Understanding the acylation mechanisms of active-site serine penicillin-recognizing proteins: a molecular dynamics simulation study.

Beta-lactam antibiotics inhibit enzymes involved in the last step of peptidoglycan synthesis. These enzymes, also identified as penicillin-binding proteins (PBPs), form a long-lived acyl-enzyme complex with beta-lactams. Antibiotic resistance is mainly due to the production of beta-lactamases, which are enzymes that hydrolyze the antibiotics and so prevent them reaching and inactivating their targets, and to mutations of the PBPs that decrease their affinity for the antibiotics. In this study, we present a theoretical study of several penicillin-recognizing proteins complexed with various beta

Prevention of Pneumococcal Meningitis.

With the success of the conjugated Haemophilus influenzae type b vaccines, Streptococcus pneumoniae has become one of the most important causes of bacterial meningitis worldwide, causing significant morbidity and mortality. Additionally, the increasing amount of resistance that this organism is developing to multiple classes of antimicrobial agents has made the treatment of pneumococcal infections, especially meningitis, much more difficult. Immunization has been shown to be one of most effective methods for preventing pneumococcal meningitis, resulting not only in a decrease in disease burden

Induced hypothermia in experimental pneumococcal meningitis.

Pneumococcal meningitis resulting from Streptococcus pneumoniae has a death rate of 28% in adults. In severe head injury and stroke, inflammatory changes and intracranial hypertension are improved by induced hypothermia, which also is neuroprotective. We hypothesized that moderate hypothermia ameliorates inflammatory changes in experimental pneumococcal meningitis. Wistar rats were cooled systemically, and meningitis was induced by pneumococcal cell wall components. The increase of regional cerebral blood flow in the meningitis animals was blocked by hypothermia at 6 hours. The reduction of in

Monoclonal antibodies against conserved epitopes of a 46-kDa protein from Neisseria meningitidis.

The purpose of the present study was to generate monoclonal antibodies (mAbs) against conserved epitopes of B meningococcus which could be applicable to the immunoscreening of bacterial meningitis. Three mAbs reactive to a 46-kDa protein conserved in eight sero-groups and several sero(sub)types of Neisseria meningitidis were selected for the present study. No reaction was detected with whole-cell lysates of Staphylococcus aureus. Streptococcus pneumoniae, Haemophilus influenzae type b or Escherichia coli. Two of these mAbs recognized 46-kDa epitopes in four other Neisseria spp, and the third,

Morphologic and biochemical study of vascular permeability of the middle ear mucosa in experimental otitis media.

The vascular permeability of the middle ear mucosa of chinchillas was examined by light and electron microscopy using horseradish peroxidase (HRP). Two kinds of experimental otitis media were used: serous otitis media (SOM) produced by eustachian tube obstruction and purulent otitis media (POM) produced by inoculation with Streptococcus pneumoniae type 7F. Normal mucosa showed no leakage of HRP. In SOM, vessels were dilated and tortuous, but they did not show HRP leakage. In POM, vessel leakage of HRP was extensive. Biochemically, we measured the middle ear effusion-to-serum ratio of both albu

Penicillin-intermediate pneumococci in a children's hospital.

During the three-year period from 1981 to 1984, all clinical isolates of Streptococcus pneumoniae were screened for resistance to penicillin in the clinical bacteriology laboratory at The Children's Hospital of Alabama, Birmingham. Twenty-eight of 828 isolates were presumed resistant by disk diffusion testing with 1-microgram oxacillin disks (zone diameter, less than 20 mm). Seventeen of the 28 (61%) were found to be intermediately sensitive to penicillin by a conventional agar dilution method. Penicillin-intermediate strains had a minimal inhibitory concentration of 0.125 to 0.5 mg/L; no peni

Ulcerative keratitis. Survey of 30 years' laboratory experience.

During a 30-year period, causative organisms were identified on Gram's stain and culture in 547 of 677 cases of infectious corneal ulcers. Even if no organisms were seen on Gram's stain, culture results were often positive. Staphylococcus was the most common isolate; Moraxella, Pseudomonas, and Streptococcus pneumoniae were the next most frequent pathogens. Only 1% of cases were fungal. Of some help in identifying the causative organism were locations, presence or absence of hypopyon, and perforation of the ulcer. Most marginal lesions were a result of staphylococcal infection; central lesions

Cardiac Tamponade Due to Purulent Pericarditis in an Immunocompetent Patient.

We report the case of a previously healthy 24-year-old man admitted with a right-sided empyema and community-acquired pneumonia who subsequently developed acute chest pain, worsening dyspnea, and hemodynamic instability. Point-of-care cardiac ultrasound (POCUS) revealed a large circumferential pericardial effusion with a "swinging heart" and diastolic collapse of the right chambers, consistent with cardiac tamponade. Urgent ultrasound-guided pericardiocentesis drained thick purulent fluid, and Streptococcus pneumoniae was identified by polymerase chain reaction (PCR). Targeted intravenous ceft

Reverse Vaccinology for Influenza A Virus: From Genome Sequencing to Vaccine Design.

Reverse vaccinology (RV) consists in the identification of potentially protective antigens expressed by any organism starting from genomic information and derived from in silico analysis, with the aim of promoting the discovery of new candidate vaccines against different types of pathogens. This approach makes use of bioinformatics techniques to screen the whole genomic sequence of a specific pathogen for the identification of the epitopes that could elicit the best immune response. The use of in silico techniques allows to reduce dramatically both the time and cost required for the identifica

The Difficulty Detecting Tuberculosis in a Child with Post-COVID-19 and Cerebral Palsy.

When hypostatic pneumonia is present at the same time as COVID-19 pneumonia, the clinical course is almost always prolonged (prolonged-COVID-19) due to persistent inflammation, long-term anti-inflammatory syndrome, followed by immune exhaustion, i.e., by immunosuppression and catabolic syndrome. In the immunosuppression phase, viral reactivation can be accompanied by a secondary infection, which, in this case, is pulmonary tuberculosis. Pulmonary tuberculosis in post-COVID-19 patients and in patients with spastic quadriplegic cerebral palsy does not have a typical clinical course nor laborator

MAIT cells, their biological and medical significance.

MAIT cells are a separate cell population differentiating in the thymus. They are mostly present in the peripheral blood, liver, intestine, and lungs, less often in other tissues, and infrequently in the lymph nodes. The presentation molecules for MAIT cells are MR1 proteins. They are evolutionarily conserved and non-polymorphic, resemble class I HLA molecules, and are expressed by all cell types. They present bacterial and yeast vitamin metabolites which arise during the synthesis of vitamin B2. The effector functions of MAIT cells are promoted through cytokine synthesis. They also act cytoto

Pneumococcal perihepatitis due to hematogeneous dissemination.

Perihepatitis is mainly caused by a direct extension of pelvic inflammatory disease, in which the causative pathogen is typically Neisseria gonorrhoeae or Chlamydia trachomatis. We herein discuss the case of a 61-year-old female patient who presented with a fever and right upper quadrant pain. Perihepatitis was diagnosed by contrast-enhanced computed tomography. She had no previous history of sexual activity, genital symptoms, remarkable physical findings or examination results indicative of pelvic inflammatory disease or other diseases.
