Mandibular advancement device (MAD) treatment outcome for obstructive sleep apnea (OSA) is variable and patient dependent. A global, clinically applicable predictive model is lacking. Our aim was to combine characteristics obtained during drug-induced sleep endoscopy (DISE), awake nasendoscopy, and computed tomography scan-based computational fluid dynamic (CFD) measurements in one multifactorial model, to explain MAD treatment outcome. A total of 100 patients with OSA were prospectively recruited and treated with a MAD at fixed 75% protrusion. In all, 72 underwent CFD analysis, DISE, and awak

To describe the scientific literature about the diagnosis, prevalence, aetiology, natural course and possible treatment modalities of disc displacements within the temporomandibular (TM) joint. PubMed was searched for specific indexing terms. The search yielded 1211 papers. After screening according to title and abstract, 695 papers were excluded, and after full-text reading, 107 papers remained. Hand-searching of the reference lists resulted in an extra 47 papers. Thirteen studies, published since the literature search was carried out, were also included, resulting in 167 papers for this revi

Bifid mandibular condyle is a rare anomaly; there are several theories about its etiology, and it has been studied in both prehistoric and historic skulls, as well as in living human beings. It is a frequent, although unexpected, finding in asymptomatic individuals during radiological treatment. Presented here is a review of the literature over the past 10 years and two new cases of unilateral bifid condyle. Computerized tomography is usually considered the test of choice for establishing the differential diagnosis, although in certain cases, its use seems questionable. The distinction between

Orofacial biofunction comprises muscular and physical effects, which may contribute to stabilization of the oropharyngeal airway. The tongue-repositioning manoeuvre (TRM) provides physical stabilization of the tongue and the soft palate together with, as a prerequisite, a nasal breathing mode. The aim of the present study was to evaluate the influence of a TRM treatment concept on primary snoring. The TRM was used to achieve a closed biofunctional rest position of the orofacial system and to re-educate the nasal breathing pattern. Pressure indicating oral shields were used for home exercises a

Balance is defined as a state of equilibrium or parity characterized by cancellation of all forces by equal opposing factors. This is the act of maintaining an upright posture (static balance) or in locomotion (dynamic balance or gait). This system depends on vestibular function, vision, and proprioception to maintain posture, to navigate in one's surroundings, to coordinate motion of body parts, to modulate fine motor control, and to initiate the vestibuloculomotor reflexes. These parts of the vestibular system provide our brains with information about changes in head movement with respect to

The goal of every dentist is to preserve the health of his or her patients' masticatory system, as well as to anticipate the long-term results when restorative treatment is provided. These goals cannot be accomplished in the presence of occlusal disease. During the routine oral examination, the signs and/or symptoms of occlusal disease must be noted and the patient educated about the need for further diagnosis and treatment. Better care can be provided to patients if occlusal disease and/or temporomandibular disorders are detected early and properly treated. Treating occlusal disease can lead

We encountered three patients with trigeminal neuralgia-like paroxysmal pain in the buccal nerve region. In all cases, paroxysmal pain was felt spreading to the receptive field of the ipsilateral buccal nerve, and clear tenderness was observed during intraoral palpation at the inner side of the mandibular ramus where the temporalis muscle runs and attaches. It was assumed that the paroxysmal pain might be caused by nerve compression related to temporalis muscle hyperactivity. To lend support to this hypothesis, we also studied the anatomical relationship between the buccal nerve and the mastic

The pivot splint was applied to 50 patients with temporomandibular joint dysfunction syndrome that included 30 patients of anterior disc displacement without reduction, 12 patients of anteriordisc displacement with partial reduction and 8 patients of upward condyle displacement. The mouth opening and joint space of the TMJ tomographies before and after treatment were measured for each case. The results indicated that treatment with pivot splint was very effective for the patients with downward diplacemenr of the condyle. As a result, among 50 patients, 36 patients were cured (72%) and 14 patie

Thirty consecutive patients seeking treatment for painful temporomandibular joint (TMJ) disease were enrolled in a prospective study to assess the relationship between the recapture of displaced disks by anterior repositioning appliances (ARA) and the relief of symptoms. After standard clinical workup including assessment of pain, maxillary and mandibular ARAs were constructed that repositioned condyles to the Gelb 4/7 position. Magnetic resonance imaging (MRI) was performed before and immediately after the insertion of ARAs. Initial MRI findings showed 26 joints with reducing disk displacemen

Background: Tinnitus is a very common symptom that often causes distress and decreases the patient's quality of life. Apart from the well-known causes, tinnitus can in some cases be elicited by dysfunctions of the cervical spine or the temporomandibular joint (TMJ). To date however, it is unclear whether alleviation of these dysfunctions, by physical therapy treatment, also decreases the tinnitus complaints. Such physical therapy could be an interesting treatment option for patients that are now often left without treatment. Objectives: The aim of this review was to investigate the current evi

