T Cell-Mediated Nasal Hyperresponsiveness in Allergic Rhinitis.

Allergic rhinitis patients suffer various symptoms such as sneezing, runny nose, and nasal congestion. As disease severity and chronicity progress, nasal hyperresponsiveness (NHR) develops in those patients. During the generation of a mouse allergic rhinitis model, we discovered that immunized mice developed NHR upon repeated nasal antigen challenge. Using genetically modified mice and an originally developed T cell-transferred mouse model, we confirmed the critical role of CD4+ T cells after differentiation into several helper subsets in NHR. On the other hand, immunoglobulin E/mast cell-depe

Total nasal reconstruction: a review of the past and present, with a peak into the future.

PURPOSE OF REVIEW: The goal of this article is to review the complex topic of total nasal reconstruction and present a wide range of options for completing this difficult surgical procedure. RECENT FINDINGS: Nasal reconstruction has a long history dating back thousands of years. Some historical techniques still exist today, including the paramedian forehead flap. The introduction of free tissue transfer and other pedicled flaps has provided multiple options for the reconstructive surgeon. The future of nasal reconstruction will include facial transplantation and likely bioengineered tissues.

Complications in Reductive Profileplasty.

The ability of a surgeon to create an aesthetically pleasing and functionally competent nasal dorsum weighs heavily on following the foundations of rhinoplasty, beginning with the examination and analysis. These same fundamental principles should be used and applied in secondary rhinoplasty. The most common chief complaints of those patients seeking primary rhinoplasty relates to the presence of a hump or dorsal convexity, and similarly complications surrounding the dorsum are the most common indications for secondary rhinoplasty. The surgeon must be able to appropriately evaluate and correct

Intractable Epistaxis: Looking Beyond the Usual Sites.

PURPOSE: Intractable epistaxis is a challenge to manage and often requires multiple interventions, both medical and surgical. After anterior and posterior packing, many options exist on how to manage the patient. PATIENTS AND METHODS: We performed a retrospective chart review of patients who presented to a private ear, nose, and throat practice with intractable epistaxis from January 2010 until July 2015. The clinical notes were reviewed, noting the demographic characteristics of the patients and the treatment plan that followed. In particular, we determined the site of bleeding that was docu

Nostril mass caused by Leishmania spp. in a dog.

Canine leishmaniasis (CanL) is a systemic zoonotic disease caused by the protozoan Leishmania, an intracellular macrophage parasite, transmitted by the bite of phlebotomine sandflies. In dogs, the clinical disease is mostly characterised by symptoms associated with viscerocutaneous lesions such as lymphadenopathy, splenomegaly, skin lesions, and renal and ocular disease caused by the deposition of immune complexes. The parasite may provoke mucosal lesions which cause atypical clinical signs. The aim of this study is to describe an atypical nostril mass in a dog infected by Leishmania. Clinical

Epistaxis: Outpatient Management.

Epistaxis is a common emergency encountered by primary care physicians. Up to 60% of the general population experience epistaxis, and 6% seek medical attention for it. More than 90% of cases arise from the anterior nasal circulation, and most treatments can be easily performed in the outpatient setting. Evaluation of a patient presenting with epistaxis should begin with assessment of vital signs, mental status, and airway patency. When examining the nose, a nasal speculum and a good light source, such as a headlamp, can be useful. Compressive therapy is the first step to controlling anterior e

A Case of Glomangiopericytoma at the Nasal Septum.

Glomangiopericytoma (GPC) is a rare sinonasal perivascular tumor that accounts for < 0.5-1% of all sinonasal tumors. GPC is categorized as a low-grade neoplasm with borderline malignancy and a tendency of local recurrence. GPC is a rare mesenchymal neoplasm characterized by the perivascular proliferation of tumor cells, and it requires being distinguished from solitary fibrous tumors. Here, we report a case of GPC in a 68-year-old male patient who presented at the emergency room of our hospital with a complaint of sudden epistaxis. A small, reddish, protruding tumor was observed on the right n

Nasal Valve Collapse: Our Treatment Protocol.

The nasal valve area is the functional area that allows airflow regulation. It could be divided in an internal area and an external one, basing on the anatomical landmarks. Many conditions can damage these areas with a consequently nasal obstruction, more often in particular extended rhinoplasty and sequels of facial trauma. To restore this area many techniques were advocated during the last decades. In this article the authors investigate on valve areas deficiency in sequel of rhinoplasty proposing a structural approach through alar batten grafts to correct internal-external nasal valves colla

Disorders of the neonatal nasal cavity: Fundamentals for practice.

Neonatal nasal obstruction is a well-known clinical entity. Fortunately, it is rarely life-threatening and usually resolves with conservative management. As with most conditions, a systematic history and thorough physical examination are crucial for correct diagnosis and management. The initial diagnosis may be elusive and require either serial or more in-depth evaluations. Occasionally, examination may reveal structural abnormalities necessitating surgical intervention. Fortunately most of these abnormalities are amenable to surgery; however, a select few are notoriously difficult to treat.

Lacaziosis-like disease in Tursiops truncatus from Brazil: a histopathological and immunohistochemical approach.

Cetacean lacaziosis-like disease or lobomycosis-like disease (LLD) is a chronic skin condition caused by a non-cultivable yeast of the order Onygenales, which also includes Lacazia loboi, as well as Paracoccidioides brasiliensis and P. lutzii, respectively responsible for lacaziosis and paracoccidioidomycosis in humans. Complete identification and phylogenetic classification of the LLD etiological agent still needs to be elucidated, but preliminary phylogenetic analyses have shown a closer relationship of the LLD agent to Paracoccidioides spp. than to L. loboi. Cases of LLD in South American c

Genetic and molecular alterations in olfactory neuroblastoma: implications for pathogenesis, prognosis and treatment.

UNLABELLED: This study examines the potential of alternative micro-geographic units of analysis compared to the widely used rectangular grid for predicting (monthly) micro-geographic crime risks using a machine learning approach. Specifically, this study compares the prediction performance of machine learning models (XGBoost) in deriving monthly micro-geographic risk predictions for three crime types across rectangular and hexagonal grids of varying resolutions, as well as street segments, using the hit rate, precision, and F1-score as key performance measures. Police-registered data on reside

Evaluation of surgical freedom for microscopic and endoscopic transsphenoidal approaches to the sella.

BACKGROUND: Microscopic and endoscopic transsphenoidal approaches to the sellar are well established. Surgical freedom is an important skull base principle that can be measured objectively and used to compare approaches. OBJECTIVE: To compare the surgical freedom of 4 transsphenoidal approaches to the sella turcica to aid in surgical approach selection. METHODS: Four transsphenoidal approaches to the sella were performed on 8 silicon-injected cadaveric heads. Surgical freedom was determined with stereotactic image guidance using previously established techniques. The results are presented as

Nasal Eosinophilic Angiocentric Fibrosis with Orbital Extension.

Eosinophilic angiocentric fibrosis (EAF) is an extremely rare, chronic, benign, idiopathic disorder that mostly affects the upper respiratory tract, particularly the nasal cavity, and features progressive submucosal perivascular fibrosis. To the best of our knowledge, only seven cases of EAF with orbital involvement have been reported. We report a case of sinonasal EAF with orbital extension that presented with left nasolacrimal duct obstruction. A 35-year-old man presented with left epiphora, proptosis, anterolateral globe displacement and nasal obstruction. Endoscopic sinus examination showe
