Acetabular reconstruction using a roof reinforcement ring with hook for total hip arthroplasty in developmental dysplasia of the hip-osteoarthritis minimum 10-year follow-up results.

The outcome of 33 total hip arthroplasties using an acetabular reinforcement ring with hook for osteoarthritis associated with developmental dysplasia of the hip was reevaluated after a minimal follow-up of 10 years (average, 10.8 years). Most cases comprised Crowe type II (22 hips) and type III (7 hips) dysplasia. The mean D'Aubigné score increased from 7 to 15 points. Three revisions (9%) were performed for aseptic loosening, 2 of them had a structural autograft. None of the unrevised 30 acetabular reconstructions showed definite radiographic signs of loosening. These results compare favorab

Identifying frailty in acutely hospitalised patients with hip fracture: are the clinical frailty scale and irish hip fracture database frailty index comparable for predicting adverse outcomes?

PURPOSE: Frail older adults are at increased risk of adverse outcomes after hip fracture. We compared the Clinical Frailty Scale (CFS) and the Irish Hip Fracture Database Frailty Index (IHFD FI) in predicting hospital outcomes. METHODS: A secondary analysis of national clinical audit data from two trauma units was conducted. The CFS and IHFD FI were derived from routinely collected data. Multivariable logistic regression assessed the predictive value of age, sex, ASA grade, plus either frailty tool for the following outcomes: delirium (positive 4AT and/or clinical diagnosis), inpatient mortal

Hip and Pelvis Movement Patterns in Patients With Femoroacetabular Impingement Syndrome Differ From Controls and Change After Hip Arthroscopy During a Step-Down Pivot-Turn Task.

BACKGROUND: Alterations in hip kinematics during functional tasks occur in positions that cause anterior impingement in patients with femoroacetabular impingement (FAI) syndrome. However, tasks that do not promote motions of symptomatic hip impingement remain understudied. PURPOSE: To compare movement patterns of the hip and pelvis during a step-down pivot-turn task between patients with FAI and controls as well as in patients with FAI before and after hip arthroscopy. STUDY DESIGN: Controlled laboratory study. METHODS: Three-dimensional motion capture was acquired in 32 patients with FAI a

Early Revision Rates of Total Hip Arthroplasty Using the Intellijoint HIP Computer Navigation System: A Study From the Australian National Joint Replacement Registry of 1911 Procedures.

BACKGROUND: Total hip arthroplasty (THA) is an effective treatment for symptomatic hip osteoarthritis. The aim of this study was to determine the revision outcome of commercially available navigation technologies. METHODS: Data from the Australian Orthopaedic Association National Joint Replacement Registry from January 2016 to December 2020 included all primary THA procedures performed for osteoarthritis. Procedures using the Intellijoint HIP navigation system were identified and compared to procedures using "other" computer navigation systems and to nonnavigated procedures. The cumulative pe

The influence of anterior and posterior knot placement on hip function after total hip arthroplasty using a modified Dall's approach: a prospective non-randomised comparative study.

BACKGROUND: The direct lateral modified Dall's approach for total hip arthroplasty (THA) provides an excellent vision of the hip joint by osteotomising the greater trochanter (GT). A robust method for the reattachment of osteotomised fragments is essential to prevent complications around the GT. Ultra-high molecular weight polyethylene cables are reported to be useful for reattachment; but the optimal suture method of these cables is unknown. The purpose of this study was to investigate the influence of the knot position on hip function after primary THA. METHODS: In a prospective non-randomi

Intelligent prosthetic hips and knees: from actuation to perception and control.

Intelligent prosthetic hips and knees represent a critical advancement in restoring natural gait and mobility for lower-limb amputees, particularly those with high-level amputations such as hip disarticulation. This systematic review examines recent progress in three fundamental aspects of intelligent prosthetic technology: actuation, perception, and control. In terms of actuation, the review highlights the limitations of passive and active prostheses and discusses emerging hybrid active-passive mechanisms that aim to replicate the natural, biarticular muscle-driven energy transfer in human ga

LongTerm Outcomes of Suspected Transient Synovitis of the Hip in NonAspirated Patients.

BACKGROUND: Transient synovitis is a common cause of pediatric hip pain that must be distinguished from septic arthritis. Although the literature includes outcome data for aspirated hips, long-term outcomes in patients managed presumptively without aspiration remain limited. This study aimed to characterize the long-term clinical outcomes of children with presumed transient synovitis managed without hip aspiration. METHODS: A retrospective study of pediatric patients diagnosed with transient synovitis and managed presumptively without aspiration. Collected variables included demographics, cli

Health economic evaluation of hip fracture care pathways in The Gambia.