According to the standard protocol, a load-free healing period is one of the most emphasized requirements for implant integration. Recent studies have encouraged a progressive shortening of the healing period for single-tooth implants and immediate loading has been proposed for the aesthetic zone in the maxilla. The present study evaluated clinical outcomes of immediately loaded FRIALIT-2 Synchro implants 12 months after placement in the maxillary incisal region. In the course of our investigation, nine patients have been treated following an immediate loading protocol. The stepped-screw type

Temporomandibular disorder is a multifactorial disease that causes pain in the jaw and face area with nondental origin, which frequently limits talking, chewing, and other jaw activities. Various factors such as malocclusion, trauma, stress, parafunctional habits (clenching and bruxing), osteoarthritis, and synovitis play a role in its occurrence, although the etiology of these disorders is little understood. Several treatments are being used to treat these disorders. Ozone therapy has been recently introduced as one of these treatments. Considering that no extensive study has been found in th

The case of a 17-year-old female with a benign osteoblastoma in the temporomandibular joint (TMJ) is reported. The patient had a 2.5-year history of reduced mouth opening accompanied by tenderness and swelling in the left TMJ. Initial treatment included stabilization of the occlusion with a splint, jaw exercises, and analgesics. At first the patient's symptoms decreased, but they then increased 18 months later, prompting a cone beam computed tomography (CBCT) evaluation of the joint. The radiographic findings showed a somewhat ill-defined, radiolucent, expansile lesion containing small scatter

BACKGROUND: Osteochondritis dissecans is a process in which segment(s) of cartilage separate from an articular surface. It is most often reported in the larger joints of the body, including the knee, elbow, hip, wrist, and ankle. Involvement of the temporomandibular joint (TMJ) is exceedingly rare.
OBJECTIVES: To describe a probable case of osteochondritis dissecans and discuss the differential diagnosis for this condition.
STUDY DESIGN: In addition to clinical examination the patient was imaged using panoramic radiography, computed tomography and magnetic resonance imaging.
RESULTS: Panora

A prospective study of a new stent for bridging choledochal stenoses was performed in seven patients (5 females, 2 males, age range 49-80 years) with benign bile duct stenosis or bilioduodenal fistula in whom conventional bougienage (3 patients) and/or month-long implantation of plastic prostheses (7 patients) failed because of reocclusion. The self-expanding mesh stents (Wallstent) were implanted by duodenoscopy in six patients. In one patient with a B-II stomach, the stent could only be inserted by the percutaneous transhepatic route. All prostheses were inserted endoscopically without compl

Patients with a midline diastema smaller than 2 mm are often treated with orthodontic elastics applied on composite resin esthetic buttons bonded to incisors or incorporated in clear removable trays. However, these methods of applying elastics for diastema closure have shortcomings, including the additional clinical steps of etching the facial tooth surface and bonding for composite resin buttons; occlusal derangement and dependence on patient compliance for trays; and compromised esthetics for both treatment approaches. This article presents an alternative technique that overcomes the limitat

INTRODUCTION: This study aimed to analyze the factors affecting the occurrence of peri-implant medication-related osteonecrosis of the jaw (PI-MRONJ) in patients using anti-resorptive drugs (ARDs) on different implant position, inclinations, and types of prosthesis.
METHODS: The data of 75 patients with bone necrosis that progressed around the implant between 2018 and 2022 were retrospectively examined to identify the factors influencing PI-MRONJ. Data, including patient demographics (age, sex, smoking status, concomitant disease, time of ARD therapy, dose of ARDs, and parafunctional habits)

BACKGROUND/AIMS: Fracture of the alveolar process is a common injury, but the traditional splinting fixation may not be possible in some cases. The aim of this study was to describe the osteosynthesis performed in segmental alveolar fractures by internal fixation and evaluate the prognosis of the fractures and teeth involved.
MATERIALS AND METHODS: Twenty-two patients who were identified as having segmental alveolar fractures treated with osteosynthesis by internal fixation at the authors' department from January 2007 to December 2016 were included with 90 traumatized teeth. The prognosis of

AIMS: To assess the effectiveness of acupuncture for the symptomatic treatment of temporomandibular disorders (TMD) from a review of studies using randomized controlled trials (RCTs).
METHODS: Electronic databases were systematically searched for articles reporting RCTs investigating acupuncture for TMD. The methodological qualities of eligible studies were assessed using the criteria described in the Cochrane Handbook.
RESULTS: Nineteen reports were systematically reviewed. There was moderate evidence that classical acupuncture had a positive influence beyond those of placebo (three trials,
