Splenic Peliosis as a Rare Cause of Spontaneous Splenic Rupture: A Case Report.

Atraumatic splenic rupture is a serious intraabdominal emergency that requires emergent intervention. This can be due to a number of causes. In this case report, we introduce a rare cause of atraumatic splenic rupture, which is an otherwise benign asymptomatic disease that only manifests clinically upon rupture, namely splenic peliosis. There is limited existing knowledge concerning the disease's etiology and diagnosis; however, this study presents the possible etiological explanations, associated risk factors, and possible radiologic diagnostic modalities.

Entry to new spiroheterocycles via tandem Rh(II)-catalyzed O-H insertion/base-promoted cyclization involving diazoarylidene succinimides.

A facile approach to novel medicinally relevant spiro heterocyclic scaffolds (namely furan-2(5H)-ones, tetrahydrofurans and pyrans spiro-conjugated with the succinimide ring) has been developed. The protocol consists of Rh(II)-catalyzed insertion of heterocyclic carbenes derived from diazoarylidene succinimides (DAS) into the O-H bond of propiolic/allenic acids or brominated alcohols, followed by base-promoted cyclization to afford the target spirocyclic compounds in good to high yields.

Alkaloid-Based Isoxazolylureas: Synthesis and Effect in Combination with Anticancer Drugs on C6 Rat Glioma Model Cells.

Alkaloid-based urea derivatives were produced with high yield through the reaction of anabasine and cytisine with isoxazolylphenylcarbamates in boiling benzene. Their antitumor activity, in combination with the commonly used five anticancer drugs, namely cyclophosphane, fluorouracil, etoposide, cisplatin, ribomustine with different mechanisms of action, was investigated. Based on the quantum chemical calculations data and molecular docking, hypotheses have been put forward to explain their mutual influence when affecting C6 rat glioma model cells.

Jean-Nicolas Marjolin (1780-1850): An ulcer, an anthrax, and a rose.

Jean-Nicolas Marjolin was a 19th century French anatomist and surgeon. Although not strictly speaking a pivotal figure in history of medicine, he deserves to be known for at least three reasons. He (more or less accurately) described a type of ulcer which is nowadays referred to as Marjolin's ulcer (1828); he had the privilege of operating on the world-famous Charles-Maurice de Talleyrand-Périgord for an anthrax (1838); and a rose has been named after him since 1860.

Superior Mesenteric Artery Syndrome in Systemic Lupus Erythematosus.

Although the gastrointestinal (GI) manifestations of systemic lupus erythematosus (SLE) are relatively less reported, they are common and occur in approximately half of individuals with SLE. These symptoms vary and include, but are not limited to, oral ulceration, dysphagia, nausea, vomiting, diarrhea, abdominal pain, and intestinal perforation. Gastrointestinal manifestations are often triggered by an inciting event, such as an infection or the side effects of medication. This case report presents a rare GI-related SLE complication, namely superior mesenteric artery syndrome.

[Disappearing bone disease of the shoulder (Gorham-Stout Syndrom)].

The spontaneous disappearance of a segment of bone in a short period without the existence of a tumoral disease that justifies it is an exceptional event. We report the case of an older adult in whom the upper epiphysis of the right humerus and part of the acromion disappeared in a period of 3 weeks in the absence of a malignant tumor. Bone biopsy revealed osteoclasts, lymphatic vessels, and blood vessels. This disorder was first reported in 1838 and in 1955 it was established as a specific disease by Gorham and Stout. Since then, it has been named in their honor.

[Ebola and Marburg virus disease].

Viral hemorrhagic fevers (VHF) are serious, often fatal diseases that affect humans and non-human primates. The nomenclature of these diseases has changed in that they are now referred to as viral diseases because the previously named symptoms of fever or hemorrhages are not obligatory. In this article, the focus will be on the VHFs Ebola and Marburg viral disease with the potential for human-to-human transmission; these diseases are so-called high-consequence infectious diseases (HCID), some with considerable potential for epidemic spread and the risk of nosocomial transmission.

Carotid Sources of Stroke.

The ancient Greeks used the term karotides for the great arteries in the neck, compression of which could plunge the victim into karoun: deep sleep or stupor. The artery name aside, studies of the patterns of brain infarction and their clinical severity have been an unbroken chain of growing sophistication in the past 150 years. Two main patterns emerged: perfusion failure with high-convexity infarction from hemodynamically important ipsilateral carotid stenosis or embolism from nonstenosing carotid plaque.

Anti-MDA5 antibody-positive dermatomyositis with mild encephalopathy with reversible splenial lesion: a possible rare association?

Central nervous system (CNS) involvement in dermatomyositis (DM) is seldom observed. However, there are very rare case reports of CNS involvement with juvenile dermatomyositis. Encephalopathy in DM may occur for a number of reasons, such as cerebral vasculitis and hypoperfusion/hypertensive encephalopathy, but mostly as a consequence of immunosuppressant treatment. We report here for the first time the case of a patient with two rare diseases, namely anti-MDA5 antibody-positive dermatomyositis and mild encephalopathy with reversible splenial lesion (MERS).

Primary intramedullary spinal-cord lymphoma (PISCL): a rare entity with a challenging diagnosis.

Primary intramedullary spinal-cord lymphoma (PISCL) is a rare cause of myelopathy and constitutes only 1% of central nervous system lymphomas. Delay to diagnosis is common due to its rarity, its similarity to other causes of myelopathy and the difficulties in obtaining pathological diagnosis. We report a case of PISCL and discuss the challenges faced on diagnosis, namely the impact of corticosteroids on histological findings, the usefulness of MRI, positron-emission tomography/CT (PET/CT) and repeated lumbar punctures.

