Latency and duration of the muscle silent period following transcranial magnetic stimulation in multiple sclerosis, cerebral ischemia, and other upper motoneuron lesions.

In muscles with sustained voluntary contraction, EMG activity is transiently inhibited after transcranial magnetic stimulation. We recorded this postexcitatory silent period (SP) at 1.5 times individual stimulus threshold level from the first dorsal interosseus muscle in 65 neurologic patients aged 11 to 80 years. When compared with 20 healthy volunteers and a subgroup of patients with peripheral neurologic conditions not affecting the tested pathways, the SP was significantly longer on the paretic side in cerebral ischemia (p < 0.001) and chronic inflammatory CNS diseases, such as multiple sc

X-linked recessive bulbospinal neuronopathy (Kennedy's syndrome): a neurophysiological study.

We examined 8 men with X-linked recessive bulbospinal neuronopathy. Quantitative electromyography showed large amplitude motor unit action potentials of prolonged duration and increased polyphasia. There was a pronounced loss of motor units in all but one muscle at maximal volition even in muscles with normal or only mildly decreased force. Denervation activity was present in 53% sampled limb muscles, and fasciculation was recorded in 33% of limb muscles. Nerve conduction studies showed small amplitude sensory action potentials in 6 out of 8 patients. The motor and sensory conduction velocity

Median nerve electrophysiologic parameters and psychomotor performance in carpal tunnel syndrome.

Psychomotor performance (PMP) involving a repeated, rapid pinch and release task was correlated with median nerve electrophysiologic parameters for control subjects (16 hands) and subjects with carpal tunnel syndrome (CTS) (14 hands). The psychomotor task was used because of its functional resemblance to many work related activities. A strain gauge dynamometer was repeatedly pinched to a predetermined force level using the index finger and thumb and then released as rapidly as possible, while measuring the actual isometric force exerted. Discrete visual and auditory feedback was provided. Medi

Magnetic stimulation of the motor cortex in cervical spondylosis.

We report a new technique of transcranial magnetic stimulation of the motor cortex to measure conduction within central motor pathways of 67 patients with cervical spondylosis or disk herniation. There were upper motor neuron signs in 34 patients (51%) and x-ray evidence of cervical cord compression in 44 (66%). Muscle action potentials (MAPs) to cortical stimulation were abnormal in 84% of patients with, and 22% of those without, radiologic signs of cervical cord compression. Median nerve somatosensory evoked potentials were altered in only 25% of patients. The frequency of MAP alterations co

Conduction properties of epidurally recorded spinal cord potentials following lower limb stimulation in man.

Spinal somatosensory evoked potentials were recorded in 35 neurologically normal patients undergoing surgery for scoliosis. During posterior procedures the recording electrodes were placed in the dorsal epidural space and during anterior operations in the intervertebral discs. Stimulation was of the tibial nerve in the popliteal fossa and the posterior tibial and sural nerves at the ankle. At thoracic levels the response consisted of at least 3 components with different peripheral excitation thresholds and spinal conduction velocities (range 35-85 m/sec). All components were conducted mainly i

Sensory conduction in medial and lateral plantar nerves.

A simple and reliable method of recording medial and lateral plantar nerve sensory action potentials is described. Potentials are recorded with surface electrodes at the ankle using surface electrodes stimulating orthodromically at the sole. The normal values obtained are higher in amplitude than those obtained by the method described by Guiloff and Sherratt and are detectable in older subjects aged over 80 years. The procedure is valuable in the diagnosis of early peripheral neuropathy, mononeuritis multiplex, tarsal tunnel syndrome and in differentiation between pre and post ganglionic L5 S1

Short-latency somatosensory-evoked potentials from radial, median, ulnar, and peroneal nerve stimulation in the assessment of cervical spondylosis. Comparison with conventional electromyography.

A study of data on 30 patients with cervical spondylosis was carried out to determine whether short-latency somatosensory-evoked responses (SEPs) to median, ulnar, radial, and peroneal nerve stimulation provided additional information to that obtained by electromyography (EMG), late responses, and peripheral conduction studies. Peripheral studies, EMG results and SEPs were within normal limits in ten patients with pain, but without objective neurological deficit. By contrast, of ten patients who had objective signs of root compression, conventional EMG results were normal in nine, but abnormal

A study of the neurological disorder associated with acute haemorrhagic conjunctivitis due to enterovirus 70.

Ninety cases of the neurological manifestations associated with acute haemorrhagic conjunctivitis caused by Enterovirus 70 (EV 70) are reported. The patients were seen during the widespread epidemics in 1971 and 1981. Male adults were predominantly affected by a "polio-like" paralysis of the limbs and/or cranial nerves. Root pains were often complained of early in the disease. In the absence of a necropsy, clinical and neurophysiological examinations helped to localise the lesions. Significant antibody titres against EV 70 were demonstrated in the serum and more relevantly in the CSF. Though o

Ischaemic neuropathy in uraemic patients caused by bovine arteriovenous shunt.

A bovine shunt, inserted between the radial artery and cephalic vein in the upper arm for access during chronic haemodialysis, caused a distal, ischaemic neuropathy in two patients. There were electrophysiological signs of axonal degeneration of motor and sensory nerve fibres in the patient with severe ischaemia, and, of only sensory fibres in the patient with mild ischaemia. Thenar muscle blood flow studies using 133Xe were a useful measure of altered tissue perfusion. Recovery was incomplete in both cases, despite banding of the graft and improved tissue perfusion in the severe case.

Motor responses of the urinary bladder and skeletal muscle in botulinum intoxicated rats.

