Bifunctional sculpturing of the bone graft for 3-dimensional augmentation of the atrophic posterior mandible.

Short implant procedures may suffer from fixture instability, if incorrectly planned and performed, and from an esthetically compromised rehabilitation, due to increased interarch distance and increased crown-implant ratio. Several procedures have been proposed to achieve alveolar ridge augmentation with different success and complication rates: onlay grafts, alveolar sandwich osteotomies, titanium micromesh, alveolar distraction, and alveolar nerve transposition. The edentulous posterior mandible suffers from the presence of a compact cortical layer, which tends to limit graft osteogenesis, b

Compression plates in the treatment of advanced anterior floor of mouth carcinoma.

Advanced anterior floor of mouth squamous cell carcinoma has been traditionally treated with wide excision in conjunction with mandibulectomy and radical neck dissection. This has resulted in significant mandibulofacial defects with functional and cosmetic significance. Efforts at primary reconstruction in the past using bone grafts, osteomyocutaneous flaps, and other methods have yielded unsatisfactory results. A history of prior irradiation has made this problem even more refractory. We present a series of 11 patients who underwent composite resection for advanced anterior floor of mouth car

Why detailed modeling matters in the FE-based pre-clinical evaluation of temporomandibular joint implants.

End-stage temporomandibular joint (TMJ) disorders often necessitate joint replacement to restore bilateral mastication. Patient-specific TMJ implants typically rely on detailed in silico modeling derived from subject-specific computed tomography (CT) scans of the mandible. However, generating highly detailed finite element (FE) models is computationally expensive. Although structural simplifications of the mandible are known to influence mechanical behavior, their quantitative impact on TMJ implant evaluation has not been systematically investigated. Our study examined the influence of three m

Tissue Engineering for Vertical Ridge Reconstruction.

This article provides an overview of basic tissue engineering principles as they are applied to vertical ridge defects and reconstructive techniques for these types of deficiencies. Presented are multiple clinical cases ranging from office-based dentoalveolar procedures to the more complex reconstruction of postresection mandibular defects. Several different types of regenerative tissue constructs are presented; either used alone or in combination with traditional reconstructive techniques and procedures, such as maxillary sinus augmentation, Le Fort I osteotomy, and microvascular free tissue

Histologic and Histometric Analysis of Bone Repair at the Site of Mandibular Body Osteotomy and at the Bone-Screw Interface After Using a Biodegradable 2.0-mm Internal Fixation System.

The aim of the study was to evaluate histologically and histometrically the bone repair at the mandibular body osteotomy and at the bone-screw interface after using a biodegradable 2.0-mm internal fixation system. Six dogs were subjected to an osteotomy in the mandibular body, which was stabilized by applying a fixation device manufactured with poly-L-DL-lactic acid (70:30). The dogs were euthanized at 2 and 18 weeks. Each screw was sectioned along its long axis, and the osteotomy sites were divided into 3 parts: the upper part was labeled the tension third (TT); the lower part, compression th

Management of massive osteolysis of the mandible: a case report.

The management of a previously reported case of massive osteolysis of the mandible with intravenous bisphosphonate, vascularized free bone graft, and dental implants is reported. Restoration of adequate cosmetics and masticatory function was achieved. Despite the use of bisphosphonates, there were no complications with osseointegration. The reconstruction remained stable and functional 6 years afterward.

Restoration of the atrophied posterior mandible with transverse alveolar maxillary/mandibular implants: technical note and case report.

Restoration of the atrophied posterior mandibular alveolus has been a surgical challenge in the past. Many treatment options have been published, each with unique shortcomings. This study will review and compare these techniques to a new type of implant, the transverse alveolar maxillary/mandibular implant (TAMMI). Using modified Nobel Biocare Brånemark System zygomatic implants that were shortened to 11.5, 13, 15, 18, 20, 22.5, or 25 mm, the authors reconstructed atrophied posterior mandibles. These TAMMIs were placed at a 45-degree angle, engaging both the crest of the ridge and the buccal c

Closure of mucosal defects over exposed mandibular plates using fibrin glue.

BACKGROUND: Mandibular plates may become exposed following radiation therapy, infection, and mucosal necrosis. This may lead to early removal of the mandibular plates with subsequent instability of bone fragments. OBJECTIVE: To compare fibrin glue, a bioadhesive, with traditional sutures in closing mucosa over exposed mandibular plates in a cat. DESIGN: Prospective matched-pairs analysis. SUBJECTS: Nine cats were used, and each cat served as its own control. INTERVENTION: Bilateral mandibular plates were fixed on the buccal side of the mandible of 9 cats. The surgical defects over the plat

[New aspects of the reconstruction of large defects of the oral mucosa with microvascularly anastomosed small intestinal grafts].

