Gastric emptying for liquids and solids after distal gastrectomy with Billroth-I reconstruction.

BACKGROUND/AIMS: Although distal gastrectomy followed by Billroth-I reconstruction has been a standard surgical procedure for over 100 years, gastric emptying of liquid and solids after this procedure remains poorly understood, despite its contribution to postgastrectomy complications such as dumping syndrome and reflux esophagitis. A simple way to standardize measurement and generate normal values is needed. This study proposed a new, accurate and easy method, "dual phase method", for gastric emptying after distal gastrectomy. METHODOLOGY: Liquid- and solid-phase gastric emptying were measur

Percutaneous transhepatic insertion of metal stents with a double-pigtail catheter in afferent loop obstruction following distal gastrectomy.

We report successful outcome following transhepatic insertion of metal stents with a double-pigtail catheter in a patient with afferent loop syndrome caused by recurrent gastric carcinoma. A 77-year-old man was admitted with a 2-week history of fever, right upper quadrant pain, and jaundice. His past medical history included distal gastrectomy for treatment of gastric cancer two years previously. Abdominal computed tomography revealed marked dilation of the jejunal limb and intrahepatic bile duct. We diagnosed the patient with afferent loop syndrome resulting from recurrent cancer. Surgical palliation in pancreatic head carcinoma and gastric cancer: the role of laparoscopy.

BACKGROUND/AIMS: In all patients with pancreatic and gastric cancer we always make a laparoscopic exploration to complete the staging. Lately we have adopted the following technique for nonresectable cancers of the head of the pancreas: following endoscopic retrograde cholangiography we position a biliary stent to restore bile flow and obtain regression of jaundice, a laparoscopic-assisted gastroentero-anastomosis (GEA) is then performed as an antecolic isoperistaltic side-to-side gastrojejunostomy. Also in case of nonresectable gastric cancer we perform a laparoscopic-assisted gastrojejunosto

Primary carcinoma of the gallbladder.

One-hundred-and-seventeen patients with histologically proven carcinoma of the gallbladder are reviewed. The majority of the patients presented with advanced disease with extension and metastasis to the liver and to lymph nodes along the common bile duct. Four patients had localized disease recognized at the time of operation; two were treated with cholecystectomy and two with cholecystectomy and hepatic resection. There were no five-year survivals among these patients. Fourteen patients had clinically inapparent carcinoma at the time of cholecystectomy; the diagnosis being established postope

Surgical experience with pancreatic and periampullary cancer.

Between 1940 and 1978, 179 patients underwent pancreatic resection (64 total, 102 Whipple, 13 distal) at the Presbyterian Hospital, predominantly for carcinoma of the pancreas and periampullary area. With respect to operative morbidity and mortality and survival, these patients have been compared with 141 patients subjected to pancreatic biopsy only, and with 172 by-passed for palliation. Likewise, total pancreatectomy has been compared to pancreaticoduodenectomy (Whipple) in terms of safety and efficacy. The overall major postoperative complication rate for pancreatic resection was 36%, in co

Evaluation of the efficacy and safety of the augmented rectangle technique for Billroth-I reconstruction in laparoscopic distal gastrectomy for gastric cancer: comparison with Roux-en-Y reconstruction.

BACKGROUND: Laparoscopic distal gastrectomy confers a large rectangular anastomotic stoma, which can be repaired with an augmented rectangle technique anastomosis in an intracorporeal Billroth-I reconstruction. We aimed to evaluate the safety and feasibility of the augmented rectangle technique (ART) for anastomosis. METHODS: We retrospectively compared the surgical outcomes and postoperative nutritional statuses of patients who received Billroth-I (n = 285; ART group) or Roux-en-Y (n = 72; R-Y group) reconstruction between January 2013 and December 2023 following laparoscopic distal gastrect

Long-term functional outcomes of Roux-en-Y versus Billroth I reconstructions after laparoscopic distal gastrectomy for gastric cancer: a propensity-score matching analysis.

