Lip incompetence resolved by active vertical control in nonsurgical treatment of a protrusion case with vertical maxillary excess.

In this case report, we present the treatment of a 28-year-old patient with lip incompetence and vertical maxillary excess (VME), using a combination of a midpalatal miniscrew-anchored cantilever clip appliance and submerged buccal shelf miniscrews. The patient exhibited a convex profile, long face, gummy smile, and protrusion, with a Class II skeletal relationship and mentalis strain. The patient declined conventional orthognathic surgery, leading to an orthodontic camouflage treatment plan involving extraction of four first premolars, maximum retraction, and active vertical control with skel

Central Giant Cell Granuloma: Diagnosis and Treatment in a Multidisciplinary Setting.

A 7-year-old girl presented with painless swelling in the anterior region of the mandible as well as buccolingual expansion and spacing between the incisors. Imaging showed a well-defined, multilocular radiolucent lesion with cortical perforation, displacement of the canine tooth bud and root resorption of adjacent incisors. Diagnosis of central giant cell granuloma (CGCG) was confirmed by incisional biopsy and histopathology. Management involved a multidisciplinary team, including oral and maxillofacial surgery, pediatric dentistry, prosthodontics and orthodontics. Surgical enucleation with p

[Interdental distraction osteogenesis: a retrospective case study].

Skeletal Class II malocclusion is the most common indication for combined orthodontic-orthognathic treatment. Interdental distraction osteogenesis can be applied during the pubertal growth spurt with the aim of avoiding orthognathic surgery at a later age. This retrospective case study included 20 patients with mandibular retrognathia for whom orthodontic treatment with functional appliances had yielded insufficient results and who were subsequently treated with interdental distraction osteogenesis. A Class I occlusion was achieved in all patients. The average mandibular length gain was 9.6 mm

A Rare Case of Bilateral Mesiodens With Dens Invaginatus Obstructing Permanent Incisor Eruption.

The co-occurrence of mesiodens and dens invaginatus (DI) is very uncommon. Such anomalies may impede the eruption of permanent maxillary central incisors and can appear radiographically similar to normally developing teeth. An 11-year-old girl presented with failure of eruption of the permanent maxillary central incisors. Panoramic radiography and cone-beam computed tomography (CBCT) revealed two palatobuccally positioned supernumerary mesiodens, both of which exhibited DI. Surgical extraction of the mesiodens was performed, and guided bone regeneration (GBR) was carried out to preserve the al

A Novel Interdisciplinary Approach for the Management of Micrognathia (Brodie Syndrome).

This study describes a multidisciplinary approach for correcting a severe Class II malocclusion with reduced mandibular transverse dimension. A 35-year-old woman presented with an increased overjet, complaining chiefly of forwardly placed upper front teeth and unpleasant smile esthetics. The patient had a convex profile with severe mentalis strain on lip closure. Intraoral examination indicated a bilateral Class II canine relationship, scissor bite, and narrow lower arch. The skull lateral view assessment revealed a skeletal Class II relationship with reduced lower facial height. According to

Slow maxillary expansion in adult patient with Hyrax expander: A case report.

Slow maxillary expansion is a technique used to achieve correction of maxillary transverse deficiency or posterior crossbite in patients where the midpalatal suture has closed. This is mostly achieved by buccal tipping of maxillary posterior teeth. The aim of this case report is to discuss the orthodontic treatment of a 20-year-old patient with bilateral posterior crossbite. The patient had moderate maxillary crowding and severe mandibular crowding, crossbites bilaterally on his posterior teeth, and maxillary lateral incisors. The patient had thin gingival biotype with gingival recession on th

Treatment of OSA beyond adenotonsillectomy.

Pediatric obstructive sleep apnea (OSA) is a common syndrome in children. While an adenotonsillectomy is effective for most, it may not be the treatment of choice for all, and possible alternative treatment options are briefly discussed herein. In obese children, in whom OSA is more prevalent, and has high rates of persistence after surgery, weight loss interventions may be recommended. Anti-inflammatory therapies, such as nasal steroids and montelukast, reduce the size of the adenoids and tonsils and are considered part of the stepwise treatment approach. Functional orthodontic therapies may

Comprehensive Management of Mandibular Canine Transmigration: A Multidisciplinary Approach.

Transmigration of mandibular canine teeth is a rare dental anomaly characterized by the tooth's aberrant migration through the alveolar bone. This deviation from the expected eruption pathway can lead to tooth impaction, root resorption, periodontal problems, and aesthetic concerns. The exact cause of transmigration is not fully understood, but it is believed to be influenced by a combination of genetic, developmental, and environmental factors. Early diagnosis and appropriate treatment are essential to prevent complications and achieve optimal functional and aesthetic outcomes. This case repo

Prosthetic treatment of cleft lip and palate patient - a case report.

