Pathophysiology of Hip Disorders in Patients with Mucopolysaccharidosis IVA.

Patients with mucopolysaccharidoses IVA (MPS IVA) have a progressive accumulation of the specific glycosaminoglycans (GAGs): chondroitin-6-sulfate (C6S) and keratan sulfate (KS), leading to the degeneration of the cartilage matrix and its connective tissue perturbing the regular microarchitecture of cartilage and successively distorting bone ossification and growth. Impaired cartilage quality and poor bone mineralization lead to serious hip disorders in MPS IVA patients. Although hip dysplasia is seen widely in musculoskeletal abnormality of this disorder, the pathophysiology of the hip bone a

Quality of life measures predict cardiovascular health and physical performance in chronic renal failure patients.

BACKGROUND: Patients with advanced chronic kidney disease (CKD) experience complex functional and structural changes of the cardiopulmonary and musculoskeletal system. This results in reduced exercise tolerance, quality of life and ultimately premature death. We investigated the relationship between subjective measures of health related quality of life and objective, standardised functional measures for cardiovascular and pulmonary health. METHODS: Between April 2010 and January 2013, 143 CKD stage-5 or CKD5d patients (age 46.0±1.1y, 62.2% male), were recruited prospectively. A control group

The role of facility-based surgical services in addressing the national burden of disease in New Zealand: an index of surgical incidence based on country-specific disease prevalence.

BACKGROUND: Surgery is a crucial component of health systems, yet its actual contribution has been difficult to define. We aimed to link use of national hospital service with national epidemiological surveillance data to describe the use of surgical procedures in the management of a broad spectrum of conditions. METHODS: We compiled Australian Modification-International Classification of Diseases-10 codes from the New Zealand National Minimum Dataset, 2008-11. Using primary cause of admission, we aggregated admissions to 91 hospitals into 119 disease states and 22 disease subcategories of the

Muscle as a molecular machine for protecting joints and bones by absorbing mechanical impacts.

We hypothesize that dissipation of mechanical energy of external impact to absorb mechanical shock is a fundamental function of skeletal muscle in addition to its primary function to convert chemical energy into mechanical energy. In physical systems, the common mechanism for absorbing mechanical shock is achieved with the use of both elastic and viscous elements and we hypothesize that the viscosity of the skeletal muscle is a variable parameter which can be voluntarily controlled by changing the tension of the contracting muscle. We further hypothesize that an ability of muscle to absorb sho

Tumours of Deep Lobe of Parotid Gland: Our Experience.

BACKGROUND: Although it has long been believed that stress has a detrimental effect on health and the risk of disease, little research has been done on the precise mechanisms by which this happens. The literature produced by past workers suggests many possibly intersecting mechanistic pathways that might be useful for future fundamental and clinical study. Exercise has been thoroughly researched as a non-pharmacologic strategy to increase bone mass. Many evidence-based treatment recommendations for patients suffering from osteoporosis-related fractures, potential courses of care, and rehabilit

Relationship between preoperative expectations, satisfaction, and functional outcomes in patients undergoing lumbar and cervical spine surgery: a multicenter study.

STUDY DESIGN: Analysis of prospectively collected multicenter data. OBJECTIVE: To explore the relationship between preoperative expectations and postoperative outcomes and satisfaction in lumbar and cervical spine surgery. SUMMARY OF BACKGROUND DATA: Back pain is one of the most common health problems, leading to the utilization of health care resources, work loss, and sick benefits. Patient expectations influence posttreatment outcomes, both surgical and nonsurgical. There is little research on the importance of preoperative expectations in spine surgery. Existing studies evaluate the techn

Racehorse injuries, clinical problems and fatalities recorded on British racecourses from flat racing and National Hunt racing during 1996, 1997 and 1998.

