Elevated liver glycogenolysis mediates higher blood glucose during acute exercise in Barth syndrome.

UNLABELLED: Barth syndrome (BTHS) is an X-linked recessive genetic disorder due to mutations in the Tafazzin (TAFAZZIN) gene that lead to cardiac and skeletal muscle mitochondrial dysfunction. Previous studies in humans with BTHS demonstrate that the defects in muscle mitochondrial oxidative metabolism result in an enhanced reliance on anaerobic metabolism during exercise to meet energy demands of muscular work. During exercise, the liver normally increases glucose production via glycogenolysis and gluconeogenesis to match the elevated rate of muscle glucose uptake and meet the ATP requirement

Iron Deficiency Is Associated With Impaired Biventricular Reserve and Reduced Exercise Capacity in Patients With Unexplained Dyspnea.

BACKGROUND: Iron deficiency (ID) is frequent and associated with diminished exercise capacity in heart failure (HF), but its contribution to unexplained dyspnea without a HF diagnosis at rest remains unclear. METHODS AND RESULTS: Consecutive patients with unexplained dyspnea and normal echocardiography and pulmonary function tests at rest underwent prospective standardized cardiopulmonary exercise testing with echocardiography in a tertiary care dyspnea clinic. ID was defined as ferritin of <300 µg/L and a transferrin saturation of <20% and its impact on peak oxygen uptake (peakVO2), biventri

Isometric agonist and antagonist muscle activation interacts differently with 140-Hz transcranial alternating current stimulation aftereffects at different intensities.

During transcranial electric stimulation, increasing intracellular Ca2+ levels beyond those needed for inducing long term potentiation (LTP) may collapse aftereffects. State-dependent plastic aftereffects are reduced when applied during muscle activation as compared with rest. Cortical surround inhibition by antagonistic muscle activation inhibits the center-innervated agonist. The objective of this study is to determine the interaction of state dependency of transcranial alternating current stimulation (tACS) aftereffects at rest and under activation of agonist and antagonist muscles during s

Recalled and momentary virtual portions created of snacks predict actual intake under laboratory stress condition.

Virtual portion tasks have been used to predict food intake in healthy individuals, severity of illness in individuals with anorexia nervosa, and weight loss in bariatric surgery patients. Whether portion creation in response to a recalled interpersonal stress ("recalled stress portions") could be used as a proxy for ad lib intake, after a stressor, remains untested, and the mechanism supporting this relationship is unclear. The present study's goals were: 1) to validate virtual portion tasks as proxies for actual food intake in a stressful context and 2) to test a causal pathway in which thes

Idiopathic Ulcerative Laryngitis: A National Survey of Academic Laryngologists.

OBJECTIVES: Idiopathic ulcerative laryngitis (IUL) is a rare disorder characterized by ulceration in the mid-membranous portions of the vocal folds. The etiology and optimal management of this condition are poorly understood. We aimed to survey laryngologists on their experiences with managing IUL in order to better understand the diagnosis and treatment of this unusual condition. METHODS: An online survey was sent to 149 laryngologists practicing at academic institutions in the United States. Survey questions assessed respondents' professional backgrounds and clinical experiences with IUL. I

Lipoma of Parotid Gland: Report of Two Cases.

AIM: Congestive heart failure (CHF) and obstructive sleep apnoea syndrome (OSAS) are both associated with an intense sympathoexcitation, including an increased muscle sympathetic nerve activity (MSNA). We have studied the firing characteristics of single vasoconstrictor fibres to the muscle vascular bed in CHF and OSAS patients, at rest and during transient sympathoexcitatory stimuli, to elucidate the mechanisms by which vasoconstrictor output is augmented in these conditions. RESULTS: The main alternatives for augmenting sympathetic output are an increased firing frequency of individual nerv

Venous thromboembolism risk and postpartum lying-in: Acculturation of Indian and Chinese women.

OBJECTIVE: many cultures have a set time of traditional rest in the postpartum period. There is limited information on how this activity may potentially increase the risk of venous thromboembolism (VTE). We aimed to investigate VTE risk by determining the prevalence of the cultural practice of postpartum "lying-in", quantifying activity and determining the factors that influence this tradition in women from China and the Indian subcontinent (India, Bangladesh, Pakistan and Sri Lanka) at an Australian tertiary referral hospital. DESIGN: we surveyed a prospective cohort of 150 women aged ≥ 18 y

Brain blood flow patterns after the development of congestive heart failure: effects of treadmill exercise.

OBJECTIVE: Congestive heart failure (CHF) is associated with left ventricular (LV) failure, neurohormonal system activation, and diminished exercise capacity. Although alterations in systemic vascular resistive properties have been recognized to occur with CHF, whether and to what degree perfusion abnormalities occur within the brain after the development of CHF remain poorly understood. Accordingly, the present study measured brain blood flow patterns in pigs after the development of pacing-induced CHF at rest and after treadmill-induced exercise. MEASUREMENTS AND MAIN RESULTS: Adult pigs (n

Effects of surgical stimulation on the apnoeic thresholds for carbon dioxide during anaesthesia with sevoflurane.