1. Type A or type D botulinum toxin administered to rats did not produce a generalized paralysis of skeletal muscles at the time of ventilatory arrest. However, if survival was extended by artificial ventilation complete blockade of neuromuscular transmission developed 6.5 hr after 100 MLD of type D and 5 hr after 1000 MLD of type A toxin. The onset of paralysis of a muscle was shortened by repetitive stimulation of the motor nerves.2. There was no consistent blockade of parasympathetically innervated viscera in animals dying after type A toxin. Animals given type D toxin displayed mydriasis a

Cross-Sectional Associations Between Hyperglycemia and Hearing Loss at the Year 35 Coronary Artery Risk Development in Young Adults Observational Cohort Study.

PURPOSE: Sustained hyperglycemia is associated with pathogenic effects on microvasculature and sensory neurons, and diabetes may also affect the vasculature and neural systems of the inner ear. We examined the association of hyperglycemia with sensorineural hearing loss (HL) at three frequency ranges in a biracial, population-based cohort. METHOD: Among 1,930 Black and White men and women aged 53-65 years who were attending the Year 35 examination of the Coronary Artery Risk Development in Young Adults study, we assessed hearing using an automated method for testing auditory sensitivity to ob

Ultrasound neuromodulation as a novel dementia therapy-Investigation of possible long-term confounds.

INTRODUCTION: Ultrasound neuromodulation has emerged as a promising adjunctive therapy in Alzheimer's disease (AD), yet a controversial issue remains: whether its reported long-term therapeutic effects could be attributed to potential confounds rather than genuine neuromodulatory mechanisms. Although auditory confounds via air- or bone-conducted sound have been discussed for immediate effects, their relevance for enduring therapeutic outcomes-essential for clinical application-remains unknown. This exploratory study is the first to examine whether long-term cognitive and neural effects of tran

Epicardial adipose tissue as a metabolically active visceral fat depot: an emerging target in cardiovascular disease.

BACKGROUND: Epicardial adipose tissue (EAT) is a metabolically and endocrinologically active visceral fat depot with unique anatomical proximity to the myocardium, and is increasingly recognized as relevant to cardiovascular health and disease. Its anatomical position, in direct contact with the myocardium and coronary arteries without an intervening fascial barrier, is consistent with paracrine and vasocrine signaling toward adjacent cardiac structures. OBJECTIVE: This review aims to provide a comprehensive analysis of EAT biology, from its physiological roles in lipid homeostasis, mechanica

Biological heart and brain ageing in subjects with cardiovascular diseases.

INTRODUCTION: The heart-brain axis hypothesis suggests a bidirectional connection between the brain and the heart with relevant implications in health and disease. Cardiovascular diseases have been empirically linked to an increased risk of neurological diseases. However, it remains unclear to what extent different cardiovascular diseases affect brain health quantitatively across subjects and if that is associated with the extent the heart is affected by a disease. Therefore, this study aims to explore how cardiovascular diseases affect biological ageing of the brain and heart by quantifying t

Genetic variation in the glucagon-like peptide-1 receptor protein encoding region is associated with higher heart rate variability, but not with neurodegeneration of the brain, retina, and peripheral nerves: The Maastricht study.

BACKGROUND: Degeneration of organ-specific nervous systems precede major clinical conditions such as heart failure, dementia, diabetic retinopathy, and diabetic neuropathy. Glucagon-like peptide-1 receptor (GLP1R) agonists may have neuroprotective effects, independent of their glucose-lowering effects, and as such may be tools for early prevention of neurodegeneration. We investigated whether genetic variation in the GLP1R protein encoding region, assessed using a genetic score, was associated with neurodegeneration measured in the heart, brain, retina, and peripheral nerves. METHODS: Data fr

Bimodal aphasia and dysgraphia: Phonological output buffer aphasia and orthographic output buffer dysgraphia in spoken and sign language.

We report a case of crossmodal bilingual aphasia-aphasia in two modalities, spoken and sign language-and dysgraphia in both writing and fingerspelling. The patient, Sunny, was a 42 year-old woman after a left temporo-parietal stroke, a speaker of Hebrew, Romanian, and English and an adult learner, daily user of Israeli Sign language (ISL). We assessed Sunny's spoken and sign languages using a comprehensive test battery of naming, reading, and repetition tasks, and also analysed her spontaneous-speech and sign. Her writing and fingerspelling were assessed using tasks of dictation, naming, and d

Cortical Correlates of the Auditory Frequency-Following and Onset Responses: EEG and fMRI Evidence.

UNLABELLED: The frequency-following response (FFR) is a measure of the brain's periodic sound encoding. It is of increasing importance for studying the human auditory nervous system due to numerous associations with auditory cognition and dysfunction. Although the FFR is widely interpreted as originating from brainstem nuclei, a recent study using MEG suggested that there is also a right-lateralized contribution from the auditory cortex at the fundamental frequency (Coffey et al., 2016b). Our objectives in the present work were to validate and better localize this result using a completely dif

Laser Therapy After Repair of the Distal Half of the Median Nerve; a Comparative Study.

BACKGROUND: Nerve injuries resulting from major or minor trauma often cause some disabilities for patients. Neurotmesis, characterized by complete anatomical rupture of the nerve, is the most severe form of the injury which will not recover without reconstructive surgery and nowadays such neural damages are improved by microsurgical procedures. Some studies have used low power laser for nerve cell growth in order to improve the rehabilitation results of peripheral nerves. Low power laser can complement the reformation of postsurgical nerve injuries. OBJECTIVES: The current study aimed to asse
