Impact of gestational hyperglycemia on maternal and child health.

PURPOSE OF REVIEW: To address the recent evidence regarding the association between hyperglycemia during pregnancy and adverse short-term and long-term outcome for both mothers and offspring. RECENT FINDINGS: Recent data suggest a relationship between hyperglycemia during pregnancy and adverse short-term fetal outcomes, mainly those associated with excessive fetal growth. The degree of hyperglycemia plays an important role in risk stratification. Moreover, the long-term effect of hyperglycemia during pregnancy is expressed mainly as cardiometabolic morbidity and increased risk for the develop

Ambient temperature during gestation and cold-related adult mortality in a Swedish cohort, 1915-2002.

For all climatic regions, mortality due to cold exceeds mortality due to heat. A separate line of research indicates that season of birth predicts lifespan after age 50. This and other literature implies the hypothesis that ambient temperature during gestation may influence cold-related adult mortality. We use data on over 13,500 Swedes from the Uppsala Birth Cohort Study to test whether cold-related mortality in adulthood varies positively with unusually benign ambient temperature during gestation. We linked daily thermometer temperatures in Uppsala, Sweden (1915-2002) to subjects beginning a

Maternal and fetal factors which affect fetometry: use of in vitro fertilization and birth register data.

BACKGROUND: Fetometry dating of gestational age is the gold standard in most developed countries but may have some inborn errors. Dating pregnancies after in vitro fertilization can be used for the evaluation of fetometric studies and for studies of variables which may affect them. METHODS: We compared the actual gestational age of 9543 singleton and 869 twin pregnancies with estimates based on second-trimester fetometry. Mean gestational age, percentage of births classified as preterm, and skewness of the distribution of differences between actual and estimated gestational age were studied.

Excessive early gestational weight gain and risk of gestational diabetes mellitus in nulliparous women.

OBJECTIVE: To estimate whether there is an association between excessive early gestational weight gain and the development of gestational diabetes mellitus (GDM) and excessive fetal growth. METHODS: This is a secondary analysis of a randomized controlled trial of vitamins C and E in nulliparous low-risk women. Maternal weight gain from prepregnancy (self-reported) to 15-18 weeks of gestation was measured, and expected gestational weight gain was determined using the Institute of Medicine 2009 guidelines for each prepregnancy body mass index category. Excessive early gestational weight gain wa

Use of fenofibrate during the first trimester of unplanned pregnancy in a patient with hypertriglyceridemia.

OBJECTIVE: To describe a successful pregnancy in a patient who used fenofibrate during the first trimester. CASE SUMMARY: A 30-year-old female with a history of polycystic ovarian syndrome was referred to our department for evaluation of hypertriglyceridemia, and fenofibrate treatment was initiated. After 1 year of therapy, a gynecologic visit for menstrual cycle abnormalities revealed an unplanned pregnancy at 8 weeks of gestation (dating from the first day of the last menstrual period). Fenofibrate was discontinued and the pregnancy was continued despite the potential risks involved with th

No "masculinization" effect of a male on birth weight of its female co-twin.

OBJECTIVE: To test the hypothesis that the presence of a male affects birth weight of its female co-twin. PATIENTS AND METHODS: We evaluated a large population dataset of bichorionic twins after exclusion of potential confounders and after controlling for parity and gestational age. We tested the hypotheses that males (M) are heavier than females (F) irrespective of gender mix, parity and gestational age, and the hypothesis that birth weight of twins might be influenced by the gender of the co-twin. RESULTS: There were 819 MM pairs, 777 FF pairs and 1097 MF sets, for a total of 2735 males an

Nonpulmonary outcomes of asbestos exposure.

The adverse pulmonary effects of asbestos are well accepted in scientific circles. However, the extrapulmonary consequences of asbestos exposure are not as clearly defined. In this review the potential for asbestos to produce diseases of the peritoneum, immune, gastrointestinal (GIT), and reproductive systems are explored as evidenced in published, peer-reviewed literature. Several hundred epidemiological, in vivo, and in vitro publications analyzing the extrapulmonary effects of asbestos were used as sources to arrive at the conclusions and to establish areas needing further study. In order t

Diffusion tensor imaging of subependymal heterotopia.

A magnetic resonance (MR) diffusion tensor imaging (DTI) study was performed in a newborn with bilateral subependymal heterotopia (SE). White matter fractional anisotropy (FA), axial diffusivity (AD) and radial diffusivity (RD) were compared to values obtained in four newborns with moderate perinatal asphyxia and normal MRI findings. The reduction of FA and increase of AD and RD in the newborn with SE were the in vivo late expression of alterations in the intermediate zone, with an underlying arrest of neuronal migration.

Neurodevelopment following fetal growth restriction and its relationship with antepartum parameters of placental dysfunction.