It is assumed that most cleft lip and palate are due to a combination of genetic and environmental factors (maternal diseases, smoking, alcohol intake, infections, and ionizing treatment) that alter morphogenesis. Treatment of these cases begins from birth to adulthood and requires a number of interventions depending on the period of development of the maxillofacial area and teeth. We present the case of a patient with unilateral cleft lip and palate. Surgery and orthodontic treatment were carried out subsequently, and then a metal-ceramic bridge was made to correct the defect. In the course of

Continuous archwire technique for the correction of completely transposed maxillary incisors.

Tooth transposition is an uncommon disorder related to ectopic eruption; it can be classified as complete or incomplete on the basis of the position of the crowns and roots of transposed teeth. Aligning the transposed teeth to a normal sequence is always complex and challenging, especially in patients with complete transposition. The segmented archwire technique with cantilever or loops has been used in many transposition patients; however, it requires considerable laboratory work and is sometimes uncomfortable for the patient. In this case report, we present a novel orthodontic treatment for

The mechanism: how dental resorptions occur in ameloblastoma.

Knife-edge or blunt root resorptions characterize ameloblastomas and are pathognomonic for this tumor, because they differentiate ameloblastomas from simple bone cysts, odontogenic keratocysts and nasopalatine duct cysts, which do not lead to resorption of involved teeth. Despite the very high frequency and importance of these characteristics for a differential diagnosis, a microscopic examination should also be conducted before defining the diagnosis and the treatment plan for these cases. This paper describes a six-step hypothesis to explain the mechanism by which ameloblastomas promote the

Facial asymmetry correction: From conventional orthognathic treatment to surgery-first approach.

The surgery-first approach (SFA), which proceeds without presurgical orthodontic treatment, is assumed to shorten the treatment course because the direction of post-surgical orthodontic tooth movement conforms to the normal muscular forces. Moreover, the regional acceleratory phenomenon (RAP), evoked by surgery, helps in tooth alignment and compensation in a faster way. Although SFA has definite advantages, especially in class III individuals, there is a lack of data about its indications in patients with facial asymmetry. In this article, we reviewed recently published articles on the treatme

Tooth Movement Associated With CPAP Therapy.

We report on a rarely documented side effect-tooth movement-associated with positive airway pressure therapy. A 64-year-old woman presented to an orthodontic practice for evaluation of unexplained tooth movement and spaces opening between her anterior teeth. The patient recently received a diagnosis of mild obstructive sleep apnea, for which she had been using continuous positive airway pressure (CPAP) delivered by a nasal pillows interface for the past 18 months. Follow-up questioning revealed that shifting of the teeth was first noticed in the months following the initiation of CPAP and the

Utility of CBCT for the measurement of palatal bone thickness.

INTRODUCTION: The palate is an alternative anchoring site for orthodontic implants. Adequate bone at mini-implant placement site can influence the success or failure of anchorage. Hence, it is imperative to measure the thickness of the palatal bone. MATERIALS AND METHODS: CBCT scans of 30 subjects in the age range of 12 to 28 years were retrospectively analyzed with the objective of measuring the palatal bone thickness. Thirty sites were identified on each CBCT scan with incisive foramen as a landmark and measurements were obtained anteroposteriorly as well as mediolaterally, using Carestream

Angle Class III malocclusion with anteroposterior and vertical discrepancy in the final stage of growth.

Angle Class III malocclusion is characterized by an anteroposterior dental discrepancy with or without anteroposterior and vertical skeletal changes. Patients usually seek orthodontic treatment because facial appearance is compromised in most cases. The present study describes the clinical case of a 12-year and 6-month-old girl in her final stage of pubertal growth presenting Class III malocclusion with anteroposterior and vertical discrepancies. Initial treatment consisted of maxillary expansion using a Hass expander followed by the use of a Petit facemask for a minimum of 16 hours a day. Dur

Improving the efficacy of chlorhexidine-releasing elastomerics using a layer-by-layer coating technique.

The aims of this study were to identify the optimal concentration of coated orthodontic elastomerics using a layer-by-layer technique that can release chlorhexidine (CHX) as an antimicrobial material, and to measure the physical properties and antimicrobial effects of the coated elastomerics. Ethyl cellulose (EC) was used as the polymer, and five study groups with various combinations of solvents (i.e., ethanol and dichloromethane [DCM]) were included. The coated elastomerics were evaluated with a spectrophotometer to confirm the release of CHX, and their surfaces were observed by SEM. The CHX
