Classifying the structural heterogeneity of peritoneal adhesions by histopathological extracellular matrix characteristics.

BACKGROUND: Peritoneal adhesions are a common complication following abdominal surgery and contribute substantially to long-termmorbidity, including bowel obstruction, chronic pain, and infertility. Their diverse clinical presentation, ranging from thin strands todense obstructive bands, suggests multiple underlying pathomechanisms. We propose a histopathological framework for classifyingperitoneal adhesions based on extracellular matrix characteristics, providing a microscopic correlation to clinical heterogeneity andsupporting translational research on adhesion prevention. METHODS: In this

Peritoneal ovarian cancer growth in the omentum proceeds independently of mature adipocytes.

The omentum, a specialized adipose tissue within the peritoneum, is a primary niche of ovarian cancer dissemination. Omental adipocytes are widely thought to promote tumor growth by supplying lipids, supported in part by studies using global FABP4 deficiency. Here, we directly test whether mature adipocytes are required for peritoneal ovarian cancer growth using mice congenitally lacking mature adipocytes within the peritoneal cavity, including the omentum. Across several ovarian cancer models (ID8p53-/-Brca2-/-, BPPNM, and KPCA), tumors preferentially seed adipose-associated regions despite t

Inhibition of VEGFR1 TK Signaling in Peritoneal Macrophages Suppresses Endometriosis Development.

BACKGROUND/AIM: Endometriosis is characterized by the accumulation of immune cells in endometrial lesions and the peritoneal cavity. Macrophages contribute to the growth and neovascularization of endometriotic lesions. Vascular endothelial growth factor receptor-1 (VEGFR1) is involved in neovascularization, while peritoneal macrophages (PMs) play a critical role in endometriosis development and establishment. We examined the role of VEGFR1 signaling in PMs during endometriosis development using a murine model of ectopic endometrial transplantation. MATERIALS AND METHODS: Endometrial fragments

Pretransplant Serum Creatinine in Peritoneal Dialysis Patients Predicts Graft Outcomes.

RATIONALE & OBJECTIVE: Although peritoneal dialysis (PD) as a pretransplant dialysis modality is associated with favorable outcomes after kidney transplant, it is unknown if pretransplant serum creatinine (Scr) level is associated with subsequent graft outcomes in candidates managed with PD. Our objective was to examine the association between Scr at the time of transplant and short-term and long-term outcomes posttransplant. STUDY DESIGN: Retrospective cohort study. SETTING & PARTICIPANTS: A total of 20,166 adult (≥18 years of age) patients who were receiving PD at the time of a first livin

The experience of social isolation in patients receiving peritoneal dialysis: a qualitative study.

BACKGROUND: Social isolation(SI), as a negative psychological state, can aggravate sleep disturbances and deterioration of renal function, seriously jeopardizing physical health, which is particularly prominent in peritoneal dialysis(PD) patients. However, most of the current research in this field is quantitative, and there are few qualitative studies on SI in PD patients. Understanding patients’ psychological experience of SI is essential for the targeted design of intervention programs. Therefore, the aim of this study is to investigate the experience of SI among PD patients. METHODS: 12 pe

Barriers and Opportunities in Access to Peritoneal Dialysis across Veterans Health Administration.

Peritoneal dialysis (PD) use among the US Veteran population is lower than in the non-Veteran kidney failure population. Enhancing access to PD within the Veteran Health Administration (VHA) may be crucial for achieving the Advancing American Kidney Health Executive Order goals. The VHA Home Dialysis Committee conducted a nationwide survey of nephrology stakeholders to assess Veterans' access to PD across the VHA and identify barriers and opportunities for the growth of VHA-affiliated PD services. Participants were invited through email and completed an electronic questionnaire consisting of 1

Analysis of TERT mRNA Levels and Clinicopathological Features in Patients with Peritoneal Mesothelioma.

BACKGROUND/OBJECTIVES: Telomerase reverse transcriptase (TERT) is the catalytic subunit of the telomerase enzyme responsible for telomere length maintenance and is an important cancer hallmark. Our study aimed to clarify the mRNA expression of TERT in peritoneal mesothelioma (PeM), and to explore the relationship between its expression and the clinicopathological parameters and prognosis of patients with PeM. METHODS: In a cohort of 13 MpeM patients, we evaluated histotype, nuclear grade, mitotic count, necrosis, inflammation, Ki67, BAP1, MTAP and p16 expression by immunohistochemistry, p16/C

Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy for Gastric Cancer Peritoneal Metastases.

