A multiancestry polygenic risk score for Alzheimer's disease is associated with cognitive decline and neuropathological hallmarks in diverse populations.

Previously derived polygenic risk scores (PRSs) for Alzheimer's disease (AD) perform inconsistently across diverse ancestries. We developed an APOE-independent multiancestry AD PRS using genome-wide association study summary statistics applied to European ancestry, African American, Caribbean Hispanic and East Asian cohorts. PRS performance was evaluated in a large independent multiancestry dataset and validated in several additional multiancestry cohorts. The PRS was significantly associated with AD in European ancestry, African American, Caribbean Hispanic and Native American Hispanic groups

Remote digital cognitive assessment for aging and dementia using the Oxford Cognitive Testing Portal OCTAL.

The global rise in dementia necessitates scalable cognitive assessments that can evolve to serve both clinical and research applications. We present the Oxford Cognitive Testing Portal (OCTAL), a remote, browser-based platform providing performance metrics for memory, attention, visuospatial and executive function domains. Four validation studies (N = 1664) confirmed cross-cultural applicability, lifespan sensitivity and clinical utility. Task performance was equivalent in English- and Chinese-speaking younger adults and mapped domain-specific ageing trajectories in mid- to late-adulthood. In

Effects of α-synuclein pathology on synaptic dysfunction and clinical outcomes in normal aging.

INTRODUCTION: α-Synuclein is the hallmark pathology of Parkinson's disease and dementia with Lewy bodies, described together as Lewy body disease (LBD). We investigated effects of α-syn biomarker positivity in clinically unimpaired (CU) individuals. METHODS: We assessed α-syn status (α-syn ±) in 269 CU individuals using a cerebrospinal fluid (CSF) seed amplification assay (SAA). Fifty-six participants with AD and 85 LBD spectrum participants were included for comparison. We compared α-syn SAA results with demographics, fluid biomarkers, cognitive performance, and clinical measures. RESULTS:

ABCA7 Mutation in Behavioral Variant of Frontotemporal Dementia: A Case Report.

INTRODUCTION: Frontotemporal lobar degeneration (FTLD), a major cause of early-onset dementia, includes a heterogeneous group of neurodegenerative disorders with a strong genetic component. Mutations in MAPT, GRN, and C9orf72 are found in about 40% of patients with the behavioral variant (bvFTD). More recently, rarer pathogenic variants have been identified in other genes, such as ABCA7, initially linked to Alzheimer's disease but increasingly implicated in other neurodegenerative conditions. Here we describe a specific variant which has not previously been reported in the literature. CASE PR

[Transition of Patients with Neurogenic Bladder Dysfunction: Challenges and Unmet Needs in Adult Care].

Transition describes the structured process by which adolescents are transferred from pediatric and family-centered, and thus predominantly externally determined, care to patient-centered, self-determined adult care. In patients with spina bifida (SB), transition is particularly relevant from a urological perspective, as neurogenic bladder and bowel dysfunction frequently require chronic pharmacological treatment, permanent bladder management (including intermittent self-catheterization), and lifelong monitoring of kidney and bladder function. At the same time, physical limitations and frequen

Unmasking the cognitive burden of adult tuberculous meningitis: A systematic review and meta analysis.

INTRODUCTION: Tuberculous meningitis (TBM) is a devastating form of extrapulmonary tuberculosis with high mortality and substantial long-term neurological sequelae. Cognitive impairment is a particularly debilitating outcome yet remains under-recognised and understudied. However, heterogeneous methodologies and tools remain a major barrier to pooling data and forming clinical guidelines. AIMS AND OBJECTIVES: To evaluate the prevalence, patterns, and determinants of cognitive impairment among adult TBM patients. METHODOLOGY: A total of 761 records were identified, and six studies met the inclus

Neural correlates of approach-avoidance tendencies toward physical activity and sedentary stimuli: An MRI study.