Hip fractures cause substantial morbidity, mortality and healthcare costs among older adults. While advances in surgical and postoperative care have improved outcomes in high-income countries, these interventions are often inaccessible in low-income and middle-income countries due to cost constraints. This study assessed the costs and outcomes of surgical versus non-surgical management of hip fractures to inform resource allocation in a resource-limited setting. Data from a prospective cohort study of patients with acute hip fractures over a 12-month postfracture follow-up period were used. Ou

The role of the ischiofemoral ligament in hip dislocation resistance: a cadaveric study.

BACKGROUND: The ischiofemoral ligament (ISFL) contributes to posterior hip stability. However, its independent role and mechanical limitations remain unclear. This study aimed to quantify the stabilizing effect of the ISFL on posterior dislocation under progressive hip flexion. METHODS: Nine fresh-frozen cadaveric hips were tested using a custom jig under three conditions: (1) capsular incision distal to the ISFL with the ISFL preserved, (2) capsular repair with the ISFL preserved, and (3) capsular incision with the ISFL transected. Condition (1) represented a posteroinferior capsular incision

[Strategies for complications prevention and management of complex total hip arthroplasty].

Although the complications of complex total hip arthroplasty (THA) are decreasing with the progress of surgical technology, it is still a difficult clinical problem. Complex THA has a high risk of dislocation, vascular and nerve injury due to anatomical disorder, poor soft tissue condition, bone defect and other factors. Recent studies have shown that spinal-pelvic linkage, soft tissue protection and reconstruction, and the removal of impingement potential factors play an important role in maintaining hip joint stability. Minimally invasive interventional therapy has become an important means

Developmental Dysplasia of the Hip: Clinical Predictors and the Impact of a Screening Protocol.

Background Developmental dysplasia of the hip (DDH) includes a spectrum of hip abnormalities that can cause significant functional impairment if not diagnosed early. Currently, no universal consensus exists regarding the optimal screening strategy. In March 2020, a new DDH screening protocol was implemented at a hospital in northern Portugal. This study aimed to assess the prevalence of DDH, identify associated risk factors and physical examination findings, and evaluate clinical practice changes post-protocol. Methodology We conducted a retrospective, observational, cohort study analyzing hip

The Role of Arthroplasty in Acute and Failed Fracture Surgery Around the Hip.

Arthroplasty plays a crucial role in the management of both acute fractures and failed fracture surgeries around the hip, particularly in the aging population. It is important to explore the indications, benefits, and challenges of total hip arthroplasty in various clinical scenarios, including geriatric acetabular fractures, proximal femur fractures, and cases of failed hip fracture surgery. Acute total hip arthroplasty is increasingly recognized as a viable alternative to open reduction and internal fixation in elderly patients with poor bone quality or preexisting arthritis. When treating p

Genetic study identifies novel genes in developmental dysplasia of the hip.

Developmental dysplasia of the hip (DDH), a morphological abnormality of the hip joint, is a well-recognized risk factor for hip osteoarthritis (OA). Much remains unknown about the genetic factors of DDH and its subtypes. To further understand its genetic basis, we conducted genome-wide association studies (GWASs) using a total of 1 085 Japanese DDH cases (including 788 hip dysplasia cases without dislocation and 297 cases with dislocated hip) and 24 000 controls. Additionally, we meta-analyzed with United Kingdom (UK) DDH GWAS and the largest hip OA GWAS to date. We identified three genome-wi

Fractures in Patients With Hip and Knee Prostheses: Epidemiology, Diagnosis, and Treatment.

BACKGROUND: Fractures in patients with hip or knee prostheses (periprosthetic fractures, PPF) are rare but clinically significant complications whose absolute number is steadily increasing as implantation rates rise. In Germany, approximately 200 000 hip and 170 000 knee joints are primarily replaced each year: PPFs account for approximately 15% of all revisions of hip surgery and 3% of all revisions of knee surgery. METHODS: This review is based on publications retrieved by a selective search in the PubMed/MEDLINE and Google Scholar databases up to December 2025. Registry analyses, meta-anal

Hip Labral Reconstruction With a Synthetic Graft: Development and Preclinical Validation.

OBJECTIVES: Hip Osteoarthritis (OA) affects a significant component of the adult population, placing itself among the most important causes of disability and need for total hip replacement. Femoroacetabular impingement (FAI) is an anatomic alteration of the proximal femur and/or acetabulum leading to chondro-labral damage and playing a prominent role in OA pathogenesis. Thus, treating FAI is fundamental to relieve hip pain and further joint tissue deterioration. Labral reconstruction is considered the treatment of choice, in particular using tendon allografts or autografts which, however, have
