Bad habits and bad genes: early 20th-century eugenic attempts to eliminate syphilis and associated "defects" from the United States.

American eugenists in the early 20th century distinguished "degenerates," including syphilitics, prostitutes, alcoholics and criminals, from the "normal" population by their particular bad habits. From eugenists' viewpoint, these bad habits were derived from bad character, a flaw that stemmed from an individual's bad genes. This essay explores how eugenists during this period characterized syphilitics and those with associated character "defects" in terms of heredity. Additionally, it examines the methods eugenists most frequently advocated to rectify these bad habits. These methods included m

Apparent failure of one injection of benzathine penicillin G for syphilis during pregnancy in human immunodeficiency virus-seronegative African women.

BACKGROUND: Syphilis remains a major cause of premature birth, fetal and perinatal death, and congenital syphilis in South Africa, despite systematic antenatal screening by rapid plasma reagin and treatment with 2.4 million U of benzathine penicillin G. GOAL: To determine whether one injection of 2.4 million of U of benzathine penicillin G, as recommended by the 1993 Centers for Disease Control and Prevention guidelines, is sufficient treatment for early syphilis during pregnancy. STUDY DESIGN: Outcome of pregnancy was prospectively analyzed after zero to three weekly intramuscular injection

Central nervous system involvement in early syphilis. Part II. Correlation between auditory brain stem responses (ABR) and cerebrospinal fluid abnormalities.

Twenty-six consecutive patients with secondary or early latent syphilis, all with normal neurological findings, were studied with auditory brainstem responses (ABR), cerebrospinal fluid (CSF) analysis and an assay for immune complexes in serum and CSF. Seven of the 26 patients studied had a pathological ABR pattern and 14 had one or more pathological CSF tests. Fifteen patients had circulating immune complexes in serum. No patient had demonstrable immune complexes in CSF. A significant correlation was found between an abnormal ABR pattern and pathological CSF findings--but not between patholog

Frequent emergency department utilization and syphilis case profiles in the Sentara healthcare system: a retrospective cross-sectional analysis in Hampton Roads.

BACKGROUND: Syphilis rates have increased substantially nationwide, disproportionately affecting marginalized populations and individuals with differing patterns of healthcare utilization. The Hampton Roads region of Virginia and North Carolina includes multiple jurisdictions with syphilis rates above the CDC screening threshold, with 7 of its 9 independent cities meeting this criterion. However, no prior study has examined the relationship between emergency department (ED) utilization patterns and documented syphilis diagnoses in this region. METHODS: A retrospective cross-sectional analysis

Cost-effectiveness of an opt-out emergency department screening program for syphilis in North Queensland's Aboriginal and Torres Strait Islander population.

BACKGROUND: This study aimed to evaluate the cost-effectiveness of an emergency department (ED) syphilis screening program for the Aboriginal and Torres Strait Islander population (aged 15-40 years) presenting to a regional Australian ED, compared with no screening program. METHODS: A decision tree model was developed using clinical data from a previous audit and outcome probabilities from the literature. We included direct financial costs from a healthcare provider perspective and outcomes measured in quality-adjusted life years (QALYs) gained, over a 40-year time horizon. Costs were derived

Doxycycline Postexposure Prophylaxis May Dramatically Reduce Syphilis Among Gay, Bisexual, and Other Men Who Have Sex With Men: A Modeling Study.

BACKGROUND: Syphilis cases in the United States have surged over the past decade, and cases are highest among gay, bisexual, and other men who have sex with men (GBMSM). The Centers for Disease Control and Prevention recommends doxycycline postexposure prophylaxis (doxy PEP) for GBMSM recently diagnosed with bacterial sexually transmitted infections to mitigate this burden. Potential drawbacks include increased antimicrobial resistance and microbiome disruption. Here, we estimate doxy PEP coverage levels required to eliminate sustained community transmission among GBMSM while balancing the dra

Prevalence of HIV/syphilis co-infection among men who have sex with men in China: a systematic review and meta-analysis.

BACKGROUND: Men who have sex with men (MSM) are key populations for HIV and syphilis infection, and co-infection with both diseases significantly increases health risks. However, the co-infection of HIV/syphilis among this population in China has not been systematically evaluated. We aimed to conduct a meta-analysis to ascertain the prevalence of HIV/syphilis co-infection among MSM in China. METHODS: We conducted a systematic literature search involving PubMed, Web of Science, CNKI and Wanfang to identify publications reporting HIV/syphilis co-infection among MSM in China. Our search encompas

Advancing syphilis diagnosis: multi-phase study evaluation of a TpN17-based double-antigen sandwich ELISA for detecting Treponema pallidum specific antibodies.

Syphilis, a sexually transmitted infection caused by the bacterium Treponema pallidum, has high incidence rates among adults, pregnant women, and newborns. Diagnostic procedures typically involve a treponemal test (such as ELISA, CMIA, and IFI), followed by a non-treponemal test (VDRL and RPR). This study aimed to assess the diagnostic performance of a double antigen sandwich ELISA (DAgS-ELISA) using the recombinant protein TpN17, analyzing serum samples from both infected and not infected with T. pallidum. A total of 712 samples were deemed eligible and recharacterized using VDRL, ELISA, and

Prevalence of HIV, syphilis, hepatitis B, and hepatitis C in patients attending STI/RTI clinic of a tertiary care teaching hospital.

BACKGROUND: Patients attending Sexually transmitted infection/ Reproductive tract infection (STI/RTI) clinics are investigated for HIV and syphilis under the National AIDS Control Program (NACP). Although sexual contact is one of the modes of transmission of hepatitis B and C, they are not investigated under NACP. This study was planned to find the prevalence of HIV, syphilis, hepatitis B, and C in patients attending STI/RTI clinics and to identify the predictive risk factors. METHODS: A prospective cross-sectional study was carried out over 5 years on 500 consenting adults. 10 ml blood was c

Serologic follow-up of solid organ transplant recipients who received organs from donors with reactive syphilis tests: A retrospective cohort study.

The increased procurement of organs from donors with risk factors for blood-borne diseases and the expanding syphilis epidemic have resulted in a growing number of organs transplanted from donors with reactive syphilis serology in our center. Based on guidelines, recipients typically receive therapy shortly after the transplant, but data on outcomes are limited. The primary objective of this study was to determine syphilis seroconversion rates at three months post-transplant in recipients of solid organs procured from donors with reactive syphilis serology. Organ donors and recipients were tes

Effect of unintended short-term 3.0 g/day amoxicillin and probenecid treatment for early syphilis on patients with HIV-1 infection.

BACKGROUND: Standard therapy for early syphilis involves intramuscular injections of penicillin G, which frequently faces shortages in several countries. Fourteen-day amoxicillin therapy has been suggested as an alternative to benzathine penicillin G, but the optimal duration of amoxicillin therapy remains unclear and could theoretically be shortened to less than 14 days. The aim of this study was to explore the effectiveness of short-term amoxicillin therapy for early syphilis. METHODS: We retrospectively explored the effectiveness of short-term amoxicillin therapy for early syphilis. The tr
