Psychological mediators in chronic spinal pain after fully remote digital rehabilitation: a real-world, large-sample study.

IMPORTANCE: Psychological factors are associated with chronic spinal pain, yet their mediating role in postrehabilitation recovery remains poorly understood, particularly in fully remote digital care. Most research has focused on baseline predictors, with few studies evaluating psychological mediators and moderators. OBJECTIVE: The objective of this study was to investigate whether changes in fear avoidance beliefs, depression, and anxiety mediate pain outcome following a digital care program (DCP) for chronic spinal conditions and whether these effects vary by Body Mass Index (BMI), self-rep

Nonsurgical Facial Rejuvenation: Principles and Techniques in Aesthetic Plastic Surgery.

The increasing demand for minimally invasive facial rejuvenation has expanded the role of nonsurgical aesthetic procedures in plastic surgery. These treatments offer meaningful improvement in skin quality, texture, dyschromia, and photoaging while avoiding the morbidity and recovery associated with operative intervention. However, safe and effective treatment selection requires an understanding of skin biology, facial aesthetic principles, phototype, baseline barrier function, and the severity of photoaging. This review presents a practical framework for nonsurgical facial rejuvenation centere

Co-designing an interprofessional care pathway for (risk of) malnutrition and sarcopenia in community-dwelling older adults.

BACKGROUND: Integrating care for (risk of) malnutrition and sarcopenia in primary care is challenging, as limited physical proximity among healthcare professionals hinders collaboration. Both health conditions are common in community-dwelling older adults and are associated with significant declines in physical functioning, independence, and quality of life. Healthcare professionals tend to manage malnutrition and sarcopenia separately, leading to missed opportunities for early (risk) identification and coordinated care. An interprofessional care pathway can provide an evidence-based, structur

Latin American Consensus for the Diagnosis, Staging, and Treatment of Peripheral T-Cell Lymphoma Not Otherwise Specified.

PURPOSE: Peripheral T-cell lymphoma not otherwise specified (PTCL-NOS) is an aggressive, heterogeneous subtype of non-Hodgkin lymphoma with poor prognosis and limited therapeutic options. In Latin America (LATAM), management is challenged by restricted access to advanced diagnostics, higher prevalence of oncogenic viruses (Epstein-Barr virus, HTLV-1), and variable health care resources. This international consensus provides evidence-based, regionally adapted recommendations for diagnosis, staging, risk stratification, treatment, and follow-up of adult patients with PTCL-NOS. METHODS: A multid

Randomized trial of adjuvant 5-FU-platinum vs paclitaxel-platinum for high-risk penile cancer.

BACKGROUND: Adjuvant chemotherapy is recommended for high-risk penile squamous cell carcinoma (SCC) metastatic to lymph nodes based on retrospective data, but no randomized trials have been conducted. We compared 5-fluorouracil (5-FU)-platinum with paclitaxel-platinum in this setting. METHODS: In this open-label, randomized trial at an Indian tertiary cancer center, men <70 years with resected node-positive penile SCC and ≥1 high-risk feature (perinodal extension, pelvic node involvement, >1 inguinal nodes, or ≥ 4 cm node) were assigned 1:1 to four 3-weekly cycles of 5-FU-platinum or paclitax

Longitudinal immunogenicity cohort study of SARS-CoV-2 mRNA vaccines across individuals with different immunocompromising conditions: heterogeneity in the immune response and crucial role of Omicron-adapted booster doses.

BACKGROUND: Individuals with primary and secondary immunodeficiencies, being more susceptible to infections, are a priority for vaccination. Here, we determined and compared in a longitudinal study the immune response elicited by SARS-CoV-2 vaccination across different groups of individuals who are immunocompromised. METHODS: In the PatoVac_COV longitudinal prospective single-centre study, the spike-specific B cell and antibody responses to SARS-CoV-2 mRNA vaccination were compared across 5 different groups of individuals with haematological malignancies, hematopoietic stem cell (HCT) or soli

Family burden among US adults experiencing secondhand harms from alcohol, cannabis or other drugs.

BACKGROUND: Family burden has not been studied in relation to alcohol and other drug harms from others. We adapted a family burden scale from studies of caring for those with mental health conditions for use in the US Alcohol and Drug Harm to Others Survey (ADHTOS). We investigated associations between a seven-item summative burden scale and different types of harms attributed to someone else's use of alcohol, cannabis, or another drug: (a) being assaulted/physically harmed; (b) having family/partner problems; (c) feeling threatened or afraid; and (d) being emotionally hurt/neglected due to ot

Navigating Diagnostic Delays: A Case of Takayasu Arteritis Complicated by Acute Heart Failure and Vascular Compromise in a 37-Year-Old Woman From Cameroon.

