How to reduce recurrent laryngeal nerve palsy in anterior cervical spine surgery: a prospective observational study.

BACKGROUND: Recurrent laryngeal nerve palsy (RLNP) occurs as a complication during anterior cervical spine surgery. In 2005 the authors demonstrated the high incidence of asymptomatic RLNP in a right-sided approach. OBJECTIVE: This follow-up prospective observational study was designed to test 2 options said to reduce the rate of RLNP: reduced endotracheal cuff pressure and sinistral approach. METHODS: Two hundred forty-two patients in whom anterior cervical spine surgery was performed were examined postoperatively with indirect laryngoscopy to evaluate the status of the vocal cords. All pat

Does extralaryngeal branching have an impact on the rate of postoperative transient or permanent recurrent laryngeal nerve palsy?

BACKGROUND: This prospective study assessed the prevalence of the extralaryngeal branching of the recurrent laryngeal nerve (RLN) and its impact on the incidence of postoperative transient or permanent RLN palsy. METHODS: Total or hemithyroidectomy was performed in 115 patients, with a total of 195 RLNs displayed. The RLN extralaryngeal branches were routinely identified and preserved. The postoperative course of each patient was evaluated. Outcomes of patients with and without branching RLN were compared. RESULTS: In all, 36 of 195 (18.5%) nerves showed extralaryngeal branching: 27 cases (2

Evaluation of the cranial cruciate ligament repair system(®) in surgery for laryngeal hemiplegia in heavy draft horses.

The purpose of this study was to evaluate the effect of the canine cranial cruciate ligament repair system on laryngeal hemiplegia in heavy draft horses. Twenty-four heavy draft horses diagnosed with grade 4 laryngeal hemiplegia were allocated to either the prosthetic laryngoplasty (PL) group (n=14) or a canine cranial cruciate ligament repair system (CCCLRS) group (n=10). Right to left angle quotients (RLQs) of abductions of the arytenoid cartilages were endoscopically evaluated before and after surgery. Post-operative RLQs in the CCCLRS group were significantly lower than those of the PL gro

Predicting aspiration after hemispheric stroke from timing measures of oropharyngeal bolus flow and laryngeal closure.

Deglutitive aspiration is common after stroke, affecting up to 50% of patients and predisposing them to pneumonia, yet it is virtually impossible to predict those patients at greatest risk. The aim of this study was to develop a robust predictive model for aspiration after stroke. Swallowing was assessed by digital videofluoroscopy (VF) in 90 patients following hemispheric stroke. Lesion characteristics were determined by computerized tomography (CT) brain scan using the Alberta Stroke Programme Early CT Score (ASPECTS). Aspiration severity was measured using a validated penetration-aspiration

Salvage surgery after induction chemotherapy with paclitaxel/cisplatin and primary radiotherapy for advanced laryngeal and hypopharyngeal carcinomas.

Induction chemotherapy followed by primary radiotherapy in responders is considered an alternative to surgery for advanced cancer of the larynx and hypopharynx (LHC). Comparison of therapeutic approaches is challenging and must respect oncological and functional outcome as well as quality of life during and after treatment. One aspect of primary radiochemotherapy is the option of salvage surgery in case of residual tumor. The outcome after salvage surgery following new organ-preserving strategies has to be examined. All patients undergoing induction chemotherapy with paclitaxel and cisplatin f

Improved detection of recurrent laryngeal tumor after radiotherapy using (18)FDG-PET as initial method.

BACKGROUND AND PURPOSE: Timely detection of recurrent laryngeal tumor after radiation is an important predictive factor for curation as well as preservation of laryngeal function. Direct laryngoscopy under general anesthesia with taking of biopsies is the standard diagnostic procedure to detect recurrence when suspicion is raised. This, however, is an invasive and potentially damaging technique. Hence, a non-invasive diagnostic procedure, such as (18)FDG-PET to stratify patients for direct laryngoscopy could be useful. (18)FDG-PET is interpreted visually so that observer variation may affect c

Collision tumor of the thyroid and larynx: a patient with papillary thyroid carcinoma colliding with laryngeal squamous cell carcinoma.

