Dose-dependent association of systemic comorbidities with periodontitis severity: A large population cross-sectional study.

BACKGROUND: To examine whether the associations between periodontitis and multiple systemic conditions increase with increasing severity of periodontitis using a multi-center electronic health record (EHR) repository. METHODS: A cross-sectional analysis was conducted using EHR data from 9 dental schools in the United States. We included 264,913 adults (≥18 years) receiving periodontal therapy between 2013 and 2023. Periodontitis severity (no, mild/moderate, severe) was determined, and out of 98 systemic and behavioural conditions, associations with 24 conditions were evaluated using weighted

Dectin-2 depletion alleviates osteoclast-induced bone loss in periodontitis via Syk/NOX2/ROS signaling.

Periodontitis is the sixth most common disease worldwide and is closely associated with various systemic diseases, impacting overall health. It is characterized by the over-differentiation and activity of osteoclasts, leading to increased bone resorption and subsequent bone loss. Current treatments for bone loss are not ideal, highlighting the need for new targeted therapeutic strategies. Dectin-2, a member of the C-type lectin receptor (CLR) family, has recently been reported to play an important role in immune regulation, but its role in osteoclastogenesis has not been documented. This study

Discovery proteomics to detect salivary biomarkers in dog and human periodontitis: Mass spectrometry-based analysis.

Periodontitis, prevalent in humans and dogs, is a chronic inflammatory disease affecting the oral hard and soft tissues. Salivary proteomics and protein homology may offer a non-invasive diagnosis, benefiting both species due to similarities in disease etiology and progression. This study compared (i) the proteomics profile of healthy and periodontitis dogs using mass spectrometry; and (ii) the protein homology of the identified differentially expressed proteins between humans and dogs. Twenty dogs were examined over six months, divided into two groups: ten healthy (control) and ten with perio

Association between serum levels of inflammatory mediators and periodontitis severity in people with down syndrome.

Evidence suggests that individuals with Down syndrome (DS) have a high prevalence of periodontal disease (PD). Factors such as muscular hypotonia, macroglossia, intellectual deficits, and reduced motor coordination, often combined with inadequate dietary habits, contribute to the development of PD. Additionally, a compromised immune response may explain the altered inflammatory profile that affects the systemic and oral health of this population. This study aimed to analyze the relationship between serum levels of inflammatory cytokines (IFN-γ, IL-10, IL-17, IL-1β, IL-4, and TNF-α) and the sev

Featured Immune Characteristics of Periodontitis and Primary Sjögren's Syndrome Revealed by Single-Cell Transcriptome Analyses.

Periodontitis (PD) and primary Sjögren's syndrome (pSS) are persistent inflammatory diseases that affect oral health, and numerous studies have uncovered phenotypic and intrinsic correlations between the two. However, a comprehensive and high-resolution analysis at the single-cell level has not been reported comparing the immune cell landscapes in PD and pSS. This study integrated single-cell transcriptome data from peripheral blood mononuclear cells (PBMCs) of healthy controls (HC), PD and pSS patients sourced from the NCBI and NGDC databases. The scRNA datasets were integrated, and cells wer

The Oral-Gut-Systemic Axis: Emerging Insights into Periodontitis, Microbiota Dysbiosis, and Systemic Disease Interplay.

The oral cavity harbors one of the most diverse microbial ecosystems in the human body, second only to the gut. Periodontitis, a chronic inflammatory disease arising from oral microbiota dysbiosis, has been increasingly associated with systemic disorders such as diabetes mellitus, atherosclerosis, rheumatoid arthritis, inflammatory bowel disease, and neurodegenerative conditions. Although hematogenous dissemination of oral pathogens and inflammatory mediators has long been proposed as a mechanistic link, emerging evidence identifies the oral-gut axis as a novel bidirectional pathway. L-arginine modified mesoporous bioactive glass with ROS scavenging and NO release for periodontitis treatment.

Periodontitis is a chronic inflammatory disease characterized by progressive alveolar bone resorption, and excessive reactive oxygen species (ROS) is a key factor to disease progression. Therefore, scavenging ROS to alleviate inflammation and promote bone regeneration are promising strategies to treat periodontitis. In this study, L-arginine (L-Arg) was used to modify mesoporous bioactive glass (MBG), forming L-Arg modified MBG (MBG@L-Arg), which showed effective ROS-scavenging and NO release properties in cells, and realize the protection and restoration of cell's activity in ROS-rich microen

Ferulic Acid Alleviates Inflammation and Promotes Osteogenic Differentiation in Periodontitis by Inhibiting NF-κB Pathway.

