Granular cell tumor of colon: report of a case and review of literature.

Granular cell tumor (GCT) is uncommon in the colon and rectum. Here we report a case of GCT in the transverse colon. A 48-year-old male patient underwent a screening colonoscopy. A yellowish sessile lesion, about 4 mm in diameter, was found in the transverse colon. An endoscopic snare resection was performed without complication. Histological examination revealed the tumor consisted of plump neoplastic cells with abundant granular eosinophilic cytoplasm containing acidophilic periodic acid Schiff-positive, diastase-resistant granules. Immunohistochemical analysis showed the tumor cells express

Chronic disseminated histoplasmosis in an apparently immuno-competent Belgian patient.

A 77 year-old Belgian man was admitted for weight loss and elevated liver enzymes. He used to live in Central Africa until 1986 and denied any travel outside Belgium during the last decade. Physical examination was unremarkable except for a lip ulceration. Blood tests showed increases of liver enzymes and CEA. In the search of an abdominal cancer, CT showed a normal liver but enlarged surrenal glands, while colonoscopy disclosed multiple mucosal lesions. Colic and lip biopsies identified Histoplasma capsulatum var capsulatum. HIV serology was negative. Lymphocyte count was normal but lymphocyt

Extranodal diffuse follicular center lymphoma mimicking mantle cell lymphoma of the intestine.

We report a case of diffuse follicular center lymphoma (FCL), which is a morphological variant of follicular lymphoma, resembling multiple lymphomatous polyposis (mantle cell lymphoma of the intestine). The patient was a 48-year-old Japanese man who was found, by colonoscopy, to have numerous small polypoid lesions along the entire large intestine. Abdominal computed tomography revealed hepatosplenomegaly and enlargement of multiple mesenteric lymph nodes. Histologically, the lesion was characterized by diffuse proliferation of small- to medium-sized lymphocytes with cleaved nuclei in the muco

Review article: colon cleansing preparation for gastrointestinal procedures.

Adequate cleansing is essential for reliable diagnostic and surgical colon procedures. Accuracy and safety depend on good preparation. Patient compliance is enhanced by simplicity and well-tolerated methods. Several methods are available. Diet and cathartic regimens utilize clear liquids or diets designed to leave a minimal colonic residue. Laxatives, cathartics and enemas are employed. Gut lavage solutions are osmotically balanced electrolyte lavage products. Oral sodium phosphate solutions and tablets are available and are attractive because of good efficacy with a small volume of administra

Post-transplant lymphoproliferative disorder after autologous peripheral stem cell transplantation in a pediatric patient.

Post-transplant lymphoproliferative disorder (PTLD) is a complication of allogeneic bone marrow transplantation (BMT). Rare cases of PTLD after autologous BMT have been reported only in adults. This case report is the first to describe PTLD in a pediatric patient after autologous peripheral stem cell transplantation (PSCT). This 2-year-old male with stage IV neuroblastoma underwent autologous PSCT. The post-PSCT course was complicated by fever with hematochezia and a lung mass. On day 94 post PSCT, colonoscopy revealed an ulcer due to a PTLD, monomorphic type, B cell phenotype, associated with

Recurrence rates following endoscopic mucosal resection versus endoscopic submucosal dissection for colorectal polyps: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND AND AIMS: Endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) are widely used for resecting large, laterally spreading colorectal polyps. While ESD offers higher en bloc resection rates, its comparative efficacy and safety relative to EMR remain uncertain, particularly in Western practice. This systematic review and meta-analysis aimed to evaluate the recurrence and adverse event rates of EMR versus ESD in adult patients with colorectal polyps  ≥ 20 mm. METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) compa

Life-threatening lower gastrointestinal bleeding after routine anorectal procedures: a case series and management implications.

BACKGROUND: This case series characterizes enema- and catheter-induced rectal injuries as an underrecognized iatrogenic cause of life-threatening lower gastrointestinal bleeding (LGIB). By focusing on elderly, high-risk patients, we highlight the diagnostic complexities and management strategies associated with these routine yet potentially catastrophic anorectal procedures. METHODS: We analyzed three cases of high-risk elderly patients who experienced acute, massive hemorrhage following routine anorectal procedures, specifically glycerin enemas and anorectal manometry. To contextualize these

Massive intraperitoneal hemorrhage followed by delayed perforation after forced cold snare polypectomy for a colonic adenoma.

Cold snare polypectomy (CSP) is widely considered a safe treatment for colorectal polyps. We report a rare case of intraperitoneal hemorrhage followed by delayed perforation after CSP in a 78-year-old man undergoing hemodialysis. CSP was performed for an 8-mm slightly elevated lesion in the ascending colon, requiring forced CSP to complete resection. Four hours after the procedure, the patient developed abdominal pain, and contrast-enhanced computed tomography revealed an intramural hematoma with active bleeding. Transcatheter arterial embolization achieved hemostasis. Two days after colonosco

Eosinophilic colitis recrudescence two decades after initial pentastomiasis.