BACKGROUND: The term "collision tumor" refers to the coexistence of two histologically distinct malignant tumors within the same mass. Collision tumors involving the thyroid gland and/or neck region are especially uncommon, with most reported cases involving papillary thyroid carcinoma in coexistence with medullary thyroid carcinoma, follicular carcinoma, and metastatic disease, the latter including a rare occurrence of metastatic liposarcoma and thyroid papillary carcinoma. A collision tumor of papillary thyroid carcinoma and squamous cell carcinoma primary to the thyroid has also been report

The pharmacological effect of epinephrine administration via laryngeal mask airway in a porcine model of asphyxial cardiac arrest.

BACKGROUND: Earlier studies have shown that the laryngeal mask airway (LMA) is an effective alternative method for the administration of drugs. The pharmacological effect of drug administration via the LMA in cardiac arrest is still unknown. This study was designed to compare the effect of epinephrine administration tracheally or through the LMA in an asphyxial resuscitated porcine model. METHODS: Asphyxial cardiac arrest was initiated in 24 pigs, which remained untreated for 2 minutes. After 1 minute of basic life support cardiopulmonary resuscitation, the pigs were assigned to receive epine

Propofol causes less postoperative pharyngeal morbidity than thiopental after the use of a laryngeal mask airway.

The insertion of a laryngeal mask airway (LMA) may result in postoperative sore throat. The choice of induction drug on airway morbidity after LMA insertion may be important. We performed this study to compare the incidence of postoperative pharyngeal morbidity after the insertion of a LMA in 340 patients administered either 2 mg/kg propofol (group P) or thiopental 5 mg/kg (group T) for induction of anesthesia. Patients were maintained at 1-2 minimum alveolar anesthetic concentration sevoflurane in 50% oxygen/air. Spontaneous or assisted spontaneous ventilation was maintained. An investigator

[On the number and morphometrical parameters of the nucleus ambiguous neurons after the injury and regeneration of the recurrent laryngeal nerve in the rat].

INTRODUCTION AND OBJECTIVES: In laryngeal nerves injuries it is essential to know the morpho-functional reorganization of the neurons which supply the larynx in order to be successful with the clinical techniques of functional reinnervation and/or orthotopic transplant. Due to the lack of this type of studies in the literature, we investigated the organization of laryngeal motoneurons in the nucleus ambiguous (NA) after the axotomy and regeneration of the recurrent laryngeal nerve (RLN) in adult rats. MATERIAL AND METHODS: We used biotinylated dextran amines (BDA, 3 kDa), this fact is an inno

Laryngeal mechanisms during human 4-kHz vocalization studied with CT, videostroboscopy, and color Doppler imaging.

The objective of this study was to investigate the underlying laryngeal mechanisms during the specific human 4-kHz vocalization. The laryngeal configuration during this vocalization was measured using high-resolution computerized tomographic scan and videostrobolaryngoscopy. The color Doppler imaging (CDI) of medical ultrasound was used to detect the vibrations of glottal and supraglottal mucosa. During the 4-kHz vocalization, the ventricular folds were adducted in the shape of a bimodal chink and the vocal folds were shaped as a "V" with an opening at the posterior glottis. In the coronal vie

Laryngeal exposure during laryngoscopy is better in the 25 degrees back-up position than in the supine position.

BACKGROUND: Good visualization of the glottis is important for tracheal intubation. This study evaluated whether the 25 degrees back-up position improves the quality of the laryngeal view during laryngoscopy. METHODS: Laryngoscopy with a curved blade was performed on 40 anaesthetized patients. The patients were randomly assigned to two groups. Laryngeal views were captured with a rigid 0 degrees endoscope. Views were recorded for each patient in Group A (n = 20) during laryngoscopies performed with the patient lying first in the supine position and then in the 25 degrees back-up position. Lar

Prognostic significance of surgical margins in transoral CO2 laser microsurgery for T1-T4 pharyngo-laryngeal cancers.

Histological affected or close margin is an adverse factor in conventional surgery of larynx-hypopharynx cancer. Our objective was to analyze the relevance of the margins in transoral laser microsurgery (TLM). A retrospective study of 357 consecutive patients with cancer of the larynx and hypopharynx (T1-T4) treated with TLM. Three possible margins were considered: tumor free, affected, and uncertain. An affected margin showed marked tumor infiltration. An uncertain margin was defined when the sample was insufficient, when it showed carbonization impeding accurate evaluation, or when tumor cel