Takayasu arteritis (TAK) is a rare form of large-vessel vasculitis that significantly impacts cardiovascular health, leading to severe complications such as acute heart failure (AHF). Its rarity in Cameroon may be attributed to limited awareness, underdiagnosis, and a lack of published data on the condition. We report the case of a 37-year-old female patient who presented with AHF and secondary hypertension, chest pain, and AHF after undergoing umbilical hernia repair two weeks prior. Initial symptoms included dyspnea at rest (New York Heart Association (NYHA) stage 4), orthopnea, paroxysmal n

A review of the scope of non-suicidal self-injury behavior in adolescents with depressive disorders: an analysis of related influencing factors.

BACKGROUND: Non-suicidal self-injury (NSSI) behavior is one of the major causes of death and disability in adolescents with depressive disorders, and this behavior has a high detection rate among adolescents globally, which seriously affects their physical and mental health and quality of life. Given the high prevalence and severity of NSSI behavior, this study provides a reference for the future construction of a risk-grading-based preventive intervention framework for NSSI in adolescents with depressive disorders by systematically sorting out the factors affecting non-suicidal self-injury.

Single-cell atlas of hepatitis C virus inoculated tree shrew liver reveals immune activation, metabolic reprogramming, and persistent inflammation.

BACKGROUND: Hepatitis C virus (HCV) remains a major global health challenge, largely due to the absence of robust animal models that recapitulate this disease, hindering mechanistic studies and vaccine development. This study employed tree shrews to determine the molecular profiles of distinct liver cell populations following HCV infection. METHODS: Tree shrews received intrahepatic injections of the HCV JFH1 strain and were analysed at two timepoints: 1 week post-inoculation (acute) and 111 weeks post-inoculation (long-term). Viral load was assessed up to 30 weeks. Liver pathology was assess

Effects of Probiotic Therapy on Periodontal and Peri-implant Treatments: An Umbrella Review.

INTRODUCTION: The effectiveness of nonsurgical periodontal treatment is related to patient- and tooth-related factors. To overcome the limitations of the conventional approach, probiotics are one of the adjunct therapies that have been studied. OBJECTIVES: This umbrella review answered the focused question: in adult patients with periodontal diseases or peri-implant diseases, does the use of probiotic therapy as an adjuvant to nonsurgical periodontal treatment when compared with nonsurgical periodontal treatment alone affect treatment effectiveness and clinical disease parameters? METHODS: A

Longitudinal trends and predictors of limitations in activities of daily living in community-dwelling older adults: evidence from the KLoSA study.

BACKGROUND: As life expectancy increases, the number of older adults with functional limitations is also increasing. Functional limitations are associated with adverse health outcomes such as reduced independence, diminished quality of life (QoL), and disability. Therefore, identifying which activities of daily living (ADLs) are limiting and understanding the influencing factors are crucial for developing tailored interventions. Although various factors influence ADL limitations, few studies have identified the longitudinal factors associated with each ADL. This study explores the longitudinal

Analyzing Subnational Immunization Coverage to Catch up and Reach the Unreached in Seven High-Priority Countries in the Eastern Mediterranean Region, 2019-2021.

Yearly national immunization coverage reporting does not measure performance at the subnational level throughout the year and conceals inequalities within countries. We analyzed subnational immunization coverage from seven high-priority countries in our region. We analyzed subnational, monthly immunization data from seven high-priority countries. Five were Gavi eligible (i.e., Afghanistan, Pakistan, Somalia, Syria, and Yemen); these are countries that according to their low income are eligible for support from the Global Alliance on Vaccine and Immunization, while Iraq and Jordan were included

The OPI•AID Zone Tool as a composite outcome for postoperative pain management quality-A protocol for an observational pilot study.

BACKGROUND: Managing postoperative pain while minimizing opioid-related adverse drug events (ORADEs) remains a significant challenge. The OPI•AID Zone Tool is proposed as a novel clinical decision support tool that - both graphically and in a scoring-system - represents the relationship between pain management and the occurrence of ORADEs, aiming to enhance patient outcomes in postoperative care. The OPI•AID Zone Tool places pain score on the x-axis and an ORADE score on the y-axis, and stratifies patients into five zones to reflect the composite impact of pain severity and ORADEs on the quali

Get Back, a person-centred digital programme targeting physical activity for patients undergoing spinal stenosis surgery-a study protocol of a randomized feasibility study.

BACKGROUND: Spinal stenosis is the most common reason for elective spine surgery, and the cardinal symptom is leg pain and discomfort when walking. Patients with spinal stenosis have a decreased level of physical activity and thereby an increased risk of poor health. Get Back is a person-centred digital programme that strives to support patients being physically active after surgery. The aim is to explore if Get Back, in its present format (referred to as Get Backfeasibility), is feasible and contributes to detectable change in variables related to intervention content. METHODS: Thirty patien
