Pheochromocytoma Presenting as Spontaneous Adrenal Hemorrhage in a Normotensive Patient: A Case Report.

Background: Pheochromocytoma is a rare tumor of the adrenal medulla, often associated with hypertension, headache, diaphoresis, and palpitations, but presentations vary; some are silent until incidentally found or when complications develop. One rare complication is spontaneous adrenal hemorrhage, previously mainly found at autopsy, which can extend into the retroperitoneum, causing acute pain and mimicking emergencies. Imaging modalities like computed tomography are vital for detection, especially in acute cases, while magnetic resonance imaging provides greater tissue detail to identify tumo

A rare case report of ectopic-adrenal paraganglioma with inferior vena cava infiltration. Case report and literature review.

INTRODUCTION AND IMPORTANCE: Phaeochromocytomas are neuroendocrine neoplasms arising from chromaffin cells of the adrenal medulla and are known as intra-adrenal paragangliomas. On the contrary, paragangliomas are non-epithelial neuroendocrine neoplasms that produce other peptide hormones such as norepinephrine, epinephrine, and dopamine. In a setting with limited resources, such a condition may result in inaccurate diagnosis and treatment, which may lose a patient life if left untreated. In addition, PPGLs present a surgical challenge to many surgeons who are not thoroughly prepared. CASE PRE

Giant Cystic Pheochromocytoma Associated With Neurofibromatosis Type 1: A Case Report.

Pheochromocytomas are tumors that develop from the chromaffin cells of the adrenal medulla. More than 40% of cases of pheochromocytomas are associated with genetic conditions such as neurofibromatosis type 1 (NF1) or von Hippel-Lindau syndrome. Cystic pheochromocytomas are rare, generally asymptomatic, and thus of bigger size at the time of diagnosis. Surgical treatment is necessary to prevent cardiovascular morbidity and malignancy risk. We report the case of a 27-year-old patient admitted for further examination of a left adrenal mass that was discovered by an abdominal CT scan in the contex

Twenty-Four Hour Blood Pressure Response to Empagliflozin and Its Determinants in Normotensive Non-diabetic Subjects.

BACKGROUND: Sodium-glucose co-transport 2 inhibitors (SGLT2i) lower blood pressure (BP) in normotensive subjects and in hypertensive and normotensive diabetic and non-diabetic patients. However, the mechanisms of these BP changes are not fully understood. Therefore, we examined the clinical and biochemical determinants of the BP response to empagliflozin based on 24-h ambulatory BP monitoring. METHODS: In this post-hoc analysis of a double-blind, randomized, placebo-controlled study examining the renal effects of empagliflozin 10 mg vs. placebo in untreated normotensive non-diabetic subjects,

Phaeochromocytomas presenting as acute crises after beta blockade therapy.

OBJECTIVE: Phaeochromocytoma crisis is a life-threatening emergency that may be undiagnosed because of its numerous, nonspecific manifestations. We analysed, retrospectively, the presentation, management and outcome of patients who were admitted to our institution with phaeochromocytoma crises over a 5-year period. RESULTS: Five patients (two males, three females; mean age 34.6 years, range 19-51 years) who presented as emergencies requiring intensive care, with multiple non-specific manifestations and previously undiagnosed pheochromocytoma, were identified. The initial presentation included

Endocrine oncology in pregnancy.

Endocrine tumours occur rarely in pregnant women but present clinicians with unique challenges. A high index of suspicion is often required to make a diagnosis since the symptoms and signs associated with many of these tumours, including insulinoma, adrenocortical carcinoma and phaeochromocytoma, mimic those of normal pregnancy or its complications, such as pre-eclampsia. The evidence base which informs management is very limited hence decisions on investigation and therapy must be individualised and undertaken jointly by the multidisciplinary medical team and the patient. The optimal strategy

Catecholamine-induced metabolic dysfunction and its reversal postsurgery in pheochromocytoma-paraganglioma patients: Insights from the "PheoMet" study.

BACKGROUND: Catecholamine excess in pheochromocytoma-paraganglioma is believed to result in numerous metabolic implications leading to insulin resistance and glucose intolerance. METHODS: PheoMet was a prospective cohort study including 40 pheochromocytoma-paraganglioma patients and 20 age- and sex-matched controls with nonfunctioning adrenal tumors. Patients with comorbidities affecting metabolism were excluded. Metabolic evaluation comprised oral glucose tolerance testing with serial glucose, insulin, and C-peptide measurements preoperatively, early postoperatively (1-2 weeks), and at 6-mon

Metastatic Medullary Thyroid Carcinoma in Multiple Endocrine Neoplasia Type 2B (MEN 2B) With RET M918T Mutation: Challenges in Long-Term Management and Targeted Therapy.

Multiple endocrine neoplasia type IIB (MEN2B) is a rare hereditary cancer syndrome characterized by medullary thyroid carcinoma (MTC), pheochromocytoma, and distinctive mucosal neuromas. MEN2B-associated MTC is most often caused by RET M918T mutations and confers an earlier and more aggressive disease progression. The aggressive nature of RET M918T-mutated MEN2B emphasizes the necessity of vigilant lifelong surveillance. It also highlights the real-world challenges of maintaining continuity of targeted therapy, where treatment interruptions may compromise disease control. This reports the case

Paraganglioma-induced reverse takotsubo syndrome treated with extracorporeal membrane oxygenation in a young patient with a history of malignancy: a case report.

BACKGROUND: Reverse takotsubo-like cardiomyopathy (rTCC) is a rare type of stress-induced cardiomyopathy associated with catecholamine surges. Reverse takotsubo-like cardiomyopathy is characterized by basal and mid-ventricular hypokinesis with apical sparing. Paragangliomas are catecholamine-secreting neuroendocrine tumours outside the adrenal gland that can cause palpitations, hypertension, and rarely cardiomyopathy. In cases of occult paraganglioma, catecholamine-induced rTCC can be rapidly reversed with adequate haemodynamic support. CASE SUMMARY: A 28-year-old woman with a history of cerv

Perception versus reality: A case-matched study assessing the intraoperative hemodynamics of minimally invasive retroperitoneal versus transperitoneal approach to pheochromocytomas.

BACKGROUND: Pheochromocytomas produce excess catecholamines that can result in intraoperative hemodynamic instability. Centers have reported variations in intraoperative hemodynamics with the retroperitoneoscopic versus the laparoscopic transperitoneal approach to adrenalectomies. When the retroperitoneoscopic approach was initiated for pheochromocytomas at our institution, the perception was of improved intraoperative hemodynamics, hypothesizing that increased retroperitoneoscopic insufflation pressures caused decreased venous return and less fluctuation in circulating catecholamines. Catecholamine-Secreting Retroperitoneal Paraganglioma: A Case Report and Literature Review.

INTRODUCTION: Paragangliomas are rare neuroendocrine tumors arising from autonomic paraganglia. These tumors have metastatic potential, and histopathological findings alone cannot reliably predict their biological behavior. Clinical manifestations range from the classic triad of headache, diaphoresis, and palpitations to incidental findings in asymptomatic patients. CASE PRESENTATION: A 75-year-old man was incidentally found to have a 5 cm retroperitoneal mass adjacent to the right renal hilum. Biochemical testing confirmed marked catecholamine hypersecretion, and 18F-FDOPA PET/CT demonstrate
