Minimal Residual Disease of T Acute Lymphoblastic Leukemia: the Experience of University Hospital Center Casablanca - Morocco.

BACKGROUND: MRD measurement is based on the ability to detect a minimal quantity of tumor cells within a population of healthy cells. Given the low sensitivity of cytology, flow cytometry in T-ALL allows the distinction between normal cells and leukemic cells and classifies patients according to their prognosis. The aim of this study was to define the immunophenotypic features of T-ALL at different stages of differentiation in terms of minimal residual disease. METHODS: This is a prospective study carried out in the Hematology Laboratory of the IBN ROCHD University Hospital in Casablanca Moro

Increasing Role of Targeted Immunotherapies in the Treatment of AML.

Acute myeloid leukemia (AML) is the most common acute leukemia in adults. The standard of care in medically and physically fit patients is intensive induction therapy. The majority of these intensively treated patients achieve a complete remission. However, a high number of these patients will experience relapse. In patients older than 60 years, the results are even worse. Therefore, new therapeutic approaches are desperately needed. One promising approach in high-risk leukemia to prevent relapse is the induction of the immune system simultaneously or after reduction of the initial tumor burde

Combined Bone Marrow and Kidney Transplantation for the Induction of Specific Tolerance.

The induction of specific tolerance, in order to avoid the detrimental effects of lifelong systemic immunosuppressive therapy after organ transplantation, has been considered the "Holy Grail" of transplantation. Experimentally, tolerance has been achieved through clonal deletion, through costimulatory blockade, through the induction or infusion of regulatory T-cells, and through the establishment of hematopoietic chimerism following donor bone marrow transplantation. The focus of this review is how tolerance has been achieved following combined bone marrow and kidney transplantation. Preclinic

The prognostic value of cytogenetic analyses in patients with acute nonlymphocytic leukemia treated with the same intensive chemotherapy.

BACKGROUND: Some specific chromosome abnormalities for the leukemias have been proven to be associated with the prognosis of acute nonlymphocytic leukemia (ANLL). However, most of these reports included patients treated with different protocols. Therefore, some bias has been involved in the evaluation of the prognostic factors in such reports. METHODS: The authors studied the morphologic, cytogenetic, and clinical features of 136 patients (86 males and 50 females) with de novo ANLL treated with the same protocol of intensive induction chemotherapy using multivariate analyses. RESULTS: Chromo

Clinical and laboratory changes induced by alpha interferon in chronic lymphocytic leukemia--a pilot study.

Ten chronic lymphocytic leukemia patients were treated with partially pure alpha interferon (IFN-alpha) at doses ranging from 3-9 X 10(6) units administered intramuscularly daily. Of these patients, three patients with disease stages 0, 1, and 3 (and prolymphocytic leukemia), respectively, responded with partial remissions lasting from 10 to 24+ months; four additional patients had minor responses. Among those responding, modulation of B cell chronic lymphocytic leukemia, surface immunoglobulins, and surface antigens were observed in four patients. These changes included a decline in the propo

Higher Cumulative Cytarabine Consolidation Improves Survival in Older Adults with Acute Myeloid Leukemia.

Background: Post-remission cytarabine consolidation is a cornerstone of therapy for acute myeloid leukemia (AML), but the optimal dosing strategy in older adults (≥60 years) remains unclear. High-dose cytarabine (HiDAC) is often avoided due to toxicity concerns, and data guiding cumulative dosing are limited. Methods: We conducted a single-center retrospective cohort study of 111 patients aged ≥60 years with AML who achieved complete remission after standard 7 + 3 induction and received at least one cycle of cytarabine consolidation between 2012 and 2024. A 90-day landmark analysis excluded ea

Serum and Histologic Eosinophilia as a Predictive Biomarker of Response to Mirikizumab in Ulcerative Colitis.

INTRODUCTION: Eosinophils may contribute to ulcerative colitis (UC) pathogenesis through inflammation and epithelial injury. Mirikizumab, a selective IL-23 inhibitor, has shown efficacy in moderate-to-severe UC. However, the relationship between eosinophils and treatment outcomes with IL-23 inhibition remains unclear. OBJECTIVE: This post-hoc analysis of the LUCENT-1 trial evaluated trends in serum and histologic eosinophils as biomarkers for early response to mirikizumab induction therapy in UC. DESIGN: Data from LUCENT-1 (NCT03518086) were analyzed to assess serum eosinophil counts and his

Case Report: Blastic plasmacytoid dendritic cell neoplasm mimicking systemic lymphoma: a multidisciplinary diagnostic challenge.

BACKGROUND: Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is an aggressive hematologic malignancy originating from plasmacytoid dendritic cell precursors. While classic cases present with prominent cutaneous nodules and lymphadenopathy, predominant generalized lymphadenopathy can obscure the diagnosis, frequently resulting in a misdiagnosis of systemic lymphoma. CASE PRESENTATION: A 56-year-old male presented with a one-month history of generalized pruritic erythema and multi-regional lymphadenopathy. Initial PET-CT demonstrated systemic lymphadenopathy and splenomegaly with increased

Selective efficacy of venetoclax in unfit patients with acute myeloid leukemia with myelodysplasia-related gene mutations under low-intensity therapy.

According to the 2022 International Consensus Classification (ICC) guidelines, nine myelodysplasia-related (MDS-related) gene mutations are classified as adverse-risk markers in acute myeloid leukemia (AML) under intensive therapy (INT). Although venetoclax (VEN) has demonstrated clinical benefit in subsets of AML with MDS-related gene mutations (AML-MR), its efficacy across all nine mutations remains unclear. In this retrospective study involving 453 AML-MR patients, the overall composite complete remission (CRc) rate was 62.6% (275/439). Among fit patients receiving INT, 57.7% (86/149) achie

Modified risk-stratified sequential treatment (subcutaneous rituximab with or without chemotherapy) in B-cell Post-transplant lymphoproliferative disorder (PTLD) after Solid organ transplantation (SOT): the prospective multicentre phase II PTLD-2 trial.

The prospective multicentre Phase II PTLD-2 trial (NCT02042391) tested modified risk-stratification in adult SOT recipients with CD20-positive PTLD based on principles established in the PTLD-1 trials: sequential treatment and risk-stratification. After rituximab monotherapy induction, patients in complete remission as well as those in partial remission with IPI < 3 at diagnosis (low-risk) continued with rituximab monotherapy and thus chemotherapy free. Most others (high-risk) received R-CHOP-21. Thoracic SOT recipients who progressed (very-high-risk) received alternating R-CHOP-21 and modifie

Nephrotic syndrome relapse in a boy with COVID-19.

Clinical data on coronavirus disease-19 (COVID-19) in children during the management of nephrotic syndrome (NS) is lacking. Patients on prednisolone are compromised hosts at the risk of severe infections. Some infections may induce NS relapse. We describe the clinical course of a child with NS and COVID-19. A 3-year-old boy was admitted with clinical and laboratory findings indicative of NS. Induction therapy with prednisolone (2 mg/kg/day) induced complete remission. While tapering the dose, he was infected with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). He developed a high
