Prognostic significance of the symptoms associated with prostatic carcinoma.

This paper discusses the prognostic significance of the symptoms of prostatic carcinoma. In a series of 123 patients, 72 presented with prostatism, of which 11% died with progressive malignant disease. Of 36 patients with retention of urine, 30% died with progressive malignant disease, and of 15 patients with a combination of skeletal pain and prostatism, 53% died of progressive malignant disease. Skeletal pain is a very poor prognostic factor.

Computed tomographic scanning of the mediastinum in the staging of bronchogenic carcinoma.

We investigated the efficacy of 2-s breath-holding computed tomographic (CT) scans and standard posteroanterior and lateral chest roentgenograms in staging the mediastinum and pulmonary hill in lung cancer. Fifty-one comparisons were made in 49 patients thought to have non-small-cell carcinoma, consecutively chosen to be free of disseminated tumor, and to be suitable candidates for thoracotomy. The CT scans accurately predicted mediastinal neoplastic lymphadenopathy in 15 or 17 instances of proved mediastinal lymph node metastasis for a sensitivity of 88%; specificity was also high (94%) with

Anterior resection for adenocarcinoma of the distal large bowel.

Among 400 patients with adenocarcinoma of the distal large bowel, anterior resection resulted in more anastomotic leaks, postoperative urinary retention and diarrhea when used for lesions of the mid-rectum than when used for lesions of the proximal rectum or sigmoid colon. However, the operative mortality, long-term morbidity and 2 year survival were similar among patients with lesions at all three locations.

[Update on the regression grading of non-small cell lung cancer].

Immune checkpoint inhibitors combined with conventional chemotherapy have led to significantly improved outcomes in the neoadjuvant and perioperative treatment of non-small cell lung cancer (NSCLC), establishing chemoimmunotherapy as the standard of care for patients in UICC stages IIA-IIIA. As a result, pathological assessment of therapy-induced tumor response has become essential. This article provides an overview of the current recommendations for macroscopic handling, histological evaluation, and staging of NSCLC resection specimens after neoadjuvant treatment. Key aspects include comprehe

Imaging in Upper Tract Urothelial Carcinoma: Diagnostic Accuracy of Current Modalities for Local Tumor Staging.

PURPOSE OF REVIEW: To summarize current imaging modalities for upper tract urothelial carcinoma (UTUC) and evaluate their diagnostic performance for tumor staging and risk stratification. RECENT FINDINGS: CT urography remains the diagnostic standard with excellent accuracy for tumor detection, though its reliability for local staging is limited. MR urography shows promise for assessing muscle invasion but is operator dependent. FDG-PET demonstrates value in nodal and metastatic staging rather than primary tumor evaluation. Retrograde pyelography maintains a role in tumor diagnosis in circumst

Magnetic Resonance Imaging After Total Neoadjuvant Therapy in Rectal Cancer: Treatment-Oriented Response Assessment and Reporting for Watch-and-Wait Management.

Total neoadjuvant therapy (TNT) is increasingly recognized as a standard treatment option for locally advanced rectal cancer. By increasing rates of clinical complete response (cCR) and thereby expanding opportunities for nonoperative management, TNT has elevated the role of magnetic resonance imaging (MRI) in assessment of rectal cancer treatment response. Restaging MRI is shifting away from reliance on tumor regression grading systems, which have demonstrated limited correlation with pathologic findings, toward more clinically relevant assessments integrated with flexible endoscopy. The iden

De-escalating radical surgery for select FIGO Stage I cervical cancer.

Four recent prospective trials demonstrated that de-escalation of radical surgery for select patients with apparent low-risk, FIGO Stage I cervical cancer does not compromise cancer-related survival but does improve quality of life and provide the opportunity to preserve fertility. We present a critical review of ConCerv, SHAPE, GOG-278, and LESSER, which have set the stage for de-escalation of surgery for low-risk, early-stage cervical cancer. ConCerv, a prospective phase II study evaluating the feasibility and oncologic outcomes of conization alone or simple hysterectomy in patients with ear

Stage-specific associations of HER2 overexpression with clinical and pathological features in bladder cancer: a systematic review and meta-analysis.

BACKGROUND: Human epidermal growth factor receptor 2 (HER2) overexpression is an established oncogenic driver and therapeutic target in several malignancies, but its clinical significance in urothelial carcinoma (UC) of the bladder remains controversial due to disease heterogeneity and lack of standardized assessment. OBJECTIVE: To synthesize evidence on the stage-specific associations of HER2 overexpression with clinicopathological features in bladder UC through a systematic review and meta-analysis. METHODS: We conducted a PRISMA-compliant systematic review and meta-analysis, searching Pub

Imaging in Advanced Epithelial Ovarian Cancer: Assessment of Peritoneal Spread.

