Anorectal Complaints: Hemorrhoids, Fissures, Abscesses, Fistulae.

Anorectal complaints are not uncommon in pediatric care, but the etiology and management can differ significantly from adults. Age is an important factor when considering etiology and management, distinguishing between infants, children, and adolescents. For all ages, malignancy is rarely a consideration, but a thorough examination of infants and children typically requires deep sedation or general anesthesia. Very little primary literature or evidence exists to guide care; so there are many opportunities for careful study to enhance our understanding beyond personal experience and historical

Delivery of an Impacted Fetal Head During Cesarean: A Literature Review and Proposed Management Algorithm.

Effective and expedient management of an impacted fetal head (IFH) at cesarean delivery is essential to avoid maternal and neonatal morbidity. The incidence of an IFH at the time of cesarean may increase because of changing practice guidelines regarding the acceptable duration of the second stage of labor. Based on limited available evidence comparing the "push," "pull," and other methods for delivery of the IFH, we suggest the primary disengagement technique be selected based on surgeon experience and propose a management algorithm "ALERT" for clinical application.

[Rhinosinusitis].

Rhinosinusitis is an important health problem which is increasing in frequency and which has a large financial repercussion on society. Better understanding of ARS management has modified our clinical approach leading us to minimize unnecessary use of radiological investigations, overuse of antibiotics, and improve the under utilisation of nasal corticosteroids. The goal of this article is to present important new guidelines mainly based on the European position paper on rhinosinusitis and nasal polyps 2012 (EPOS 2012) in order to facilitate ARS management in general practice.

Faecal incontinence in critical illness.

Diarrhoea is a common problem in ICUs, occurring in as many as half of patients who are critically ill. There is a lack of specific guidance on the management of faecal incontinence in these patients. A literature review was used to identify best practice of faecal incontinence in patients in critical care and draw up local guidelines. A specific focus was put on the management of faecal incontinence to protect and heal damaged skin. Nurses are ideally placed to support patients with this condition.

Buprenorphine treatment: factors and first-hand experiences for providers to consider.

The viability of using buprenorphine to treat opiate dependence was well documented prior to federal approval in October 2002. What has been lacking in the literature is "hands-on" experience of providers from a clinical management and practice management perspective. This article adds to the knowledge base by providing information about buprenorphine treatment as well as anecdotes from patients treated by the authors, leading to a detailed list of factors worth considering for the treatment provider contemplating adding an opiate-addicted population to an existing treatment base.

Contemporary minimally invasive approaches to the management of acute cholecystitis: a review and appraisal.

Acute cholecystitis is one of the most common emergency admissions in surgical practice. This review appraises the available evidence from the English-language literature regarding the minimally invasive approaches to the management of this condition. The following aspects of care are reviewed and appraised: (1) the diagnostic criteria for acute cholecystitis, (2) the optimal timing for cholecystectomy (early, delayed, or interval surgery), (3) the optimal approach to cholecystectomy (laparoscopic versus open), (4) the role of intraoperative cholangiography, and (5) the management of patients

Management of Distal Femoral Fractures in the Elderly.

Distal femoral fractures (DFF) cause significant morbidity and mortality comparable with neck of femur fractures. In view of this, NICE, National Institute of Clinical Excellence, has introduced a Best Practice Tariff (BPT) for management of distal femoral fractures in the elderly to improve morbidity and reduce regional variation of standards. Operative management includes application of external fixation, open reduction and internal fixation, or arthroplasty. While these are associated with significant morbidity, non-operative management may also be considered with immobilisation or traction

Preparation of Nurses to Manage Emergencies: An Educational Focus.

Prelicensure nursing education inadequately prepares nurses for emergency management, creating significant gaps between theoretic knowledge and practical emergency competencies despite nurses' vital role in emergency response. This article examines current evidence on educational strategies for preparing nurses to effectively manage emergency situations while identifying best practices that optimize patient outcomes and nurse competency. Three core competencies are essential for emergency nursing: caring for acutely ill patients, effective communication, and care coordination. Implementation c

Improved forest management for increased carbon sequestration: An assessment of the most prominent approaches in Norway.

Intensification of forest management is seen as one important measure to increase carbon sequestration and contribute to balance CO2 emissions and mitigate climate change. Potential measures for forest management intensification include increasing the percentage of the area that is actively reforested after felling, planting at higher densities and with genetically improved material, nitrogen fertilization, and pre-commercial thinning. Here we assessed the mitigation potential of these practices in Norwegian forests from 2018 until the end of the 21st century. As a result, when these managemen

Potential associations between the mite Varroa destructor and other stressors in honeybee colonies (Apis mellifera L.) in temperate and subtropical climate from Argentina.

The presence of Varroa destructor in colonies of Apis mellifera is explained by the interaction among a number of factors including beekeeping practices and surrounding environment features. The aim of this study was to evaluate the relative impact of environment geographical region and beekeeping management on Varroa infestation levels throughout a year. A monitoring study was carried out during 2015 in north-central regions from Argentina, consisting of three sampling dates: 1) autumn survey before autumn acaricide treatment; 2) autumn survey after autumn acaricide treatment and 3) spring su

Congenital portosystemic shunts in cats: investigation, diagnosis and stabilisation.

PRACTICAL RELEVANCE: Although a relatively uncommon condition, the investigation, diagnosis and initial medical management of feline congenital portosystemic shunts is often undertaken within general practice. Early recognition and appropriate treatment are important to ensure a good outcome. CLINICAL CHALLENGES: Clinical signs associated with CPSSs in cats are extremely variable and can be intermittent. Signs can affect a variety of organ systems including the nervous system, and gastrointestinal and urinary tracts. Thus, the differential diagnosis list may be very long and a CPSS may not be