Automatic tendencies toward physical activity and sedentary stimuli are involved in the regulation of physical activity behavior. However, the brain regions underlying these automatic tendencies remain largely unknown. Here, we used an approach-avoidance task and magnetic resonance imaging (MRI) in 42 healthy young adults to investigate whether cortical and subcortical brain regions underpinning reward processing and executive function are associated with these tendencies. At the behavioral level, results showed more errors when avoiding sedentary stimuli (i.e., avatars in a sitting position)

Alcohol use disorder and body mass index show genetic pleiotropy and shared neural associations.

Despite neurobiological overlap, alcohol use disorder (AUD) and body mass index (BMI) show minimal genetic correlation (rg), possibly due to mixed directions of shared variants. Here we applied MiXeR to investigate shared genetic architecture between AUD and BMI, conjunctional false discovery rate to detect shared loci and their directional effect, local analysis of (co)variant association for local rg, functional mapping and annotation to identify lead single-nucleotide polymorphisms, Genotype-Tissue Expression (GTEx) to examine tissue enrichment and BrainXcan to assess associations with brai

Impact of Renin-Angiotensin-Aldosterone System Modulation on Cognitive and Neuropsychiatric Outcomes: A Systematic Review of Clinical and Mechanistic Evidence.

This systematic review explores the influence of the renin-angiotensin-aldosterone system (RAAS) on cognitive and neuropsychiatric outcomes, synthesizing evidence from clinical and preclinical studies investigating RAAS-modulating therapies such as angiotensin-converting enzyme inhibitors (ACEIs) and angiotensin receptor blockers (ARBs). A comprehensive literature search across four major databases yielded seven studies that met the inclusion criteria, including randomized controlled trials, observational studies, and one animal model experiment. The findings suggest that RAAS modulation, part

Fetal Alcohol Spectrum Disorders.

Prenatal alcohol exposure (PAE) can cause a wide range of physical, neurobehavioral, and neurocognitive impairments that impact developmental trajectories throughout the lifespan. Clinically, individuals who have been exposed to alcohol prenatally and who show physical or neurobehavioral difficulties may be classified as having a condition included in fetal alcohol spectrum disorders (FASDs). The aim of this chapter is to summarize the current knowledge of FASD including diagnostic criteria, neurobehavioral outcomes, lifespan considerations, and interventions. Individuals with PAE exhibit chal

Abnormal Brain Activation Patterns in Patients With Post-Acute Sequelae of COVID-19 (PASC) During Recovery: A fNIRS Study.

COVID-19 has increased the likelihood of cognitive impairment in patients with post-acute sequelae of COVID-19 (PASC). There is a lack of direct evidence regarding the working memory performance of mild patients during the recovery period. This study employed functional near-infrared spectroscopy (fNIRS) to construct a mixed effects model for PASC patients performing the N-back task, assessing brain activation levels and brain connectivity. PASC patients exhibited abnormally low activation in the parietal lobe (β = -0.21) and abnormally high activation in the occipital lobe (β = 0.40). There w

Frontal Disorders and Consciousness: A Review.

PURPOSE OF REVIEW: The frontal cortex plays a key role in many functions considered to be relevant for consciousness. Furthermore, altered consciousness appears to exist in those with disorders that disrupt or damage frontal areas and/or networks. This article reviews recent research discussing the impact of frontal disorders on consciousness. RECENT FINDINGS: Many theories of consciousness have been developed over the last century to help explain the neural correlates of consciousness. Some theories argue that the frontal cortex is necessary for consciousness, while others argue that posteri

Optimizing trajectories of social adaptive competencies after extreme prematurity during the first 1000 days.

Over 75% of surviving extremely preterm infants do not have major neurodevelopmental disabilities; however, more than half face difficulties with communication, coordination, attention, learning, social, and executive function abilities. These "minor" challenges can have a negative impact on educational and social outcomes, resulting in physical, behavioral, and social health problems in adulthood. We will review assessment tools for social-emotional and adaptive functional skills in early childhood as these determine family and early childhood supports. We highlight bronchopulmonary dysplasia