BACKGROUND: Only a few reports discuss long-term functional outcomes and changes observed in patients over several years following a distal gastrectomy. We investigated long-term functional outcomes and changes after laparoscopic distal gastrectomy based on the analysis of postoperative conditions and endoscopic findings observed in patients. METHODS: The study group comprised 159 patients who underwent Roux-en-Y reconstruction following laparoscopic distal gastrectomy (R-Y group) between December 2008 and November 2012 and 78 patients who underwent Billroth I reconstruction (B-I group) betwe

Dynamic enhanced computed tomography imaging findings of an inflammatory fibroid polyp with massive fibrosis in the stomach.

Inflammatory fibroid polyp (IFP) is a rare benign lesion of the gastrointestinal tract. We report a case of computed tomography (CT) imaging finding of a gastric IFP with massive fibrosis. CT scans showed thickening of submucosal layer with overlying mucosal hyperenhancement in the gastric antrum. The submucosal layer showed increased enhancement on delayed phase imaging. An antrectomy with gastroduodenostomy was performed because gastric cancer was suspected, particularly signet ring cell carcinoma. The histopathological diagnosis was an IFP with massive fibrosis. The authors suggest that whe

Efficacy and safety of endoscopic retrograde cholangiopancreatography in patients with Billroth II anastomosis.

BACKGROUND/AIMS: Endoscopic retrograde cholangiopancreatography (ERCP) procedure is quite difficult to perform in patients with Billroth II anastomosis because of anatomical alterations. The aim of this study is to evaluate retrospectively the results of ERCP applications done in patients with Billroth II operation. METHODOLOGY: Out of the 1632 patients who underwent ERCP between 1992 and 2002, 27 (1.65%) had Billroth II operation. The records of these 27 patients were reviewed. Details noted included indications for ERCP, therapeutic interventions, causes of failure and complications. RESUL

An analysis of when patients eat after gastrojejunostomy.

Eighty-five patients who underwent a gastrojejunostomy either alone or in conjunction with vagotomy or gastric resection were evaluated for the day of beginning an oral intake adequate enough to sustain nonoliguric output. This occurred on the average by the seventh to eighth postoperative day, by which time at least 78% of patients were consuming an adequate oral intake. Adequate liquid intake occurred on the average 5.3 days in the absence of vagotomy and 8.8 days in the presence of vagotomy. However, this significant difference is accounted for by the patients who had gastrojejunostomy with

The Feasibility of Conventional Forward-viewing Endoscope for Endoscopic Retrograde Cholangiopancreatography in Patients With Altered Gastrointestinal Anatomy.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) has been commonly used for biliopancreatic diseases of patients with normal gastrointestinal (GI) anatomy with a success rate of >90%. However, this procedure may also be necessary in patients with altered GI anatomy such as Billroth II or Roux-en-Y reconstructions. Performing ERCP in these patients may pose extreme technical challenges, and increase the risk of complications. The aim of this study was to analyze the feasibility of ERCP using forward-viewing endoscopy in patients with altered GI anatomy. MATERIALS AND METHODS:

Durable Flap-Valve Mitigation of Duodenogastric Reflux,  Remnant Gastritis and Dumping Syndrome Following Billroth I Reconstruction.

BACKGROUND: We have reported the short-term results of pylorus reconstruction gastrectomy (PRG) that prevents duodenogastric reflux (DGR) and remnant gastritis after distal gastrectomy. We herein report the long-term results of the PRG. PATIENTS AND METHODS: PRG was performed in 37 patients (age 31 to 86 [mean 67.8 ± 12.3] years, male:female = 22:15) with gastric cancer from June 2006 through December 2013. We examined the long-term outcome in 28 patients (age 41 to 86 [mean 67.0 ± 10.7] years, male:female = 18:10) that passed over 3 years after surgery (LTR 44.1 ± 11.7 months), and compared