BACKGROUND: Periodontitis refers to a chronic inflammatory illness that induces the destruction of periodontal tissues and can be driven by bacterial lipopolysaccharide (LPS) of pathogens. This study investigated the anti-inflammatory potential and underlying mechanisms of ferulic acid (FA) in periodontitis. METHOD: An in vitro periodontitis model was established by treating human periodontal ligament stem cells (hPDLSCs) with 10 µg/mL LPS for 24 h. The experimental groups included a control group, an LPS-treated group, and an LPS + FA cotreatment group. In addition, phorbol 12-myristate 13-a

V-ATPase and Lysosomal Energy Sensing in Periodontitis and Medicine-Related Osteonecrosis of the Jaw.

Diabetes is a risk factor for periodontitis. Increasing evidence suggests that a central player in this link is the vacuolar H+-ATPase (V-ATPase), which provides a physical and functional core for regulation by the catabolic lysosomal AMP-activated protein kinase complex (L-AMPK) and the anabolic mammalian target of rapamycin complex 1 (mTORC1). These complexes detect levels of various cellular nutrients, including glucose at the lysosome, and promote cellular responses to restore homeostasis. The high-glucose conditions of diabetes foster anabolic mTORC1 signaling that increases inflammation

Mitoribosome-Targeting Antibiotics Suppress Osteoclastogenesis and Periodontitis-Induced Bone Loss by Blocking Mitochondrial Protein Synthesis.

The onset and progression of periodontitis are closely related to metabolic reprogramming in the periodontal microenvironment, with osteoclasts playing a critical role in tissue destruction. Single-cell RNA sequencing (scRNA-seq) of periodontal tissues from healthy individuals and patients with severe chronic periodontitis revealed a significant increase in the expression of mitochondrial-related genes during osteoclast differentiation, suggesting the critical role of mitochondrial function in this process. This study investigates the potential of the novel mitoribosome-targeting antibiotic ra

Effect of Oral Pathogens Associated With Pulpitis and Apical Periodontitis on Odontogenic Mesenchymal Stem Cells.

Dental mesenchymal stem cells (MSCs) play an essential role in the development of immature permanent teeth. Bacterial infection of the pulp and periapical tissues of immature permanent teeth, the associated oral pathogens, and their virulence factors affect the viability, proliferation, differentiation, and cytokine secretion of MSCs. Bacteria and virulence factors can also trigger an inflammatory response that induces pro-inflammatory cytokine secretion and destroys odontogenic MSCs in the pulp and periapical region, negatively affecting the development of immature permanent teeth. The presen

Pathophysiological Insights Into the Oral-Brain Axis: Evidence-Based Mechanisms Connecting Periodontitis and Alzheimer's Disease.

The bidirectional association between Alzheimer's disease (AD) and periodontitis (PD) has been recently demonstrated, indicating how the oral-brain axis connects the two conditions. Chronic oral inflammation caused by PD is accompanied by biological immunological responses, unchecked inflammation, and the spread of periodontal bacteria, all contributing to nervous system inflammation and AD pathogenesis. There are two primary pathways: (a) Inflammatory cascades, in which pro-inflammatory cytokines, such as IL-1β and TNF-α, originating from periodontal lesions, cause inflammation in the brain r

Periapical actinomycosis of the mandible: A diagnostic challenge mimicking conventional apical periodontitis- A case report.

Actinomycosis is a rare, chronic suppurative infection caused by filamentous, Gram-positive anaerobic bacteria, primarily Actinomyces israelii, with cervicofacial involvement being the most common presentation. Periapical actinomycosis, a rare central subtype of cervicofacial actinomycosis accounting for approximately 1-2% of central cases, is characterized by an indolent, less invasive course. This case report describes an unusual presentation of mandibular periapical actinomycosis in a 26-year-old immunocompetent female with no history of trauma or predisposing comorbidities. The patient pre

Periodontitis increases the risk of gastrointestinal dysfunction: an update on the plausible pathogenic molecular mechanisms.

Periodontitis is an immuno-inflammatory disease of the soft tissues surrounding the teeth. Periodontitis is linked to many communicable and non-communicable diseases such as diabetes, cardiovascular disease, rheumatoid arthritis, and cancers. The oral-systemic link between periodontal disease and systemic diseases is attributed to the spread of inflammation, microbial products and microbes to distant organ systems. Oral bacteria reach the gut via swallowed saliva, whereby they induce gut dysbiosis and gastrointestinal dysfunctions. Some periodontal pathogens like Porphyromonas. gingivalis, Kle

Intermittent hyperglycaemia induces macrophage dysfunction by extracellular regulated protein kinase-dependent PKM2 translocation in periodontitis.

Early fluctuations in blood glucose levels increased susceptibility to macrophage dysfunction. However, the underlying pathological mechanisms linking glucose variations and macrophage dysregulation remains elusive. In current study, we established an animal model of transient intermittent hyperglycaemia (TIH) to simulate early fluctuations in blood glucose levels. Our findings revealed that both TIH and diabetic group exhibited more severe periodontal lesions and increased secretion of pro-inflammatory cytokines compared to healthy controls. In immortalized bone marrow-derived macrophages (iB