Mechanical hyperventilation may produce hypocapnic apnoea below the carbon dioxide off-switch threshold whereas an increase in arterial PCO2 after post-hyperventilation apnoea causes reappearance of respiratory effort above the carbon dioxide on-switch threshold. To study the effects of surgical stimulation on these two thresholds, we have measured end-tidal PCO2 (PE'CO2) at the two thresholds, before and during surgical stimulation, in 14 patients undergoing mastectomy, anaesthetized with sevoflurane (1.2 MAC). Based on the reproducibility of the results, data from 11 patients were analysed a

Medial Elbow Injuries in the Throwing Athlete.

Medial elbow injuries in the throwing athlete are common and increasing in frequency. They occur due to repetitive supraphysiologic forces acting on the elbow during the overhead throw. Overuse and inadequate rest are salient risk factors for injury. Most athletes improve substantially with rest and nonoperative treatment, although some athletes may require surgical intervention to return to play. Because of advances in conservative and surgical treatments, outcomes after medial elbow injury have improved over time. Currently, most athletes are able to return to a high level of play after ulna

Habitual non-traumatic cricothyroid joint dislocation: a rare case of rotated larynx.

A case of a 21 year old gentleman is described, with no history of preceding trauma, presenting with intermittent dysphagia to solids and fluids for 4 years. Neck examination at rest was normal. However on deep inspiration, the right thyroid lamina protrudes or becomes more prominent. The patient is able to return the larynx to its normal position with manual manipulation. Laryngeal examination with fibreoptic scope during rest and deep breath shows gross rotation of the laryngeal structures for more than 60 degrees on deep breath, with the vocal cords axis rotated to the left side. Repeat cerclage prolongs pregnancy in women with prolapsed membranes.

We evaluated repeat cerclage in women with prolapsed membranes. Twenty-two women with bulging membranes after primary cerclage were offered repeat cerclage; 11 chose a repeat cerclage and 11 chose bed rest. The median gestational age at delivery, birthweight and survival rates were significantly higher in the repeat cerclage group compared to the bed rest group (mean 26.8 weeks vs. 21.7 weeks, p= 0.04, mean birthweight 1180 g vs. 491 g, p= 0.01 odds ratio for survival 22.0, 95% CI, 2.1-236). Quantitative measurement of oxygen consumption and forearm blood flow in patients with mitochondrial myopathies.

Five patients with chronic progressive external ophthalmoplegia (CPEO) and 27 healthy controls were examined by near-infrared spectroscopy (NIRS) for the non-invasive and direct quantitative measurement of muscle oxygen consumption during rest as well as during static isometric handgrip exercise at 10% of their maximum voluntary contraction. In patients with CPEO, we found a significantly decreased oxygen consumption during exercise, but more remarkably already during rest. Our results suggest that NIRS is able to discriminate between CPEO patients and healthy controls, which makes NIRS a prom

Exercise electrocardiography in the evaluation of cardiac dysrhythmias in children.

Exercise electrocardiography is a useful means to evaluate dysrhythmias in children and young adults. Exercise may suppress a dysrhythmia present at rest and thus suggest a relatively benign problem. It may provoke a latent dysrhythmia which is absent at rest but suggested by a history of exercise intolerance or palpitation with exertion. Exercise may also modify a dysrhythmia. Maximum exercise testing has been used to evaluate each of these situations in children. The results of testing will be presented. Since no dysrhythmias were provoked in 170 healthy children, we think that any dysrhythm

Treatment of traumatic hyphaema.

A series of 131 successive hospitalized cases of traumatic hyphaema after blunt injury of the eye were treated with complete bed-rest for 6 days and double eye-patching but without any topical medication. Seven patients (5.3%) had a secondary bleeding. The visual acuities of 115 patients were tested, and 105 regained their normal vision, i.e. 1.0 or over. These results are no better than those reported in series treated with moderate bed-rest for 3-4 days combined with topical administration of corticosteroids and mydriatics. The results indicate that our present treatment may be unnecessarily

Parotid salivary secretory pattern in bulimia nervosa.

Parotid gland enlargement occurs in about 25% of patients with the binge eating syndrome of bulimia nervosa. The parotid salivary secretory patterns in 28 bulimics were determined in order to investigate the functional abnormality in the glands. Bulimia patients had a reduced resting flow rate. Bulimics who developed sialadenosis (4 patients) had reduced resting and stimulated flow rates. The salivary amylase activity was increased in both the resting and stimulated states in bulimics and the sialadenosis group. The resting total protein levels were greater in the bulimics.
