Management of Pediatric Urolithiasis Using a Combination of Laparoscopic Lithotomy and Pyeloscopy.

BACKGROUND: Minimally invasive procedures have become the standard option for treatment of urinary stones; the use of more invasive techniques is not a common practice currently. This emergent technology is not always available and laparoscopy is the alternative option. There are few reports of laparoscopic management for urinary lithiasis in pediatric patients. This study is about the use of laparoscopic surgery combined with pyeloscopy as a feasible first-line treatment for pediatric urolithiasis in renal and proximal ureteral locations. PATIENTS AND METHODS: We reviewed the records of 14 p

A Rare Cause of Gastric Outlet Obstruction: A Case Report of Bouveret Syndrome.

Bouveret syndrome (BS) is a rare phenomenon of gallstone ileus causing gastric outlet obstruction that can present significant challenges in management, given its lack of formal treatment guidelines and endoscopic difficulty. We present a case of an 84-year-old woman with vomiting and abdominal pain who was found to have a large gallstone in the duodenum due to a cholecystoduodenal fistula, as well as findings consistent with acute cholecystitis. She was treated with antibiotics, and gastroenterology and general surgery were consulted. Endoscopy was performed due to high suspicion for BS, reve

Inverted Y ureteral duplication with an ectopic ureter and multiple urinary calculi: A case report.

BACKGROUND: In the clinical treatment of diseases related to ureteral duplication, it is very important to make a clear diagnosis before surgery because different types of ureteral duplication correspond to different treatment options. Inverted Y ureteral duplication with ectopic ureters and multiple urinary calculi is clinically rare. This case can help clinicians increase their understanding of this disease and gain some experience in its diagnosis and treatment. CASE SUMMARY: A 36-year-old male who was previously healthy presented to the hospital with lumbar pain. Percussion of the right k

Predictors and associations of complications in ureteroscopy for stone disease using AI: outcomes from the FLEXOR registry.

We aimed to develop machine learning(ML) algorithms to evaluate complications of flexible ureteroscopy and laser lithotripsy(fURSL), providing a valid predictive model. 15 ML algorithms were trained on a large number fURSL data from > 6500 patients from the international FLEXOR database. fURSL complications included pelvicalyceal system(PCS) bleeding, ureteric/PCS injury, fever and sepsis. Pre-treatment characteristics served as input for ML training and testing. Correlation and logistic regression analysis were carried out by a multi-task neural network, while explainable AI was used for the

FIRE Stones: impact of forced diuresis on the residual fragment rate after flexible ureteroscopy for destruction of kidney stones with laser-protocol for a randomized controlled two-parallel group multicenter trial with blinding evaluation.

BACKGROUND: Lithiasis is a common and recurrent disease. Flexible ureteroscopy (fURS) is the cornerstone of laser treatment of kidney stones. Kidney stones destruction requires its laser pulverization into small fragments in order to remove them through the ureter or improve their spontaneous expulsion along the urinary tract. However, most of the time, all the micro-fragments and dust created cannot be extracted using our surgical tools and may stay intra-renally at the end of the procedure. Adjuvant treatments (such as forced diuresis, inversion or mechanical pressure) were previously descri

Ureteroscopy and laser stone fragmentation (URSL) for large (≥1 cm) paediatric stones: Outcomes from a university teaching hospital.

INTRODUCTION: The treatment of large renal stones (≥1 cm) in paediatric patients is challenging. The usual treatment options include percutaneous nephrolithotomy (PCNL) or shockwave lithotripsy (SWL). There is a rise in the use of ureteroscopy and laser stone fragmentation (URSL) for paediatric stones; however, outcomes of treatment for large renal stones in this age group are still largely unknown. OBJECTIVE: To investigate the outcome of URSL for large paediatric renal stones ≥1 cm treated at the present institution over the last 5 years. METHODS: A retrospective review was performed of ou

Cholangioscopy-guided ERCP: expanding diagnostic and therapeutic applications.

Endoscopic retrograde cholangiopancreatography (ERCP) remains a cornerstone technique for the management of pancreaticobiliary diseases, but conventional ERCP relies largely on fluoroscopic images and cannot directly evaluate intraductal mucosal abnormalities. Cholangioscopy has expanded the diagnostic and therapeutic scope of ERCP by enabling direct visualization of the biliary tree, targeted tissue acquisition, and visual control of intraductal interventions. The technology has evolved from early dual-operator mother-baby systems to single-operator cholangioscopy, high-resolution digital dis

Delayed Ureteral Obstruction Due to Intramural Endo-Endoscopic Gastrointestinal Anastomosis Staples Following Laparoscopic Sigmoid Resection: A Case Report.

Ureteral injuries are among the most significant urological complications associated with colorectal surgery. Although most iatrogenic ureteral injuries are detected intraoperatively or shortly after surgery, a considerable number are identified only later. Very delayed manifestations that occur months or even years after the initial procedure are uncommon and pose diagnostic challenges. We report a case of a 50-year-old female patient who presented with intermittent abdominal discomfort and was found to have a left-sided mid-ureteral stone with grade I hydronephrosis. Her surgical history inc