Our experience in reconstruction of extensive defects in the oral mucosa with free jejunal grafts in 15 patients is reported. The jejunum provides a well vascularised mucosal lining, which is resistant to mechanical stress and being a thin flap contours well in the mouth. Methods of avoiding technical failure are described. The technique permits radical resection of oral tumours.

A Three-Dimensional Finite Element Analysis of Stress Distribution in Cement-Retained and Screw-Retained Implant-Supported Restorations in Mandible Using Porcelain-Fused-to-Metal and Zirconia Crowns.

Introduction Dental implants are widely used to replace missing teeth; however, the biomechanical performance of implant-supported restorations may be influenced by the prosthesis retention type, restorative material, and direction of occlusal loading. Unfavorable stress distribution may lead to biological and mechanical complications, affecting long-term implant success. The present study aimed to evaluate and compare the stress distribution patterns in cement-retained and screw-retained implant-supported restorations fabricated with porcelain-fused-to-metal and zirconia crowns under axial an

Biomechanical evaluation of three full-arch immediate loading protocols in the mandible via finite element analysis: All-on-4, Trefoil, and Five-implant design.

BACKGROUND: Immediate-loading full-arch concepts such as All-on-4, Trefoil, and Five-Implant protocols are widely used for mandibular rehabilitation, yet their comparative biomechanical behavior remains insufficiently defined. This study aimed to biomechanically compare three immediate loading full-arch rehabilitation protocols-Trefoil, All-on-4, and 5-Implant designs-in the edentulous mandible using three-dimensional (3D) finite element analysis (FEA), focusing on stress distribution, strain accumulation, and deformation patterns under functional loading. MATERIALS AND METHODS: A digital ede

Biomechanical behavior of cantilevered 3-unit implant-supported prostheses made of PEKK and monolithic zirconia: A finite element study.

OBJECTIVE: To evaluate the biomechanical performance of various designs of cantilevered three-unit implant-supported prostheses, using two distinct prosthetic materials and under different loading conditions. METHOD: Three mandibular models were created with varying implant positions to support a 3-unit prosthesis using two materials (Zirconia and PEKK), resulting in three different designs: distal cantilever (M1), fixed-fixed (M2), and mesial cantilever (M3). The geometric model was created by segmenting a CBCT scan of an edentulous mandible using Mimics software, followed by refinement in 3

Three-Dimensional Radiological Assessment of Alveolar Bone Volume Preservation Using Bovine Bone Xenograft.

INTRODUCTION: Alveolar bone is critical in supporting natural teeth, dental implants as well as a removable and fixed prosthesis. Alveolar bone volume diminishes when its associated natural tooth is lost. OBJECTIVE: The aim of this study is to evaluate the effectiveness of bovine bone granules on alveolar bone socket augmentation for ridge preservation following atraumatic tooth extraction. MATERIALS AND METHODS: Twenty medically fit patients (12 males and 8 females aged between 18 and 40 years) who needed noncomplicated tooth extraction of 1 mandibular premolar tooth were divided randomly a

Is the burden of oral diseases higher in urban disadvantaged community compared to the national prevalence?

BACKGROUND: The urban low income has often been assumed to have the greatest dental treatment needs compared to the general population. However, no studies have been carried out to verify these assumptions. This study was conducted to assess whether there was any difference between the treatment needs of an urban poor population as compared to the general population in order to design an intervention programme for this community. METHODS: A random sampling of living quarters (households) in the selected areas was done. 586 adults over 19 years old living in these households were clinically ex

Antibiotic spacers of the lower jaw after complex reconstructive surgery: 10-year single-center experience.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: In orthopedic surgery, the use of antibiotic-impregnated spacers is a well-established method for treating periprosthetic joint infections with excellent outcomes. However, their application in head and neck surgery remains poorly documented, despite the high susceptibility of these patients to persistent infections. The objective of this study was to evaluate the 10-year outcomes of antibiotic spacers in the treatment of persistent mandibular infections unresponsive to conventional methods, prior to definitive reconstruction. METHODS: We

Cerebrospinal fluid leak associated with proplast implant removal from the temporomandibular joint.

Severe bone erosion within the temporomandibular joint reconstructed with polytetrafluoroethylene implants is a well-recognized phenomenon. Although erosion into the middle cranial fossa has been reported, we know of no report of dural disruption with cerebrospinal fluid leakage. We report a case of cerebrospinal fluid leak noted at the time of PTFE implant removal and its subsequent management. Perforation of the contralateral glenoid fossa and its reconstruction is also reported.
