Neural dynamics of cognitive control: Current tensions and future promise.

Cognitive control is a suite of processes that helps individuals pursue goals despite resistance or uncertainty about what to do. Deficits of cognitive control underlie compulsive or risky behavior, as well as other clinical challenges associated with difficulties in regulating impulses, attention, thoughts, and feelings. Although cognitive control has been extensively studied as a dynamic feedback loop of perception, valuation, and action, it remains incompletely understood as a cohesive dynamic and distributed neural process. Here, we critically examine the history of and advances in the stu

Digital mental health and hidden support: a qualitative analysis of non-suicidal self-injury communities on TikTok.

This study examines the digital representation of Non-Suicidal Self-Injury (NSSI) on TikTok, with particular attention to the emergence of online communities and the communicative strategies users employ to share content while evading platform moderation. As TikTok becomes increasingly influential among adolescents and young adults, understanding how sensitive mental health topics like NSSI circulate on the platform is critical for developing effective digital health interventions. We conducted a qualitative content analysis of 400 posts referencing NSSI, collected using a mixed-method approac

Effects of acceptance and commitment therapy on fatigue interference in patients with advanced lung cancer and caregiving burden: protocol for a pilot randomised controlled trial.

INTRODUCTION: Cancer-related fatigue is common in patients with advanced lung cancer. It not only interferes with patients' health-related quality of life, but also increases the caregiving burden of their caregivers. Acceptance and commitment therapy is emerging as a novel way to advocate accepting negative experiences and taking effective actions based on their own values to help patients commit meaningful actions in the course of cancer diseases. This trial aims to test the feasibility, acceptability and preliminary effects of acceptance and commitment therapy for fatigue interference in pa

Beyond the 'second brain': the gut microbiota as a constitutive co-constructor of embodied cognitive network.

Traditional cognitive science has historically confined the mind within the cranium. While the "second brain" metaphor underscores the autonomy of the enteric nervous system, it remains entrenched in a neurocentric paradigm. Here, we propose a transformative framework: the gut microbiota may function as a constitutively relevant contributor to specific embodied cognitive architectures. We contend that cognition, emotion, and behavior are not fully understandable in brain-isolated terms. Instead, these processes emerge from a sustained, bidirectional dialogue between the host and its symbiotic

Chain of Brain Preservation--a concept to facilitate early identification and initiation of hypothermia to infants at high risk for brain injury.

BACKGROUND: Therapeutic hypothermia has been associated with improved outcomes in term infants particularly in those who present with moderate hypoxic-ischemic encephalopathy (HIE). However, in the three major studies the time to initiate cooling was at approximately 4.5 postnatal hours. OBJECTIVE: To determine in term infants who meet criteria for therapeutic hypothermia whether specific clinical and/or biochemical parameters might identify those high risk infants destined for abnormal neurodevelopmental outcome even sooner than is currently possible. DESIGN/METHODS: Retrospective chart rev

Men's accounts of erectile function over time following radical prostatectomy: A narrative study.

BACKGROUND: Prostate cancer requiring radical prostatectomy surgery, invariably affects many aspects of men's lives. Whilst altered sexual function is well documented as a potential outcome of surgery, the impact on men's lives over time is little understood. OBJECTIVE: This study explored men's accounts of erectile dysfunction over three distinct time points: pre-operatively (n = 13), three months post operatively (n = 10) and six to nine months post operatively (n = 11). DESIGN: Narrative Inquiry. SETTINGS: A large tertiary teaching hospital in Dublin, Ireland. PARTICIPANTS: The first pa

Launching a multi-level data strategy for rare eye diseases: a methodological case study from national to European scale.

Rare eye diseases bring unique challenges in clinical research and patient care due to their heterogeneity, low prevalence, and dispersed expertise. To address these challenges, an integrated multilevel data strategy has been developed in France and Europe, enabling structured, interoperable data collection and reuse across national and transnational initiatives. In this paper, we present a methodological framework using the metaphor of a spaceship to illustrate the vertical articulation of this ecosystem, from national infrastructures to European platforms. At the base of the spaceship lies B

Critical evaluation of internal and external calibration methods for accurate strontium isotope ratio determination.

BACKGROUND: The isotopic composition of strontium n(87Sr)/n(86Sr) provides important information relevant to earth, life, food, archaeological and anthropological sciences. To determine accurate n(87Sr)/n(86Sr) isotope ratios, it is necessary to perform instrumental isotopic fractionation (mass bias) correction, which is a popular metaphor for isotope ratio calibration. Despite the importance of calibration in n(87Sr)/n(86Sr) analysis, experimental studies which critically evaluate the accuracy of conceptually different calibration methods remain scarce. Therefore, this work 1) investigates if

The lived experience of renal cachexia: An interpretive phenomenological analysis.