Gastric cancer with peritoneal metastases (GC-PM) carries a poor prognosis and estimated survival is less than 6 to 12 months. One potential treatment of GC-PM is cytoreduction surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). Success of this treatment largely relies on tumor biology and patient selection. These operations carry a high risk of morbidity and mortality and their efficacy in GC-PM remains controversial. This study will review the updated literature for CRS ± HIPEC in patients with GC-PM.

Educational Review: Updates on Therapeutic Strategies for Gastric Cancer with Peritoneal Metastasis.

Gastric cancer (GC) commonly presents in advanced stages with metastatic spread to the peritoneal cavity, and outcomes associated with gastric cancer with peritoneal metastasis (GCPM) continue to carry a dismal prognosis. Persistent challenges in the detection of peritoneal metastasis (PM) have resulted in a relative paucity of high-quality data to inform management decisions. Several consensus groups have published recommendations to guide management, including most recently the National Comprehensive Cancer Network guidelines, which now include cytoreductive surgery (CRS) with hyperthermic i

[Reflections on the current state of diagnosis and treatment for peritoneal metastasis in colorectal cancer].

Peritoneal metastasis of colorectal cancer is the second most common metastatic pattern after liver metastasis, clinically common and associated with a poor prognosis. Refractory subtypes such as mucinous adenocarcinoma, signet-ring cell carcinoma, and BRAF V600E-mutated colorectal cancers account for a relatively high proportion in peritoneal metastasis. While previous diagnosis and treatment faced significant challenges, recent advances in new technologies and evolving therapeutic concepts have achieved progress in management. Many patients with colorectal cancer peritoneal metastasis have o

[Selection and research advances of intraperitoneal drug treatment for colorectal peritoneal metastasis].

Peritoneal metastasis is a common form of distant metastasis in patients with colorectal cancer, and it is typically associated with a poor prognosis. The development of peritoneal metastasis involves complex molecular mechanisms and multifactorial regulation of the tumor microenvironment. Due to the presence of the blood-peritoneal barrier, only a small amount of systemic medication reaches the peritoneal cavity, resulting in limited efficacy against peritoneal metastasis. Intraperitoneal administration shows significant therapeutic advantages as it can directly target the tumor microenvironm

Different Diseases, Different Outcomes: A Comparative Study of Peritoneal and Pleural Subtypes.

INTRODUCTION: Peritoneal and pleural mesotheliomas have often been grouped together in studies, with limited exploration of their distinct characteristics. We conducted an analysis of demographics, clinical features, treatment types, and overall survival in these two mesothelioma subtypes. METHODS: We queried the 2021 iteration of the National Cancer Database for adult patients with mesothelioma, and categorized patients by the primary site (pleural versus peritoneal). Patients with a primary site other than pleural or peritoneal were excluded. Clinical and demographic characteristics were co

Peritoneal Metastases in Gastrointestinal Malignancies: A Review of the Pathophysiology of Metastasis and the Tumour Microenvironment.

Peritoneal metastasis is a significant source of morbidity and mortality for patients with cancer from gastrointestinal origin such as gastric, colorectal, and appendiceal cancers, and treatment options are limited. Cross-sectional imaging is limited in sensitivity and specificity, and treating peritoneal metastases is limited by the bioavailability of systemic cytotoxic chemotherapy and metastatic burden. Within this review, we specifically explore the pathophysiology underlying peritoneal metastasis while emphasising the contributions of the tumour microenvironment and immune system.

Trends and surgical outcomes in laparoscopic and open peritoneal dialysis catheter insertion.

BACKGROUND: Peritoneal dialysis (PD) catheters can be inserted via open or laparoscopic techniques. Recent guidelines suggest advanced laparoscopic insertion for suitable patients, due to known catheter-related benefits, but surgical outcomes literature is lacking. MATERIALS AND METHODS: We analyzed 11,731 PD catheter insertions from 2013 - 2018 in the National Surgical Quality Improvement Program database, including preoperative characteristics, operative details, and 30-day outcomes. Trends over time and factors associated with approach were assessed. Logistic regression examined the associ

Pericyte-myofibroblast transition: a novel mechanism in peritoneal fibrosis and the effect of Asiaticoside.

INTRODUCTION: Peritoneal fibrosis (PF), a major complication of long-term peritoneal dialysis (PD), is characterized by excessive extracellular matrix (ECM) deposition mediated by activated myofibroblasts (MyoFs). While mesenchymal transition of mesothelial cells has been widely studied, the cellular origins of MyoFs remain incompletely defined. Recent evidence suggests pericytes-mural cells surrounding microvessels-may contribute to fibrosis in other organs, but their role in PF pathogenesis is undefined. OBJECTIVE: This study aimed to clarify pericyte-myofibroblast transition (PMT) as a key