Visceral pentastomiasis is a rare parasitic infection that can mimic gastrointestinal inflammatory disorders. We report a Thai female farmer in her early 60s who presented with recurrent eosinophilic colitis two decades after an incidentally diagnosed pulmonary pentastomiasis. The clinical course was characterised by chronic watery diarrhoea without weight loss, peripheral eosinophilia (11%) and colonoscopy revealing multiple submucosal polypoid lesions throughout the colon. Radiological imaging showed numerous crescent-shaped calcifications, consistent with longstanding quiescent pentastomias

A rare case of colonic malakoplakia at the bottom of an adenoma mimicking an invasive carcinoma.

We present the case of a 47-year-old man with a history of refractory multiple myeloma who was referred to our hospital for the presence of a colonic lesion suspected of having signs of deep submucosal invasion. After a surgical resection, the histological study revealed that it was a case of malakoplakia that had developed at the base of a tubular adenoma.

Tubular adenoma with squamoid morules in colonic polyp: Cases report and literature review.

RATIONALE: Squamoid morules (SM) are rare in colorectal adenomas. Submucosal pseudoinvasion in adenomas is similar to that in invasive carcinomas and needs to be differentiated, especially in the presence of mucin spillage. PATIENT CONCERNS: We present 2 cases of adenomatous polyps associated with SM. One was approximately 3.2 × 1.8 cm-sized pedunculated polyp in the descending colon of a 57-year-old man, and the other was a 1 cm-sized fungating polyp in the ileocecal valve (ICV) of a 44-year-old man. DIAGNOSES: The polyp of the descending colon was accompanied with a low-grade villotubular

Low-grade NTRK-rearranged spindle cell neoplasm presenting as a colonic polyp and managed by polypectomy: a rare case report and literature review.

BACKGROUND: NTRK-rearranged spindle cell neoplasms constitute a novel, heterogeneous group of mesenchymal neoplasms originally described predominantly in soft tissue locations. They are commonly characterized by co-expression of S100 and CD34 immunostains and presence of NTRK fusions. While exceedingly rare, there are increasing reports of this lesion involving the gastrointestinal tract, presenting predominantly as large masses of the stomach, small bowel and colorectum. CASE PRESENTATION: We present a case of a 37-year-old male who on colonoscopy was found to have a one cm polyp of the sigm

Rejection of Fecal Immunochemical Tests Within the Lower Gastrointestinal Diagnostic Pathway: A Cohort Study.

INTRODUCTION/OBJECTIVES: The fecal immunochemical test (FIT) helps triage primary care patients at risk of colorectal cancer (CRC). Improving FIT returns has received recent attention, however uncertainty exists regarding the accurate completion of samples provided for laboratory analysis. This study aims to identify the rejection rate of returned FIT samples and determine rejection causes. METHODS: FIT samples from symptomatic patients within South Yorkshire, Bassetlaw, and North Derbyshire are processed at a central laboratory. Tests requests are made from 225 GP practices, which serve an e

Gastrointestinal Biofilms: Endoscopic Detection, Disease Relevance, and Therapeutic Strategies.

Gastrointestinal biofilms are matrix-enclosed, highly heterogenic and spatially organized polymicrobial communities that can cover large areas in the gastrointestinal tract. Gut microbiota dysbiosis, mucus disruption, and epithelial invasion are associated with pathogenic biofilms that have been linked to gastrointestinal disorders such as irritable bowel syndrome, inflammatory bowel diseases, gastric cancer, and colorectal cancer. Intestinal biofilms are highly prevalent in ulcerative colitis and irritable bowel syndrome patients, and most endoscopists will have observed such biofilms during

Severe ulcerative colitis induced by COVID-19 vaccination.

A 37-year-old woman developed severe colitis with diffuse mucosal erythema and ulcerations throughout the entire colon after the 3rd vaccination of COVID-19. Stool culture was negative, and the pathological findings showed increased lymphoplasmacytic and neutrophilic infiltration in the colonic lamina propria, which were consistent with ulcerative colitis. After the treatment with anti-tumor necrosis factor-α agent, the ulceration markedly improved with development of severe colonic stenosis, which was successfully dilated with endoscopic balloon dilation. In case of COVID-19 vaccination, it s

Role of different examination methods in colorectal cancer screening: Insights and future directions.

Colorectal cancer is a malignant tumor with the third and second highest incidence and mortality rates worldwide, and its mortality rate is increasing annually. Colorectal cancer evolves gradually over a long period of time. The evolving process from colorectal adenoma to colorectal cancer takes approximately 10-15 years, providing a sufficiently long "window period" for early screening and diagnosis of colorectal cancer. The recurrence and mortality rates can be controlled at a low level with an early intervention. Metaxas et al sum-marized existing screening methods and their applicable scop
