Pharmacokinetic modelling during long-term anesthesia: minimizing the gap.

INTRODUCTION: Surgical procedures exceeding six hours and intensive care for traumatic injuries often require prolonged general anesthesia, commonly maintained via induced coma. Similar protocols are used for COVID-19 patients under mechanical ventilation. These conditions pose challenges such as tissue drug accumulation and overdose risk. Extended supine or prone positioning alters tissue volumes based on their physical properties. Accurate anesthesia management depends on patient-specific models that characterize pharmacokinetics (PK, drug distribution) and pharmacodynamics (PD, drug effects

The diagnostic value of an upright chest radiograph in diagnosing acromioclavicular joint dislocation.

INTRODUCTION: A chest radiograph (CXR), whether supine or upright, is the primary tool for assessing blunt thoracic and abdominal trauma. Thoracic injuries often come with shoulder girdle injuries such as scapular or clavicular fractures, and acromioclavicular joint (ACJ) dislocations. The Zanca view is standard for diagnosing ACJ dislocation. While upright CXR can screen for high-grade ACJ dislocation, its diagnostic value in these cases remains unreported. OBJECTIVE: This study aimed to determine the diagnostic value of upright CXR, compared to the standard Zanca view in diagnosing ACJ disl

Visual Acuity Thresholds in Preterm Newborns: An Experimental Study.

Purpose: Visual acuity plays a role in mediating neurological development in infants by enabling the differentiation of shapes and discriminating objects. Given the rapid structural development of the brain in the first days of life, this aspect is particularly significant for preterm infants, who typically experience this developmental phase while hospitalized in the neonatal intensive care unit (NICU). Therefore, this study aimed to assess visual acuity thresholds in preterm infants during hospitalization and to evaluate possible correlations between visual acuity and clinical parameters. Me

An ape partial postcranial skeleton (KNM-NP 64631) from the Middle Miocene of Napudet, northern Kenya.

An ape partial postcranial skeleton (KNM-NP 64631) was recovered during the 2015-2021 field seasons at Napudet, a Middle Miocene (∼13 Ma) locality in northern Kenya. Bony elements representing the shoulder, elbow, hip, and ankle joints, thoracic and lumbar vertebral column, and hands and feet, offer valuable new information about the body plan and positional behaviors of Middle Miocene apes. Body mass estimates from femoral head dimensions suggest that the KNM-NP 64631 individual was smaller-bodied (c. 13-17 kg) than some Miocene taxa from eastern Africa, including Ekembo nyanzae, and probably

Lung ultrasound response to awake prone positioning predicts the need for intubation in patients with COVID-19 induced acute hypoxemic respiratory failure: an observational study.

BACKGROUND: Awake prone positioning (APP) reduces the intubation rate in COVID-19 patients treated by high-flow nasal cannula (HFNC). However, the lung aeration response to APP has not been addressed. We aimed to explore the lung aeration response to APP by lung ultrasound (LUS). METHODS: This two-center, prospective, observational study enrolled patients with COVID-19-induced acute hypoxemic respiratory failure treated by HFNC and APP. LUS score was recorded 5-10 min before, 1 h after APP, and 5-10 min after supine in the first APP session within the first three days. The primary outcome was

Collateral ligament sprains of the metacarpophalangeal joint of the long fingers: Results of a surgical series of 15 patients.

INTRODUCTION: Collateral ligament sprains of the metacarpophalangeal joint (MP) of the long fingers are rare and mostly treated conservatively. Clinical examination to diagnose the severity of these injuries is mandatory. The purpose of our study is to report the results of 15 patients treated surgically. METHODS: Twenty-three patients, mean age 48, underwent surgery for Stage 3 radial collateral ligament (RCL) injuries of the middle finger (12), the ring finger (4) and the little finger (7). The mean time from trauma to surgery was 53 days. The clinical evaluation consisted of measuring acti

Meeting international aerobic physical activity guidelines is associated with enhanced cardiovagal baroreflex sensitivity in healthy older adults.

