Association between ventricular volume measures and pulsatile and static intracranial pressure scores in non-communicating hydrocephalus.

BACKGROUND: In non-communicating hydrocephalus (HC), enlarged cerebral ventricles are often thought to reflect increased intracranial pressure (ICP) or increased pulsatile ICP. The present study was undertaken to explore the association between ventricular volume measures and pulsatile or static ICP scores in patients with non-communicating HC. Since linear measures of ventricular size have the most widespread use, we also examined how linear and volume measures of ventricular size compare. METHODS: The patient material includes all patients with non-communicating HC that underwent continuous

A 12 Mb deletion of 6p24.1-->pter in an 18-gestational-week fetus with orofacial clefting, the Dandy-Walker malformation and bilateral multicystic kidneys.

We report an 18-gestational-week fetus with oligohydramnios, orofacial clefting, bilateral multicystic kidneys and the Dandy-Walker malformation. Characteristic craniofacial features include a turricephalic prominent forehead, hypertelorism, low-set ears, a flat nasal bridge, mid-face hypoplasia, bilateral cleft lip and palate, and a thick nuchal fold. Array-comparative genomic hybridization (CGH) analysis demonstrated a 12Mb deletion of 6p24.1-->pter.

Complications of fourth-ventricular shunts.

Fourth-ventricular shunting is commonly used to treat symptomatic posterior fossa cysts of the Dandy-Walker malformation and trapped fourth ventricle. Although the benefits of this procedure have been widely reported, there is a paucity of data on the pitfalls of posterior fossa shunting in the neurosurgical literature. During the 4-year period from July 1989 to June 1993, we placed fourth-ventricular shunts in 12 patients. Remarkably, 5 patients suffered complications related to posterior fossa catheter placement (42% rate). Three of these patients developed new cranial nerve dysfunction caus

[Role of diagnostic imaging in Rubinstein-Taybi syndrome. Personal experience with 8 cases].

Both etiology and pathogenesis of Rubinstein-Taybi syndrome (RTS) are still questionable, even though a genetic factor seems to be certain. A typical face, psychomotor delay, and thumb and halluces abnormalities (big, prevalently short, and often "spoon-like" toes) are the main characteristic patterns of RTS. Eight subjects (4 male and 3 female children aged 26 days-7 years, and a 31-year-old woman, mother of 1 of the affected children) with different signs of RTS were studied over the last 3 years. The results are here reported, with a special emphasis on malformations detected with conventio

Management of Dandy-Walker complex-associated infant hydrocephalus by combined endoscopic third ventriculostomy and choroid plexus cauterization.

OBJECT: Dandy-Walker complex (DWC) is a continuum of congenital anomalies comprising Dandy-Walker malformation (DWM), Dandy-Walker variant (DWV), Blake pouch cyst, and mega cisterna magna (MCM). Hydrocephalus is variably associated with each of these, and DWC-associated hydrocephalus has mostly been treated by shunting, often with 2-compartment shunting. There are few reports of management by endoscopic third ventriculostomy (ETV). This study is the largest series of DWC or DWM-associated hydrocephalus treated by ETV, and the first report of treatment by combined ETV and choroid plexus cauteri

Phenotypical variation in cousins with the identical partial trisomy 9 (pter-q22.2) and 7 (q35-qter) at 16 and 23 weeks gestation.

From the study of numerical and structural chromosomal abnormalities, there is convincing evidence and accumulating information of a direct karyotype to phenotype correlation. Knowledge of phenotypic consequences of a specific chromosomal imbalance is important for genetic counseling and prenatal diagnosis. However, for unbalanced non-Robertsonian translocations a precise karyotype to phenotype correlation is difficult to predict for several reasons: (I) unbalanced non-Robertsonian translocations are rare, (II) the published case reports are often not age-matched, (III) varying breakpoints res

[Disorders of neuronal migration: clinical and radiological signs in 21 patients].

We describe 21 patients affected by neuronal migration disorders. The main clinical manifestations were epilepsy, hemiparesis with hemiatrophy and psychomotor retardation. The neuronal migration disorders most frequently diagnosed were various forms of heterotopia and schizencephaly. Magnetic resonance imaging was more sensitive and specific that computed tomography in the diagnosis of these disorders. Schizencephaly correlates well with hemiparesis and hemiatrophy, as does nodular heterotopia with focal epilepsy and diffuse neuronal migration disorders with severe encephalopathies.

Abnormalities of the neonatal cerebral ventricles.

