Which laboratory tests should be performed on children with apparent febrile convulsions? An analysis and review of the literature.

There exist wide variations among clinicians as to management of the child who presents with the first febrile convulsion. In 1980, the American Academy of Pediatrics issued a consensus statement on febrile convulsions which included recommendations for initial management. Multiple studies have also addressed this topic. From these studies, guidelines for management have been drawn. Review of these studies reveals little scientific support for current management strategies. The decision to perform lumbar puncture is analyzed as an example of the practical limits of clinical investigations. Rel

Guidelines for the management of actinic keratoses.

These guidelines stemmed from a consensus meeting held by the British Photobiology Group (BPG) in 1999. Following this meeting one of the authors (J.M.M.) was invited to draw up guidelines for the management of actinic keratoses by the British Association of Dermatologists Therapy Guidelines and Audit Subcommittee. Relevant evidence was sought using the search terms 'solar keratosis' and 'actinic keratosis' in Medline from 1966 onwards. Additional and earlier literature was reviewed on the basis of references within post-1966 publications. All articles of apparent relevance were reviewed indep

A survey on the clinical practice of herpes zoster and postherpetic neuralgia management by Korean medicine doctors: towards the development of Korean medicine clinical practice guidelines.

BACKGROUND: Herpes zoster (HZ) is a painful viral disease caused by varicella-zoster virus reactivation. Postherpetic neuralgia (PHN), characterized by severe pain, is its most frequent complication. Due to the limitations of western medicine (WM) in managing these conditions, there is increasing demand for Korean medicine (KM). However, Korean medicine clinical practice guidelines (KM-CPGs) for HZ and PHN are not yet established. This study investigates current clinical practices and perceptions of Korean medicine doctors (KMDs) regarding HZ and PHN to facilitate KM-CPGs development. METHODS

Management of dens fractures in older people and the role of the geriatrician: a survey of practice in UK and Ireland major trauma centres.

PURPOSE: Dens fractures are common in individuals aged 65 years and older, with most managed non-operatively. In older adults, these fractures are frequently treated with rigid collars. However, rigid collars have limited evidence base of benefit to support their use and are associated with adverse outcomes. This survey aimed to investigate standard local practices for the management of dens fractures, including the role of the geriatrician in adults aged 65 and over, across major trauma centres in the United Kingdom and Ireland. METHODS: Clinical leaders at all major trauma centres in the UK

Implementation of a patient-focused psychosocial intervention guideline for people with severe mental illness: Cluster-randomised controlled trial.

BACKGROUND: Psychosocial interventions are vital in treating severe mental illness, yet their use remains limited, and patients often lack adequate information about them. Patient-focused versions of clinical guidelines are designed to enhance mental health literacy and inform patients about available treatments, but these resources are underutilized. This study evaluated the impact of implementing a patient-focused psychosocial intervention guideline on empowerment, knowledge, and use of psychosocial interventions among individuals with severe mental illness. METHODS: Multicentre, cluster-ra

Comparison of CPG's for the diagnosis, prognosis and management of non-specific neck pain: a systematic review.

BACKGROUND: Neck pain (NP) is a very common musculoskeletal condition with potential for a high burden in disability and length of disorder. Clinical practice guidelines (CPG) give recommendations to clinicians for providing optimal care for patients however best practice recommendations are often contradictory. The purpose for this review was to conduct a SR of CPGs to assess the management recommendations for NP (diagnosis, treatment, prognosis, imaging). METHODS: Standard SR methodology was employed including a grey literature search (including the National Guideline Clearing House). Medli

Refeeding in anorexia nervosa.

Refeeding in anorexia nervosa is a collaborative enterprise involving multidisciplinary care plans, but clinicians currently lack guidance, as treatment guidelines are based largely on clinical confidence rather than more robust evidence. It seems crucial to identify reproducible approaches to refeeding that simultaneously maximize weight recovery and minimize the associated risks, in addition to improving long-term weight and cognitive and behavioral recovery and reducing relapse rates. We discuss here various approaches to refeeding, including, among others, where, by which route, how rapidl

Challenges of implementing fibromyalgia treatment guidelines in current clinical practice.

The current diagnostic and treatment pathway for patients with fibromyalgia (FM) is lengthy, complex, and characterized by multiple physician visits with an average 2-year wait until diagnosis. It is clear that effective identification and appropriate treatment of FM remain a challenge in current clinical practice. Ideally, FM management involves a multidisciplinary approach with the preferable patient pathway originating in primary care but supported by a range of health care providers, including referral to specialist care when necessary. After the publication of individual clinical studies,

Adjunctive drug therapies for treatment of respiratory diseases in the newborn: based on evidence or habit?

Respiratory distress syndrome is a disease of prematurity and is caused by a relative deficiency of endogenous surfactant production. Respiratory distress syndrome is the most common cause of mortality and morbidity in the newborn population and the standard of care is to provide exogenous surfactant therapy. This saves lives and reduces respiratory complications but, despite treatment, a significant proportion of these infants go onto develop chronic lung disease, the severest form of which is bronchopulmonary dysplasia. Once developed, this is a multisystem disease and treatment is mostly su
