Natural course of Elschnig pearl formation and disappearance.

PURPOSE: To observe and analyze the daily and weekly changes in morphology and size of Elschnig pearls in eyes with after-cataract. METHODS: Pseudophakic eyes with pronounced regeneratory posterior capsule opacification (PCO) were included in this prospective study. High-resolution retroillumination images were taken, and the changes in size and shape of Elschnig pearls were analyzed at baseline and at 2, 7, and 14 days later using dedicated imaging software. RESULTS: In total, 6309 Elschnig pearls in 85 eyes of 77 patients were analyzed. On average, four pearls were found per square millime

Posterior chamber intraocular lenses: long-term results in pediatric cataract patients.

Major problems in pediatric cataract patients include noncompliance with contact lenses resulting in amblyopia. Advances in intraocular cataract surgery have provided a better environment to perform intraocular lens (IOL) implantation in children. We prospectively analyzed the results of 34 consecutive pediatric patients who underwent cataract removal and insertion of an IOL. Operative technique performed was posterior scleral beveled or frown incision and IOL endocapsular fixation. Subgroups included 10 eyes with traumatic cataract, and 24 eyes with developmental cataracts. Six patients had b

Case Report: Resolution of negative dysphotopsia after capsulorhexis revision in an extended depth of focus intraocular lens.

Negative dysphotopsia (ND) is a known cause of patient dissatisfaction following cataract surgery with intraocular lens (IOL) implantation. While IOL explantation is often considered in persistent cases, identifying reversible anatomical factors is essential to minimize surgical risk. Capsulorhexis size and configuration have been implicated in the pathogenesis of ND but remain underrecognized as a direct therapeutic target. A 73-year-old woman underwent uncomplicated phacoemulsification with implantation of an extended depth of focus (EDOF) IOL in the left eye. Postoperatively, she experience

Bag-in-the-lens intraocular lens implantation before 12 weeks of age in congenital cataract.

PURPOSE: The aim of this study was to evaluate the intra- and postoperative complications, especially the risk of visual axis opacification (VAO), following the implantation of the bag-in-the-lens (BIL) intraocular lens (IOL) in infants with congenital cataract operated on before 12 weeks of age. METHODS: Infants operated on before 12 weeks of age between June 2020 and June 2021 with a follow-up longer than 1 year were included in the present retrospective study. This was a first-experience cohort of an experienced paediatric cataract surgeon with this type of lens. RESULTS: Nine infants (13

Senescence-Driven IL-17A Inflammatory Circuit Promotes Epithelial-Mesenchymal Transition (EMT) and Progression in Age-Related Posterior Subcapsular Cataracts.

Cataract is a leading cause of visual impairment worldwide, and its prevalence is increasing with population aging. The majority are age-related cataracts (ARC), clinically classified into nuclear, cortical, and posterior subcapsular cataracts (PSC) by opacity location. Mechanisms of cataractogenesis remain incompletely understood. While cortical and nuclear cataracts are largely attributed to crystallin aggregation, such protein-centric mechanisms fail to explain the early onset and axial location of PSC. Morphologically, PSC resembles posterior capsule opacification (PCO), a secondary catara

Advances in loading-release-repair strategies of hydrogels for anterior segment disease therapy.

Anterior segment (AS) diseases are a major group of ocular disorders that impair ocular surface homeostasis, optical transparency, aqueous humor circulation, and postoperative visual quality, including Dry Eye Disease (DED), corneal injury, Infectious Keratitis (IK), uveitis, glaucoma, and Posterior Capsule Opacification (PCO). The therapeutic goal of these diseases extends beyond symptom relief toward the establishment of integrated treatment systems capable of overcoming complex ocular barriers and actively regulating the pathological immune microenvironment and inflammatory cascades through

GSH Levels Serve As a Biological Redox Switch Regulating Sulforaphane-Induced Cell Fate in Human Lens Cells.

PURPOSE: Sulforaphane (SFN) is a therapeutic phytochemical agent for many health conditions. SFN-induced cytotoxicity is shown to have promise in preventing posterior capsule opacification (PCO). In the current study, we aimed to elucidate key processes and mechanisms linking SFN treatment to lens cell death. METHODS: The human lens epithelial cell line FHL124 and central anterior epithelium were used as experimental models. Cell death was assessed by microscopic observation and cell damage/viability assays. Gene or protein levels were assessed by TaqMan RT-PCR or immunoblotting. Mitochondria

PURPOSE: To compare the effect of intravitreal and orbital floor triamcinolone acetonide (TA) on macular edema, visual outcome, and course of postoperative inflammation after cataract surgery in uveitis patients. DESIGN: Prospective, randomized clinical trial. METHODS: Monocenter study (40 patients) with chronic endogenous uveitis who underwent phacoemulsification with intraocular lens implantation with either 4 mg intravitreal TA (n = 20) or 40 mg orbital floor TA (n = 20). Phacoemulsification cataract extraction and posterior chamber lens implantation in patients with uveitis.

PURPOSE: This study reports outcomes of phacoemulsification cataract extraction and posterior chamber intraocular lens implantation within the capsular bag in patients with uveitis. METHODS: We retrospectively reviewed the charts of 32 patients (39 eyes) with uveitis who underwent phacoemulsification cataract extraction and posterior chamber intraocular lens implantation by two surgeons at The Cleveland Clinic Foundation from January 1990 to June 1998. Patients with less than 3 months of follow-up were excluded. RESULTS: Diagnoses of uveitis included idiopathic (15 eyes), sarcoidosis (10 eye

Clear lens extraction and implantation of negative-power posterior chamber intraocular lenses to correct extreme myopia.

PURPOSE: To evaluate the effectiveness, predictability, and safety of clear lens extraction to correct extreme myopia. SETTING: Clinica de Nuestra Señora de la Concepción, Fundación Jiménez Díaz, Madrid, Spain. METHODS: This retrospective study comprised 26 eyes of 17 highly myopic patients who had clear lens extraction and implantation of a negative-power posterior chamber intraocular lens (IOL). The IOL power was calculated using the SRK/T formula. Analyzed were visual and refractive results and intraoperative and postoperative complications. Follow-up was at least 12 months in all cases.

Optical Quality Degradation Following Nd:YAG Laser-Induced Intraocular Lens Pitting: A Multimodal Experimental Study.

Nd laser posterior capsulotomy is the standard treatment for posterior capsule opacification following cataract surgery. Although generally considered safe, inadvertent laser impacts on the intraocular lens (IOL) optic may induce permanent surface defects that contribute to postoperative dysphotopsias and reduced visual quality. This experimental study investigated the optical consequences of Nd laser-induced damage on two commercially available hydrophobic acrylic IOLs, focusing on retinal light distribution and optical image quality. Two hydrophobic acrylic monofocal IOL models, the CT LUCIA