High-resolution ultrasonography allows accurate assessment of ventricular size and contour in the infant, so that conditions such as hydrocephalus, holoprosencephaly, and agenesis of the corpus callosum can be diagnosed. Ventricular shunt catheter placement can be guided, and some of the complications of cerebrospinal fluid shunt systems can be identified. Infection of the ventricular fluid causes striking findings, including increased echogenicity and septation within the lumen.

Early prenatal diagnosis of hydrocephalus.

The aim of the present study was to assess the accuracy of various measurements of the lateral ventricles (LVs) for the early prenatal diagnosis of evolving congenital hydrocephalus. A prospective study was conducted among 61 normal pregnancies, and 20 pregnancies complicated by neural axis malformations, between 19 and 24 weeks' gestation. The following ultrasound measurements were obtained: atrial width (AW); atrial-width/cerebro-atrial distance (CAD); left ventricular width/hemispheric distance, and cerebro-frontal horn/hemispheric width (HW) distance. Atrial width or AW/CAD had the highest

Intraventricular vancomycin-induced cerebrospinal fluid eosinophilia: report of two patients.

Pediatric neurosurgeons commonly instill vancomycin into the ventricles to treat shunt infections. This use for vancomycin, however, has not been studied in the laboratory to evaluate possible toxicities and side effects. We report two cases of cerebrospinal fluid eosinophilia (CSFE) secondary to the intraventricular administration of vancomycin. Two other cases of shunt infection during the same time period were treated for only 2 days with intraventricular vancomycin and did not manifest CSFE. We address the clinical problems and possible detrimental effects of CSFE in the setting of shunt i

Relation of age and cerebral ventricle size to central canal in man. Morphological analysis.

The central canal of the spinal cord in man with and without hydrocephalus was studied histologically. The lumen was patent in most patients in the first two decades of life. Cells lining the canal in the prenatal and newborn state and in the first decade of life were predominantly pseudostratified ciliated epithelium. In the second decade, the epithelium became simple columnar or cuboidal. The central canal closed in most cases after the age of 20 years, secondary to proliferation of ependymal cells and astrocytes. Mechanisms whereby the number of glial cells increase are considered. The cana

Congenital Melanocytic Nevus Syndrome: A Case Series.

Congenital melanocytic nevus syndrome (CMNS) is the result of an abnormal proliferation of melanocytes in the skin and central nervous system caused by progenitor-cell mutations during embryonic development. Mutations in the NRAS gene have been detected in many of these cells. We present 5 cases of giant congenital melanocytic nevus, 3 of them associated with CMNS; NRAS gene mutation was studied in these 3 patients. Until a few years ago, surgery was the treatment of choice, but the results have proved unsatisfactory because aggressive interventions do not improve cosmetic appearance and only

PHACES syndrome: a review of eight previously unreported cases with late arterial occlusions.

PHACE and PHACES are acronyms for a syndrome of variable expression comprising posterior cranial fossa malformations, facial haemangiomas, arterial anomalies, aortic coarctation and other cardiac disorders, ocular abnormalities and stenotic arterial disease. We review five girls and three boys aged 1 month-14 years with disorders from this spectrum. Six had large facial haemangiomas but recent reports suggest that small haemangiomas may occur; hence our inclusion of two possible cases. We also focus on the recently recognised feature of progressive intracranial arterial occlusions, present in

Prenatal diagnosis of aortopulmonary window associated with aberrant subclavian artery.

Aortopulmonary window is a rare cardiac developmental anomaly characterised by a communication between the ascending aorta and the pulmonary artery. Aortopulmonary window may be isolated or associated with cardiac defects such as ventricular septal defect, atrial septal defect, interrupted aortic arch, and tetralogy of Fallot. We report a case of aortopulmonary window associated with aberrant subclavian artery based on fetal two-dimensional echocardiogram. The mother was referred for fetal echocardiography because of multiple fetal anomalies. Prenatal echocardiography at 30 weeks of gestation

A deletion in the VLDLR gene in Eurasier dogs with cerebellar hypoplasia resembling a Dandy-Walker-like malformation (DWLM).

Dandy-Walker-like malformation (DWLM) is the result of aberrant brain development and mainly characterized by cerebellar hypoplasia. DWLM affected dogs display a non-progressive cerebellar ataxia. Several DWLM cases were recently observed in the Eurasier dog breed, which strongly suggested a monogenic autosomal recessive inheritance in this breed. We performed a genome-wide association study (GWAS) with 9 cases and 11 controls and found the best association of DWLM with markers on chromosome 1. Subsequent homozygosity mapping confirmed that all 9 cases were homozygous for a shared haplotype in
