The early evolving lesions of the hair follicles are described in pseudopelade, a type of cicatricial alopecia where clues for the diagnosis of lupus erythematosus or lichen plano-pilaris are lacking. A sudden and synchronized cell death of all the cells of the epithelial sheaths of the hair follicles occurs and is associated with a dense infiltration by lymphocytes. The epidermis remains uninvolved. This unique aspect is most probably related to the mechanism of apoptosis. The case is described of a 71-year-old woman presenting with acute erosive lichen planus. The lesions appeared one week after starting treatment with clopidogrel (Plavix), a new antiplatelet drug, for the prevention of cerebral thrombosis. Lichenoid eruptions are represented by lichen planus and lichen striatus. They are characterized clinically by a papulous eruption, and histogically by a dermal superficial infiltrate of lymphocytic cells and a cytotoxic reaction directed against basal keratinocytes. This study investigated the distribution and diversity of epiphytes in London in relation to NO(x) using fine-scale atmospheric dispersion modelling. The survey recorded over 3000 epiphytes from 334 trees (Fraxinus excelsior) representing 74 lichen, 14 moss, 7 fungal and 3 algal species. There was a significant inverse relationship between diversity and NO(x). Diversity declined where NO(x) exceeded 70 microg m(-3) and NO2 exceeded 40 microg m(-3), suggesting a phytotoxic effect. The clinical features and differential diagnosis of chronic erosive vulval disease are described.