For improvements to the safety and welfare of racehorses to be possible, it is essential to have access to basic descriptive information about the veterinary incidents encountered during horseracing. A 3 year surveillance study (1996-1998) was conducted by The Jockey Club into racing injuries, other postrace clinical problems and fatalities from all 59 British racecourses (mainland Britain only) to identify risk factors. During the survey there were 222,993 racing starts: 106,897 starts in flat races on turf (47.9%), 26,519 starts in flat races on all-weather surfaces (11.9%), 30,932 starts in

The role of inflammatory mediators and matrix metalloproteinases (MMPs) in the progression of osteoarthritis.

Osteoarthritis (OA) is a chronic musculoskeletal disorder characterized by an imbalance between (synthesis) and catabolism (degradation) in altered homeostasis of articular cartilage mediated primarily by the innate immune system. OA degenerates the joints resulting in synovial hyperplasia, degradation of articular cartilage with damage of the structural and functional integrity of the cartilage extracellular matrix, subchondral sclerosis, osteophyte formation, and is characterized by chronic pain, stiffness, and loss of function. Inflammation triggered by factors like biomechanical stress is

[Injuries in Karate Sports: A Survey Performed During the World Championship 2014].

Background: In literature, the competitive sport of modern karate is almost always characterised as a combat sport involving injuries caused by impact effects and physical contact with opponents. There is a lack of data regarding the outcome after karate injuries, specifically with a view to the contact-free Kata karate. Methods: Performing a random test using a questionnaire, we collected data concerning regular medical treatment, prior surgeries of the locomotor system, and medical care. This study included 300 athletes from 65 countries (average age: 24.1 years; 176 male, 124 female) partic

Are Complications Associated With the Repiphysis(®) Expandable Distal Femoral Prosthesis Acceptable for Its Continued Use?

BACKGROUND: Reconstruction of the distal femur after resection for malignant bone tumors in skeletally immature children is challenging. The use of megaprostheses has become increasingly popular in this patient group since the introduction of custom-made, expandable devices that do not require surgery for lengthening, such as the Repiphysis(®) Limb Salvage System. Early reports on the device were positive but more recently, a high complication rate and associated bone loss have been reported. QUESTIONS/PURPOSES: We asked: (1) what are the clinical outcomes using the Musculoskeletal Tumor Soci

Safety evaluation of subcutaneous and intravenous administration of infliximab: a real-world study based on the FAERS database.

BACKGROUND: Inflammatory Bowel Disease (IBD) is characterized by recurrent flare-ups, significantly impairing patients' long-term quality of life and increasing the risk of complications. As the first chimeric monoclonal antibody targeting tumor necrosis factor-alpha (TNF-α), infliximab blocks TNF-α-mediated inflammatory cascades and has become an important therapeutic agent for moderate-to-severe IBD. Although clinical trials have highlighted its safety and efficacy, real-world data on adverse events (AEs) associated with different administration routes remains limited. This study aims to ana

Emergency physicians' perspectives on integrating physiotherapists into emergency departments: a national survey from France.

BACKGROUND: Emergency departments (EDs) worldwide are facing increasing patient volumes and thus crowding, prolonged waiting times and rising healthcare costs. In France, ED visits rose from 8.5 to 16 million between 2014 and 2024. To address these challenges, several countries (including Australia, the UK and Canada) have integrated physiotherapists into EDs in advanced practice roles-primarily for musculoskeletal (MSK) conditions. In contrast, this type of integration is rare in the French healthcare system. The objective of the present study was to assess the attitudes of French emergency p

A matter of definition : variation in treatment success of fracture-related infections.

AIMS: The establishment of the fracture-related infection (FRI) consensus definition and the FRI classification system provide a foundation for research and the development of clinical recommendations. To date, however, the evaluation of treatment success is still impeded by a lack of clear and standardized outcome definitions. We hypothesized that applying different outcome measurements to the same cohort of FRI patients results in significantly different treatment success rates. METHODS: Adult patients undergoing curative surgery for FRI at the Center of Musculoskeletal Infections, Universi