Copper-Catalyzed 1,1-Boroalkylation of Terminal Alkynes: Access to Alkenylboronates via a Three-Component Reaction.

A copper-catalyzed three-component reaction of terminal alkynes, diazo compounds, and B2pin2 to prepare trisubstituted alkenylboronates has been developed. This difunctionalization of alkynes selectively occurs at the terminal carbon atom and proceeds via a tandem sequence. The copper catalyst plays dual roles in the whole process, namely, the initial copper-catalyzed cross-coupling and the following copper-catalyzed stereoselective boration reaction. Typically, different carbene precursors selectively lead to Z- and E-alkenes.

Magnetic Resonance Imaging patterns in central nervous system lymphomas: A pictorial review.

Five distinctive Magnetic Resonance Imaging (MRI) patterns of Central Nervous System Lymphomas (CNSL) are introduced in this pictorial essay - in an attempt to differentiate lymphoma from other abnormalities with similar MRI appearance, - namely a. solitary supratentorial, b. multiple supratentorial, c. infratentorial, d. intravascular, and e. extraparencymal. Recognition of a specific imaging pattern on brain MR imaging, may facilitate the early diagnosis and prompt treatment initiation, thus improving prognosis of brain lymphoma.

Lymphangitis-Associated Rickettsiosis by Rickettsia sibirica mongolitimonae.

Rickettsia sibirica mongolitimonae is considered a rare pathogen that can cause different clinical presentations. Approximately, one-third of the patients with this infection experience lymphangitis from the inoculation eschar to the draining lymph nodes, and, in that case, the infection is named "lymphangitis-associated rickettsiosis" (LAR). There are several reports of infections by this Rickettsia but none of LAR in children. We report a case of LAR in a Spanish child, which confirms the distribution of this agent in our country, and his implication in pediatric population.

Adverse drug reactions in dentistry.

An adverse drug reaction (ADR) is an undesirable effect of a drug. ADRs are possible with any medication that is prescribed or administered in the dental office. While most pharmacological agents in use today have favourable drug profiles and are relatively safe, the prudent clinician must be aware of the potential ADRs that can occur and be prepared to manage any complications. Here we review the most commonly used agents in dentistry, namely local anaesthetics, sedatives, analgesics and antibiotics, and their ADRs and management.

Polymyalgia Rheumatica (PMR) and Lung Involvement: The Forgotten Association.

Polymyalgia rheumatica (PMR) is an inflammatory condition of unknown etiology, commonly occurring in the elderly and is associated with a good prognosis. Patients usually present with pain in the neck, shoulders, and hips. The onset is often abrupt and is associated with depression and flu-like symptoms. Lung involvement in patients with PMR is unusual. Here we report a rare case of a 66-year-old man who presented with clinical features of PMR and respiratory symptoms, namely exertional dyspnea and dry cough.

Cuddling Teeth: A New Terminology.

Literature is replete with information on the diverse patterns exhibited by impacted teeth and a clinical scenario in which >1 impacted teeth appears in close proximity is documented very rarely. Occasionally, molars are found to be impacted in interesting patterns which were described previously as "rosette teeth" or "kissing molars". This article intends to introduce a new term, namely, "cuddling teeth" to broadly describe the clinical presentations where 2 or >2 impacted teeth are found in close approximation.

[Analysis of the way of thinking on pharmacology in Lu Zhiyi's Bencao Chengya Banji].

More than 410 kinds of prescriptions of medicines were presented, some of which were collected in Shennong Bencao Jing(, Shennong's Classic of Materia Medica) and other herbal works, are included in Lu Zhiyi's Bencao Chengya Banji(). He introduced the indications of these medicines by focusing on the name of the herbs, or the species of them, and the features and growth morphology of the herbs. He abstracted the efficacy of drugs on the human body. Therefore, he noted the indications of all the medicines list in the book.

Dual-energy CT characterization of winter sports injuries.

CT is a readily available imaging modality for cross-sectional characterization of acute musculoskeletal injuries in trauma. Dual-energy CT provides several additional benefits over conventional CT, namely assessment for bone marrow edema, metal artifact reduction, and enhanced assessment of ligamentous injuries. Winter sports such as skiing, snowboarding, and skating can result in high speed and high energy injury mechanisms; dual-energy CT is well suited for the characterization of those injuries.

Poland syndrome before Alfred Poland: the oldest medical description (Paris, France, 1803).

Here, we present a description of Poland syndrome from the second session of the Anatomical Society (Paris, France) on 11 December 1803 of congenital mammary absence and muscular atrophy on the right side. This case report predates the first official description of the disease published by Alfred Poland in Guy's Hospital Reports (London, 1841). Consequently, perhaps would it be necessary to do justice to its French discoverer, and to name from now on this nosological entity the "syndrome of Marandel"?

Endothelium-dependent Hyperpolarization-Mediated Relaxation Pathway in Bovine Mesenteric Lymph Nodes.

Endothelium-dependent relaxation mechanisms have been studied in phenylephrine-precontracted capsules of bovine mesenteric lymph nodes studied in vitro. Tetraethylammonium chloride and TRAM-34 in a solution with L-NAME and Indomethacin, which suppress the production NO and prostacyclin of endothelium, increased the tone of the lymph nodes. We believe that in bovine mesenteric lymph nodes, the dilation mechanism is mediated by hyperpolarization of the endothelium, which is associated with activation of large- and intermedium conductance Ca2+-activated potassium channels.