PURPOSE: Cardiovagal baroreflex sensitivity (cvBRS) reflects the efficiency of modulating heart rate in response to changes in systolic blood pressure. International guidelines recommend that older adults achieve at least 150 min of moderate-vigorous physical activity per week. We tested the hypothesis that older adults who achieve these guidelines will exhibit greater cardiovagal baroreflex sensitivity versus those who do not. METHODS: A cross-sectional comparison of older adults who did (active, 66 ± 5 years, 251 ± 79 min/week; n = 19) and who did not (inactive, 68 ± 7 years, 89 ± 32 min/we

Midterm Follow-up of Treating Volar Marginal Rim Fractures with Variable Angle Lcp Volar Rim Distal Radius Plates.

BACKGROUND: Specific treatment of the volar marginal rim fragment of distal radius fractures avoids occurance of volar radiocarpal dislocation. Although several fixation systems are available to capture this fragment, adequately maintaining internal fixation is difficult. We present our experience of the first 10 cases using the 2.4 mm variable angle LCP volar rim distal radius plate (Depuy Synthes®, West Chester, US), a low-profile volar rim-contouring plate designed for distal plate positioning and stable buttressing of the volar marginal fragment. METHODS: Follow-up patient satisfaction, r

Human resting muscle tone (HRMT): narrative introduction and modern concepts.

Human resting muscle (myofascial) tone (HRMT) is the passive tonus or tension of skeletal muscle that derives from its intrinsic (EMG-silent) molecular viscoelastic properties. The word tone has been used to convey varying clinical and physiological features that have led to confusion and controversy. HRMT is the vital low-level, passive tension, and resistance to stretch that contributes importantly to maintain postural stability in balanced equilibrium positions. In contrast, co-contraction of muscle is an active neuromotor control that provides greater levels of tonus for increased stabiliz

Large artery stiffness is elevated in women with a prior pregnancy conceived via in vitro fertilization.

In vitro fertilization (IVF) is an effective treatment for infertility; however, IVF may increase future maternal cardiovascular disease (CVD) risk. Vascular dysfunction, characterized by increased large elastic artery stiffness and endothelial dysfunction, predicts future CVD risk. However, whether IVF use influences maternal vascular dysfunction is unknown. We studied premenopausal women with a live birth within the past 1-5 yr conceived either without medical assistance (n = 24) or via IVF (n = 15), matched for age, body fat percentage, and seated blood pressure. Large artery stiffness (car

Cerebral perfusion mismatch: challenging the validity of cerebral vascular resistance index in orthostatic stress.

The cerebral vascular resistance index (CVRi), a single-parameter model based on systemic blood pressure rather than cerebral perfusion pressure (CPP), is widely used to assess cerebrovascular resistance. However, it does not accurately reflect the pressure-flow/velocity relationships in the cerebral circulation, as cerebral blood flow (CBF) is primarily regulated by CPP. This study evaluated the validity of CVRi during orthostatic stress induced by head-up tilt (HUT), which alters CPP independently of arterial blood pressure at the heart level and CBF. Twenty young, healthy participants (aged

The Neurophysiological Effects of Cervical Transcutaneous Spinal Cord Stimulation With and Without a High Frequency Carrier in Able-Bodied Adults.

BACKGROUND: Transcutaneous spinal cord stimulation (tSCS) is a promising avenue in spinal cord injury (SCI) rehabilitation; however, high currents are required to excite afferents in the spinal cord roots, which patients may not tolerate. Modulating tSCS pulses with a kHz carrier frequency (kHz-tSCS) may be used to reduce discomfort; however, the way that kHz-tSCS interacts with neural networks, compared to unmodulated pulses (conv-tSCS), is largely unknown. METHOD: Ten able-bodied participants received conv-tSCS, kHz-tSCS, and sham interventions for 20 min over the C7/T1 vertebrae. Charge de
