Prognostic model for pancreatic cancer based on machine learning of routine slides and transcriptomic tumor analysis.

BACKGROUND: Prognostication for pancreatic ductal adenocarcinoma (PDAC) using histologic images is difficult due to tumor heterogeneity. We developed an artificial intelligence (AI) model to predict postoperative recurrence using histologic image patches. METHODS: We included 591 patients with resected PDAC to train an AI model for recurrence prediction at 12 or 24 months and validated it using external cohorts (n = 302 in total). Image patches from hematoxylin and eosin-stained slides were clustered via uniform manifold approximation and projection (UMAP) and used to train a random forest mo

Explainable Artificial Intelligence Unravels the Possible Distinct Roles of VKORC1 and CYP2C9 in Predicting Warfarin Anticoagulation Control.

Background: Warfarin pharmacogenomics is critical due to its narrow therapeutic index and significant interpatient variability. While machine learning (ML) can predict anticoagulation control status (ACS), its "black-box" nature limits clinical translatability. Explainable Artificial Intelligence (XAI) addresses this by providing interpretable insights. This study applied ML and XAI to a warfarin pharmacogenomic dataset to predict poor ACS and explain model decisions. Methods: A post hoc analysis was conducted on a cross-sectional dataset of 232 patients receiving warfarin for ≥6 months. Data

Machine Learning and Blood-Targeted Proteomics Enable Early Prediction and Etiological Discrimination of Hypertensive Pregnancy Disorders.

Imperfect first-trimester screening for hypertensive disorders of pregnancy (HDP) means many high-risk women miss the window for preventive aspirin, and the biological heterogeneity of HDPs is overlooked. This study aimed to leverage first-trimester serum proteomics to create a more precise tool for predicting preeclampsia (PE) and differentiating it from other HDPs. A prospective nested case-control study (n = 172) was conducted using targeted liquid chromatography-multiple reaction monitoring-mass spectrometry (LC-MRM-MS) proteomic profiling of 115 proteins. Machine learning (ML) methods wer

Towards clinically interpretable machine learning in emergency surgery: feature importance and insights across clinical time points in abdominal pain cases.

PURPOSE: Machine learning (ML) may support decision-making for acute abdominal pain (AAP), but limited interpretability hinders adoption. We evaluated a random-forest classifier for predicting urgent abdominal surgery and examined how predictive value and feature importance evolve across clinical timepoints. METHODS: In this retrospective single-center study, we included adults presenting with AAP to the emergency department. The outcome was urgent abdominal surgery within 24 h. Features were grouped into stepwise sets. For each feature set, models were trained in 20 random 80/20 train-test s

Development and validation of a machine learning clinicogenomic model to improve prognostic stratification in ER-positive/HER2-negative early breast cancer.

BACKGROUND: Several gene expression signatures (GESs) are used for risk stratification in estrogen receptor-positive/human epidermal growth factor receptor 2-negative (ER-positive/HER2-negative) early breast cancer. Recent integrative approaches combine tumour proliferation, estrogen receptor signalling, immune activity, and clinicopathologic features, yielding gains in prognostic accuracy. We therefore developed a machine-learning-based clinicogenomic prognostic model integrating a research-grade implementation of an established GES with immune-related and clinicopathologic features to improv

Systemic inflammatory and metabolic indexes for aortic dissection: a machine learning analysis.

BACKGROUND: Inflammation plays a crucial role in the development of aortic dissection. The systemic immune-inflammation index (SII) and systemic inflammation response index (SIRI) are widely used surrogate markers for systemic inflammation, while the neutrophil to high-density lipoprotein cholesterol ratio (NHR), platelet to high-density lipoprotein cholesterol ratio (PHR), lymphocyte to high-density lipoprotein cholesterol ratio (LHR), and monocyte to high-density lipoprotein cholesterol ratio (MHR) incorporate high-density lipoprotein, reflecting both inflammatory and metabolic status. Rapid assessment of the intrinsic hazard potential of different types of plastic additives using machine learning (RF-MLP).

