Lactylation Dynamics and Its Regulatory Roles in Orthopedic Pathologies: A Research Update.

Post-translational modifications (PTMs) dynamically regulate cellular processes by modifying protein function. Among these, lactylation, a modification derived from lactate, functions through direct or indirect modification of histones or nonhistone proteins. While glycosylation and phosphorylation have established roles in bone metabolism and joint disorders, the biological significance of lactylation in musculoskeletal diseases remains underexplored. This study synthesizes current evidence investigating lactylation in four major orthopedic diseases: intervertebral disc degeneration (IVDD), o

A Pediatric Case of Vertebral Osteomyelitis Caused by Aggregatibacter aphrophilus: First to be Reported.

Pediatric cases of pyogenic vertebral osteomyelitis are rare; therefore, identifying the causative organism and selecting appropriate antimicrobial therapy can be challenging. In this report, we present the first pediatric case of vertebral osteomyelitis caused by Aggregatibacter aphrophilus, identified in an 11-year-old Japanese boy. The patient, who had no history of dental procedures, presented with intermittent fever and lower back pain that persisted for 50 days. A. aphrophilus was identified by the 16S rRNA gene sequencing of cultures of intervertebral disc biopsies and repeated blood sa

Deletion of Pax1 scoliosis-associated regulatory elements leads to a female-biased tail abnormality.

Adolescent idiopathic scoliosis (AIS), a sideways curvature of the spine, is sexually dimorphic, with increased incidence in females. A genome-wide association study identified a female-specific AIS susceptibility locus near the PAX1 gene. Here, we use mouse enhancer assays, three mouse enhancer knockouts, and subsequent phenotypic analyses to characterize this region. Using mouse enhancer assays, we characterize a sequence, PEC7, which overlaps the AIS-associated variant, and find it to be active in the tail tip and intervertebral disc. Removal of PEC7 or Xe1, a known sclerotome enhancer near

Construction of a nomogram for predicting the prognosis of patients with cervical spondylotic myelopathy: a retrospective study.

Cervical spondylotic myelopathy (CSM) is a neurological disorder characterized by degenerative changes in the spinal cord and compression of the spinal cord and its adjacent structures due to various reasons, such as intervertebral disc herniation. The Japan Orthopaedic Association score is a disease-specific outcome tool that provides quantitative measurements for CSM patients. At present, no scholars have developed a model that can directly predict the prognosis of CSM patients. This study used binary logistic regression analysis to identify clinical independent predictive factors, and then

Engineered extracellular vesicles as therapeutics of degenerative orthopedic diseases.

Degenerative orthopedic diseases, as a global public health problem, have made serious negative impact on patients' quality of life and socio-economic burden. Traditional treatments, including chemical drugs and surgical treatments, have obvious side effects and unsatisfactory efficacy. Therefore, biological therapy has become the focus of researches on degenerative orthopedic diseases. Extracellular vesicles (EVs), with superior properties of immunoregulatory, growth support, and drug delivery capabilities, have emerged as a new cell-free strategy for the treatment of many diseases, including

Surgical Management of Traumatic Cervicothoracic Junction Spondyloptosis Without Neurological Injury: A Case Report and Review of the Literature.

Acute traumatic cervical spondyloptosis in neurologically intact patients is uncommon and involvement of the cervicothoracic junction is rare. Herein, we report a case of traumatic C7-T1 spondyloptosis in a 56-year-old neurologically intact male patient, with radiographic findings of C7-T1 grade V traumatic listhesis associated with C7 floating segment, cord compression with myelomalacia, extensive ligamentum injury, and intervertebral disc traumatic change and protrusion. He underwent global spine fixation starting with a posterior approach. Follow-up at six months showed good outcomes. The p

Lumbar discal cyst and post-operative discal pseudocyst: a case series.

Four cases of lumbar cyst (2 discal cysts and 2 post-operative discal pseudocysts) who presented predominantly with features of sciatica without any motor, sensory or sphincteric disturbances. The patients were treated conservatively, and the management was aimed to avert any untoward surgical intervention taking into consideration patient safety and care. Two had previous lumbar decompressive discectomy. During the mean follow-up period of 13 months, there was progressive recovery of symptoms in all our 4 patients. All our patients were successfully managed by conservative approach. An interv

Radicular Pain Syndromes: Cervical, Lumbar, and Spinal Stenosis.