BACKGROUND: Chronic kidney disease is common, affecting up to 13 % of the global population, and is predicted to become the fifth leading cause of 'life years lost' by 2040. Individuals with end-stage kidney disease commonly develop complications such as protein-energy wasting and cachexia which further worsens their prognosis. The syndrome of 'renal cachexia' is poorly understood, under-diagnosed and even if recognised has limited treatment options. OBJECTIVE: To explore the lived experience of renal cachexia for individuals with end-stage kidney disease and the interrelated experiences of t

Communication competences of multiple sclerosis neurologists during advance care planning conversations: A multi-observer study.

BACKGROUND: The ConCure-SM intervention [ISRCTN48527663] consists of an advance care planning (ACP) training program for neurologists/other professionals caring for people with progressive multiple sclerosis (PwPMS). We assessed the communication competences of ACP-trained neurologists who participated in the trial. METHODS: Eighteen ACP conversations were audio-recorded. After each conversation, participants (PwPMS, significant others [SOs], and neurologists) rated the neurologist's communication skills using dedicated versions of the Quality of Communication questionnaire (QOC). Consultations about randomised controlled trials are shorter and less in-depth for socioeconomically disadvantaged patients compared to socioeconomically advantaged patients: qualitative analysis across three trials.

BACKGROUND: Patients from socioeconomically disadvantaged backgrounds are underserved in randomised controlled trials, yet they experience a much greater burden of disease compared with patients from socioeconomically advantaged areas. It is crucial to make trials more inclusive to ensure that treatments and interventions are safe and effective in real-world contexts. Improving how information about trials is verbally communicated is an unexplored strategy to make trials more inclusive. This study examined how trials are communicated verbally, comparing consultations involving patients from th

Can Artificial Intelligence "Hold" a Dermoscope?-The Evaluation of an Artificial Intelligence Chatbot to Translate the Dermoscopic Language.

This survey represents the first endeavor to assess the clarity of the dermoscopic language by a chatbot, unveiling insights into the interplay between dermatologists and AI systems within the complexity of the dermoscopic language. Given the complex, descriptive, and metaphorical aspects of the dermoscopic language, subjective interpretations often emerge. The survey evaluated the completeness and diagnostic efficacy of chatbot-generated reports, focusing on their role in facilitating accurate diagnoses and educational opportunities for novice dermatologists. A total of 30 participants were p

Cross Recruitment of Domain-Selective Cortical Representations Enables Flexible Semantic Knowledge.

Knowledge about objects encompasses not only their prototypical features but also complex, atypical, semantic knowledge (e.g., "Pizza was invented in Naples"). This fMRI study of male and female human participants combines univariate and multivariate analyses to consider the cortical representation of this more complex semantic knowledge. Using the categories of food, people, and places, this study investigates whether access to spatially related geographic semantic knowledge (1) involves the same domain-selective neural representations involved in access to prototypical taste knowledge about

Acoustic spectral feedback in vocal learning: a systematic review of psychological mechanisms and skill outcomes.

BACKGROUND: Vocal skill acquisition is inherently "invisible": learners cannot directly observe the movements of their vocal organs, so traditional instruction depends heavily on the teacher's auditory judgment and metaphorical language. This leaves learners ill-equipped to detect errors and self-correct during independent practice. Spectral and acoustic visualization technologies-pitch-trajectory displays, spectrograms, formant analysis, and physiological biofeedback-can externalize this hidden process, yet systematic synthesis at the level of psychological mechanisms remains scarce. A systematic review culturally adapted Cognitive Behavioral Therapy (CaCBT) for Arabic-speaking populations including North African subgroups.

BACKGROUND: Cognitive behavioral therapy (CBT) is an effective treatment for common mental disorders. However, most CBT protocols have been developed within Western cultural frameworks, which may limit their relevance for culturally diverse populations. Culturally adapted CBT (CaCBT) seeks to enhance treatment acceptability and effectiveness by integrating linguistic, cultural, religious, and contextual factors. Despite growing interest in culturally adapted interventions, evidence regarding Arabic-speaking populations, including North African subgroups, remains fragmented. METHODS: Following

Children's emerging concepts of resilience: insights from using body mapping in an East London cohort sample of 7-10-year-old children.

BACKGROUND: Understanding resilience factors in children is essential for developing early mental health interventions. Middle childhood is an understudied developmental stage, with many quantitative measures lacking validation for this age group and not capturing diverse experiences. This study aimed to use body mapping, an arts-based method, as a novel approach to understand 7-10-year-old children's concepts of resilience (including definitions and factors that contribute to resilience) in East London. An advisory group of six children commented on the findings. METHODS: Body mapping was in

Changing public perceptions of alcohol, alcohol harms and alcohol policies: A multi-methods study to develop novel framing approaches.

BACKGROUND AND AIMS: Public perceptions of alcohol and its related harms and policies are shaped by multiple discourses and can influence behaviour and policy support. As part of a FrameWorks-informed project to test framing approaches to improve public understanding and support for evidence-based alcohol policies in the UK, this research aimed to (i) summarise relevant evidence; (ii) compare how public understanding of alcohol harms differs from those of academic and charity experts; and (iii) develop novel framing approaches. METHODS: (1) a literature review including systematic, scoping an