As plastic pollution becomes increasingly severe, the additives released during its degradation have attracted widespread attention. How to effectively assess the potential hazards of plastic additives and identify candidates with relatively lower predicted intrinsic hazard potential for further evaluation has become an urgent issue. Experimental hazard assessment is costly and time-consuming, and the limited availability of baseline data prevents comprehensive screening of the large number of plastic additives in use. Currently, most additives lack sufficient baseline data, making it difficul

Real-time continuous assessment of fatigue from surface electromyography with deep learning for training load regulation in elite cyclists.

BACKGROUND: Accurate assessment of exercise-induced muscle fatigue is essential for optimizing training loads and preventing overtraining in elite athletes. This study presents a deep learning framework for continuous fatigue estimation from raw surface electromyography (sEMG) signals during high-intensity cycling. METHODS: Fourteen elite track cyclists performed a 30-second all-out sprint on a cycle ergometer. Power output was recorded at 1 Hz to derive a continuous fatigue index (percentage decline from peak power). Simultaneously, sEMG signals were recorded at 1000 Hz from four lower-limb

Machine learning vs. traditional regression analysis for fluid overload prediction in the ICU.

Fluid overload, while common in the ICU and associated with serious sequelae, is hard to predict and may be influenced by ICU medication use. Machine learning (ML) approaches may offer advantages over traditional regression techniques to predict it. We compared the ability of traditional regression techniques and different ML-based modeling approaches to identify clinically meaningful fluid overload predictors. This was a retrospective, observational cohort study of adult patients admitted to an ICU ≥ 72 h between 10/1/2015 and 10/31/2020 with available fluid balance data. Models to predict fl

Design of (quinolin-4-ylthio)carboxylic acids as new Escherichia coli DNA gyrase B inhibitors: machine learning studies, molecular docking, synthesis and biological testing.

Spread of multidrug-resistant Escherichia coli clinical isolates is a main problem in the treatment of infectious diseases. Therefore, the modern scientific approaches in decision this problem require not only a prevention strategy, but also the development of new effective inhibitory compounds with selective molecular mechanism of action and low toxicity. The goal of this work is to identify more potent molecules active against E. coli strains by using machine learning, docking studies, synthesis and biological evaluation. A set of predictive QSAR models was built with two publicly available

Encoding and Decoding Models in Cognitive Electrophysiology.

Cognitive neuroscience has seen rapid growth in the size and complexity of data recorded from the human brain as well as in the computational tools available to analyze this data. This data explosion has resulted in an increased use of multivariate, model-based methods for asking neuroscience questions, allowing scientists to investigate multiple hypotheses with a single dataset, to use complex, time-varying stimuli, and to study the human brain under more naturalistic conditions. These tools come in the form of "Encoding" models, in which stimulus features are used to model brain activity, an

Evaluating the validity and performance of a modifiable prediction model to predict new long-term opioid use in prescription drug monitoring programs.

OBJECTIVE: Prescription drug monitoring programs (PDMPs) offer a promising data source for the development of clinical decision support (CDS) systems to guide opioid prescribing, but most PDMP agencies lack capacity for in-house model development-instead relying on third-party tools. This study aims to externally validate a PDMP-based model, developed using 2018 California PDMP records, to predict new long-term use among Kentucky opioid-naïve patients and compare its performance to Kentucky-specific machine learning (ML) models. MATERIALS AND METHODS: We identified opioid-naïve Kentucky resid

Reduced-rank random regression for improved yield prediction across the Australian wheatbelt.

Predicting complex traits like yield in new environments is challenging due to genotype-by-environment interactions (GEI). Environmental covariates (ECs) can improve prediction by describing environmental variation relevant to GEI. However, approaches that model genotype-specific responses to one or multiple ECs can become highly parameterized and difficult to fit because of the need to estimate complex covariance structures. In addition, EC-based prediction methods typically assume that future environmental conditions are known. Building on recent work, we extended a reduced-rank factor-analy