Ovarian cancer affects over 250 000 women worldwide, with epithelial ovarian cancer accounting for approximately 90% of cases. Lack of effective screening and absence of early symptoms result in nearly 70% of patients presenting with late-stage disease. A distinct feature of advanced ovarian cancer is that peritoneal spread does not preclude curative surgical resection. Ovarian cancer is staged using the International Federation of Gynecology and Obstetrics, or FIGO, system, last updated in 2014. US is the first-line imaging modality for evaluating the symptomatic female pelvis, while MRI is t

Role of staging laparoscopy in gastric cancer management: Evaluation of occult disease and its influence on clinical decision making.

BACKGROUND: Accurate staging is critical in gastric cancer, as the presence of peritoneal metastasis directly influences treatment strategy and prognosis. Despite advances in cross-sectional imaging, occult peritoneal disease may remain undetected. Staging laparoscopy has been increasingly utilized to improve diagnostic accuracy; however, its true impact on clinical decision-making and the predictive factors for occult metastasis remain areas of ongoing investigation. METHODS: This retrospective study included patients with biopsy-proven gastric adenocarcinoma who underwent staging laparoscop

Impact of Multicancer Screening on Late-Stage Cancer at Diagnosis in Breast Cancer Survivors: A Modeling Study.

PURPOSE: Breast cancer survivors are at elevated risk of multiple new primary cancers compared with the general population. The impact of multicancer screening on late-stage cancer diagnosis is under clinical investigation in average-risk individuals, but it is unclear whether findings will generalize to breast cancer survivors. METHODS: We adapted an existing multicancer natural history model for the average-risk US population to reflect increased risks of second primary cancers and all-cause mortality among female breast cancer survivors. We modeled 16 cancer types other than breast cancer

Ten-year survival of women with invasive breast cancer by stage, molecular subtype, and grade combined: a population-based study.

BACKGROUND: Stage at diagnosis, histologic grade, and molecular subtypes are established prognostic indicators for women with breast cancer (BC), but their impact on population-based survival estimates is poorly documented. This study assessed long-term BC survival trends according to these factors. METHODS: We identified women aged <75 years diagnosed with stage I-IV invasive BC between 2004 and 2014 from the Friuli Venezia Giulia (north-eastern Italy) Cancer Registry (N = 10 467). Follow-up through 2023 was used to estimate overall and net survival (NS) according to combinations of prognost

[PROGNOSIS OF NEPHROURETERECTOMY FOR UPPER URINARY TRACT UROTHELIAL CARCINOMA].

(Objective) We retrospectively assessed prognostic factors following total nephroureterectomy for renal pelvis and ureter cancer. (Subjects and methods) Of the 291 patients who underwent total nephroureterectomy for renal pelvis and ureter cancer without distant metastasis between May 2005 and March 2022, we excluded those who underwent simultaneous total cystectomy, received presurgical chemotherapy, or had pN-positive status. The prognosis was evaluated based on metastasis-free survival (MFS) and overall survival (OS). Clinicopathological factors affecting prognosis were analyzed. (Results)

Siewert II esophagogastric junction cancer: total gastrectomy or esophagectomy?

BACKGROUND: The surgical approach for esophagogastric junction cancers (EJC), Siewert II, has been controversial regarding margin control, reconstruction, and lymphadenectomy extension. Therefore, predicting the need for total/subtotal esophagectomy and proximal gastrectomy (TEPG) or total gastrectomy with distal esophagectomy (TGDE) can be challenging, with each direction usually excluding the other. Historically, complication rates for TEPG are higher, affecting further systemic treatment and long-term outcomes. AIMS: The aim of this study was to describe a surgical strategy for approaching

Surgical Considerations for Patients With De Novo Metastatic Breast Cancer Based on Prognostic Group.

INTRODUCTION: Advances in systemic therapy for de novo metastatic breast cancer (dnMBC) led to the development of a novel staging system (IVA-D), which stratifies patients into prognostic subgroups based on tumor characteristics and metastatic extent. Overall survival (OS) varies between subgroups; therefore, different treatment approaches may be considered. This study explores the association of surgery of the primary tumor with OS among prognostic subgroups. METHODS: Patients with dnMBC (2010-2020) receiving systemic therapy were selected from the National Cancer Database (NCDB) and stratif