Back pain is a top primary and urgent care complaint; radicular pain can be caused by herniation of the nucleus pulposus (intervertebral disc), spinal stenosis, or degenerative changes to the vertebrae. The focus of this clinical review will be the clinical approach and treatment of lumbar radicular pain, cervical radicular pain, and spinal stenosis. Usually localized through neurological history, exam, and imaging, specific signs and symptoms for lumbar radicular, spinal stenosis, and cervical radicular pain can help determine etiology. Once radicular back pain has been diagnosed, a multitude

Failure of the human lumbar motion-segments resulting from anterior shear fatigue loading.

An in-vitro experiment was designed to investigate the mode of failure following shear fatigue loading of lumbar motion-segments. Human male lumbar motion-segments (age 32-42 years, n=6) were immersed in Ringer solution at 37°C and repeatedly loaded, using a modified materials testing machine. Fatigue loading consisted of a sinusoidal shear load from 0 N to 1,500 N (750 N±750 N) applied to the upper vertebra of the motion-segment, at a frequency of 5 Hz. During fatigue experiments, several failure events were observed in the dynamic creep curves. Post-test x-ray, CT and dissection revealed tha

Mediastinal vacuum phenomenon: atypical pneumomediastinum caused by gas replacement of diminished fat.

We report a case involving an 83-year-old man with interstitial lung disease who developed atypical pneumomediastinum caused by gas replacement of diminished fat. The patient presented with a complaint of worsening symptoms of respiratory difficulty since a diagnosis of chronic eosinophilic pneumonia 5 months back. He had been under observation with no particular treatment for 5 months. Computed tomography performed on admission revealed pneumomediastinum. When the current scan was compared with that obtained 5 months ago, it was evident that the fat surrounding the mediastinum had been replac

Interventional spine procedures in athletes.

Back pain is common in athletes and a source of missed time from practice and competition. Pain generators include muscle (strain), ligament (myofascial sprain and strain), intervertebral disc (herniation and degeneration), nerve (radiculopathy), joint (facet and sacroiliac (SI) joint), and bones (pars interarticularis defect). The goal of treatment of an athlete with back pain is to relieve symptoms and facilitate safe but rapid return to play with no change in performance. Initial conservative treatment includes relative rest, anti-inflammatory medications, and physical therapy. The use of i

Osteoclastogenesis and osteoclast activation in dialysis-related amyloid osteopathy.

Dialysis-related amyloid osteopathy (DRAO) is characterized by local osteoarticular lytic lesions, which sometimes cause a pathological fracture and reduce the quality of life in affected patients. In DRAO, active osteoclastic bone resorption is found at the bone surface facing the invaded synovial tissue and/or intervertebral disc, whereas reactive bone formation is absent. The eroded bone surface is covered with osteoclasts, suggesting the local promotion of osteoclastogenesis and osteoclast activation around DRAO. Inflammatory cells infiltrating the synovial tissue are likely to promote inf

Infra-red thermographic evaluation of spinal cord electrostimulation in patients with chronic pain after failed back surgery.

This study sought to visualize spinal-cord stimulation activity by infra-red thermography in humans suffering from chronic lumbosciatic pain. All the patients had previously undergone neurosurgery for a herniated intervertebral disc. Temperature changes were evaluated in two defined body areas after starting, stopping, maintaining or not starting the stimulation. In one body area, corresponding to the pain location, the patient experienced stimulation paraesthesia, whereas in the non-painful (second) area no stimulation paraesthesia were present. The patients were studied on four consecutive d

Anterior microsurgical approach for degenerative cervical disc disease.

One hundred-ninety-six patients with spinal cord or nerve root compression were operated on by anterior microsurgical discectomy without graft between 1978 and 1987. The patients presented either with spinal cord compression (33%) or radicular symptoms with a duration of 12 days to more than then years (66%). Investigations to confirm these clinical syndromes employed myelography, CT or MRI. Two-hundred-eighty-eight levels were explored; 124 one level, 54 two level, 17 three level and 2 four level operations were done.