Placental dysfunction leading to fetal growth restriction (FGR) is an important risk factor for neurodevelopmental delay. Recent observations clarify that FGR evolves prenatally from a preclinical phase of abnormal nutrient and endocrine milieu to a clinical phase that differs in characteristics in preterm and term pregnancies. Relating childhood neurodevelopment to these prenatal characteristics offers potential advantages in identifying mechanisms and timing of critical insults. Based on available studies, lagging head circumference, overall degree of FGR, gestational age, and umbilical arte

A new method of prenatal alcohol classification accounting for dose, pattern and timing of exposure: improving our ability to examine fetal effects from low to moderate alcohol.

BACKGROUND: When examining the association between prenatal alcohol exposure and fetal effects, the timing and intensity of exposure have been ignored in epidemiological studies. The effect of using dose, pattern and timing of consumption ("composite" method) was investigated in this study, to examine the association between prenatal alcohol exposure and fetal effects. METHODS: The composite method resulted in six categories of exposure (abstinent, low, moderate, binge <weekly, binge 1-2×/week and heavy). The odds of language delay and child behaviour problems were calculated for the composit

Age and pattern of the onset of differential growth among growth plates in rats.

Differential growth is the phenomenon whereby growth plates in the same individual at the same time all have uniquely different axial growth velocities. Differential growth is clearly present in the adolescent skeleton. In this study we ask two questions. When and by what pattern does the phenomenon of differential growth begin? Second, to what extent are the development of differential growth velocities correlated with changes in hypertrophic chondrocyte volume and/or with changes in chondrocytic production/turnover? Four growth plates (proximal and distal radial; proximal and distal tibial)

Developmental programming of hypertension: insight from animal models of nutritional manipulation.

Hypertension is a multi-factorial disorder thought to result from both genetic and environmental factors. Epidemiological studies suggest that cardiovascular diseases such as hypertension may be programmed in-utero. Experimental studies demonstrate that developmental programming occurs in response to a nutritional insult during fetal life and leads to slow fetal growth and permanent structural and pathophysiological changes that result in the development of hypertension and cardiovascular disease. A reduction in nephron number, hyperfiltration and increased susceptibility to renal injury, acti

Modeling genetic and environmental factors to increase heritability and ease the identification of candidate genes for birth weight: a twin study.

Heritability estimates of birth weight have been inconsistent. Possible explanations are heritability changes during gestational age or the influence of covariates (e.g. chorionicity). The aim of this study was to model birth weights of twins across gestational age and to quantify the genetic and environmental components. We intended to reduce the common environmental variance to increase heritability and thereby the chance of identifying candidate genes influencing the genetic variance of birth weight. Perinatal data were obtained from 4232 live-born twin pairs from the East Flanders Prospect

The short- and long-term implications of maternal obesity on the mother and her offspring.

Obesity's increasing prevalence has reached epidemic proportions in the USA, with close to one-third of the adult population affected in 2000. Additionally, there is increasing prevalence of obesity in other industrialised areas of the world such as Europe. Of potentially more concern is the potential risks associated with obesity and related metabolic complications in the developing world. The maternal, fetal, peripartum and neonatal complications of obesity in pregnancy have far-reaching implications for both mother and offspring. Of alarming interest is the increasing rate of obesity among

Latest advances in understanding preeclampsia.

Preeclampsia is a relatively common pregnancy disorder that originates in the placenta and causes variable maternal and fetal problems. In the worst cases, it may threaten the survival of both mother and baby. We summarize recent work on the causes of preeclampsia, which reveals a new mode of maternal immune recognition of the fetus, relevant to the condition. The circulating factors derived from the placenta, which contributes to the clinical syndrome, are now better understood. This brief review on preeclampsia does not cover all aspects of this intriguing condition but focuses on some new a

Developmental toxicity potential of paclobutrazol in the rat.

Paclobutrazol is used as a fungicide and as a plant growth regulator, but there are few studies about its potential toxic effects. Experiments were conducted to determine whether this compound has adverse effects on the reproduction and development of offspring. The influence of 1.0 mg/kg paclobutrazol (10 x the acceptable daily intake-ADI), by oral exposure during gestational organogenesis of rats, was evaluated on maturational and behavioural aspects of offspring development. This dose did not promote evidence of maternal toxicity and the weight of gravid uterus, fetus, and ovary on days 16

Trace elements in human physiology and pathology. Copper.

Copper is a trace element, important for the function of many cellular enzymes. Copper ions can adopt distinct redox states oxidized Cu(II) or reduced (I), allowing the metal to play a pivotal role in cell physiology as a catalytic cofactor in the redox chemistry of enzymes, mitochondrial respiration, iron absorption, free radical scavenging and elastin cross-linking. If present in excess, free copper ions can cause damage to cellular components and a delicate balance between the uptake and efflux of copper ions determines the amount of cellular copper. In biological systems, copper homeostasi
