We studied the effectiveness of uvulopalatopharyngoplasty under general anesthesia and classical surgery in 26 patients treated surgically for chronic snoring (CS) and obstructive sleep apnea syndrome (OSAS). Appropriate management for the diabetic patient with a fracture or sprain depends on recognition of "at-risk" factors. For patients with stable, minimally displaced injuries, conservative modalities (prolonged immobilization and non-weight-bearing) are sufficient. The lateral temporal fullness of the upper eyelid in patients presenting for a blepharoplasty is sometimes due to a prolapse of the lacrimal gland. The operative procedures that attempt to reposition the migrated lacrimal gland have been either dangerous in that they cause a dry-eye syndrome or unsatisfactory in that they have a high rate of recurrence or other complications. A new and different operative approach to a herniated or prolapsed lacrimal gland deals with an extraglandular concept of repositioning the gland that corresponds to the general principles of hernial surgery.

The authors evaluate the efficiency of the surgical technique of split anterior tibialis transfer, describe the long term functional results of the operation, and discuss the indications of this method of treatment. By combining echography and per-operative hormone assay, surgery for primary hyperparathyroidism due to a single adenoma can be performed via a limited unilateral access route with local anaesthesia. In a recent series of 204 patients, this technique was not contraindicated in 88 (43%). The operation was successful in 81 of these (92%). In 8 cases, the operation had to be converted into bilateral cervicotomy under general anaesthesia. Six of them were successful. The overall cure rate was 98%. Peroperative assay of 1-84 parathyroid hormone is an essential element in this technique.

An experimental model of hypertrophic pyloric stenosis was made by suture of the pyloric wall and gastrostomy in 10 rabbits under general anesthesia. Blood sampling indicated severe alkalosis and hypochloremia 3h 30 min after surgery. To correct the derangement, we tested an ion exchange resin (Dowex SAR), coated with a methacrylic hydrogel. A cartridge containing 18 g of this resin was inserted in an extracorporeal circuit. This chloride charged resin achieved uptake of HCO3- ions, and elution of Cl- ions. The electrolytic balance was fully restored after 10 min of treatment.

During the period from 1987-1990 152 patients underwent a Whipple procedure at the Department of General Surgery of Münster University Hospital. 27 operations were performed for chronic pancreatitis (17.8%), 52 for pancreatic (34.2%) and 73 (48.0%) for ampullary carcinoma. A retrospective study of the principal causes of mechanical bowel obstruction occurring in Western Turkey between 1979 and 1989 was undertaken. The records of 14,777 operations performed in the general surgery departments of two hospitals were reviewed. Mechanical bowel obstruction occurred in 582 patients. We report the long-term results of the Matti-Russe operation for pseudarthrosis of the scaphoid in 100 cases, reported previously by Mulder in 1968. Clinical results for 77 patients and radiographic data for 74 were reviewed at 22 to 34.8 years after surgery. In general, there was satisfactory relief of pain and stiffness but some patients had limitation of motion and reduced grip-strength, with usually slight osteoarthritic changes. There was poor correlation between subjective, objective, and radiographic results but 88% of the patients were satisfied with their results.

Two cases of arteriovenous malformations of the cecum treated by transcatheter embolization are presented. In both cases, treatment resulted in improvement of the clinical and biological signs. Pulmonary valvular stenosis had been diagnosed since infancy in a 3 year-old girl. Cyanotic episodes occurring when the child cried led to the decision to remove the stenosis, whose gradient was 58 mmHg. Transluminal dilatation through the femoral vein, after premedication and local anesthesia induced a decrease of the gradient to 18 mmHg with a good angiographic result. This technique, which may be used in numerous types of pulmonary stenosis, allowed avoidance of the disadvantages of surgery under general anesthesia with median sternotomy and cardiopulmonary by-pass.

The medicamentous therapy of tuberculous coxitis follows the general rules of antituberculous chemotherapy. This treatment should be completed by an operative therapy in every case. The method of radical surgery with sine-plastic, described by Göb in 1980, provides best conditions to highest radicality of this treatment. Between 1980 and 1984 13 patients were operated upon by this method in the Orthopedic Department of the Bad Berka Central Hospital. The results are mainly good. By means of a modification installed by the author a more subtle approach is achieved.

We have thus presented a series of decision-making processes that are easy to follow, clearly defined, and cost effective. We have found that with the use of this decision-making algorithm our patients receive the therapeutic measures most helpful at the optimal time and are neither denied helpful surgery nor submitted to operations that are useless surgical exercises. With the use of this protocol, we believe that our efficiency as surgeons, physicians, and advisors will increase and the specter of fear found in the general population regarding low back pain will diminish.

Several earlier studies have shown improvement of glaucomatous visual fields after pressure reduction achieved by acetazolamide or filtering surgery, while other studies have given conflicting results. In the present study laser trabeculoplasty was used to reduce the IOP in 42 eyes with glaucomatous visual field defects. Although good pressure reduction was achieved, no general regression of field defects was observed. The changes of the visual field were not correlated to the degree of pressure reduction. Gallbladder volvulus is an extremely rare disorder. Three cases have been treated recently in the Surgical Department of the Faculty of Medicine, Liège. The clinical picture is that of an acute abdomen requiring urgent surgery, diagnosis usually being made during the operation. Treatment is fairly simple : cholecystectomy, which no problem of dissection ro of instrumental exploration of the bile duct, the volvulus being normally free from stones. Prognosis for the volvulus and the gallbladder is a function of the general condition and the rapidity of operation, and is currently generally good.
