnaire assessing information about risk practices, HIV serostatus, etc, and 13 true or false questions concerning HIV-transmission knowledge. The main purpose of the present study was to analyse the relationship between the level of information found in patients and factors potentially related to it, including the practice of high-risk activities for HIV transmission, the sources of information and

Sleep findings in young adult patients with posttraumatic stress disorder.

BACKGROUND: Laboratory sleep studies in posttraumatic stress disorder (PTSD) have not provided consistent evidence of sleep disturbance, despite apparent sleep complaints. Most of these studies have investigated middle-aged chronic PTSD subjects with a high prevalence of comorbidities such as substance dependence and/or personality disorder. METHODS: Ten young adult PTSD patients (aged 23.4 +/- 6.1 years) without comorbidities of substance dependence and/or personality disorder underwent 2-night polysomnographic recordings. These sleep measures were compared with those of normal control subje

A gender perspective of dual diagnosis: a narrative review.

Dual diagnosis (DD) refers to the coexistence in the same individual of a substance use disorder and other mental disorder. The increase in substance use among women over the past two decades, along with related health issues and mortality rates, has underscored the importance of integrating a gender-based approach in addiction research and treatment. This transition has led to an increased focus on substance use among women and individuals of other gender identities. This narrative review aims to examine the evidence on gender differences in DD to enhance knowledge that supports the developme

Third-Generation Antipsychotics and Lurasidone in the Treatment of Substance-Induced Psychoses: A Narrative Review.

This narrative review explores the efficacy and tolerability of third-generation antipsychotics (TGAs)-aripiprazole, cariprazine, brexpiprazole, and lurasidone-for the management of substance-induced psychosis (SIP). SIP is a psychiatric condition triggered by substance misuse or withdrawal, characterized by unique features distinct from those of primary psychotic disorders. These distinctive features include a heightened prevalence of positive symptoms, such as hallucinations and delusions, in addition to a spectrum of mood and cognitive disturbances. This review comprehensively investigates

Time to pay attention: an underrecognized casualty of the opioid epidemic.

Maternal mortality and overdose deaths have both been on the rise in the USA, but the relationship between the two is unclear. Recent reports have pointed toward accidental overdoses and suicides as leading causes of maternal mortality. This short communication collected data on psychiatric-related deaths, suicide and drug overdose, from each state's Maternal Mortality Review Committee to better conceptualize the rate at which these deaths are occurring. Data was collected from each state's most recent online MMRC legislative report and met inclusion criteria if the reports included the number

Unhealthy Alcohol Use Associated with Obsessive-Compulsive Symptoms: The Moderating Effects of Anxiety and Depression.

Alcohol use occurs among individuals with obsessive-compulsive symptoms at a rate significantly greater than the general population. In clinical populations, obsessive-compulsive disorder (OCD) and substance use disorders (SUD) have been shown to share neurological substrates, but little is known about the mechanisms underlying substance use in individuals with OCD. Aspects of anxiety and depression frequently contribute to various SUD and are thought to play a role in the relationship between increased substance use and obsessive-compulsive symptomatology. The present research examines the mo
