The effect of Trichinella spiralis infection on the mechanical properties of the mammalian diaphragm.

Trichinella spiralis larvae infect and develop within skeletal muscle cells causing major changes to their mechanical properties. The aim of this investigation was to determine the effects of T. spiralis on the power output and fatigue resistance of the mammalian diaphragm under conditions simulating in vivo operation and to relate these to respiratory performance. Infection with T. spiralis leads to major reductions in mechanical stress, work, power output and fatigue resistance. These changes are associated with the number of larvae present in the muscle and the duration of infection. Howeve

Isolated triceps weakness in exercise-induced radial neuropathy.

OBJECTIVE: We report a case of isolated triceps weakness resulting from radial neuropathy. PATIENT: A middle-aged healthy man, a keen athlete, developed acute weakness of the triceps brachii muscle due to radial neuropathy. No other radial nerve innervated muscles were involved. Triceps function completely recovered in six weeks clinically and electrophysiologically suggesting focal conduction block secondary to demyelination. DISCUSSION: Acute radial neuropathy after strenuous arm exercise resulting from a compression lesion with acute conduction block, has been previously described. Howeve

[The rectus abdominis syndrome].

Three athletes (one female, two males), aged 18-21 years, developed acute abdominal pain, two of them immediately after exercise including abdominal muscle training. The female patient had a pulmonary infection of uncertain cause at the time. The second patient obviously suffered from an allergic or parasitic disease (eosinophilia of 26%). The third patient, a swimmer, had the symptoms in the course of a flu-like infection after drinking about 100 g alcohol. Serum creatinine kinase activity was 7,800-17,500 U/l, making acute muscle damage likely, probably rhabdomyolysis. Ultrasound examination

[A child case of atypical chronic fasciitis--diagnostic usefulness of muscle CT scan].

Eosinophilic fasciitis is an uncommon disorder characterized by peripheral blood eosinophilia, hypergamma-globulinemia, and inflammation of fascia and skin. Its typical symptom in the acute stage includes muscle pain and swelling, followed by slowly progressive joint contractures. We report an 11-year-old girl with pathological diagnosis of chronic fasciitis, probable eosinophillic fasciitis. Her first symptom was chronic symmetrical joint contractures, which developed when she was 4 years old. She had atypical course as compared with patients described in literature, lack of acute symptom and

Intractable muscle pain syndrome, osteomalacia, and axonopathy in long-term use of phenytoin.

Muscle pain syndrome is a complicated situation in that even an extensive work-up may overlook the primary etiology. A patient with multiple medical problems, including a history of partial gastrectomy due to trauma, a below-knee amputation from arteriosclerotic disease, and chronic phenytoin therapy for a seizure disorder, developed intractable, generalized muscle pain, with total dependency in mobility and self care. He was admitted to an acute care hospital and was eventually discharged with a diagnosis of occult malignancy. After being referred to our amputee clinic for prosthetic manageme

Malignant hyperthermia in the otolaryngologic patient: prospective anesthetic and surgical management of eight children.

Malignant hyperthermia (MH) is a rare genetic myopathy whose hallmark is rise in body temperature. This hypermetabolic state is triggered by inhalational anesthetics and/or depolarizing muscle relaxants such as succinylcholine. Even the use of dantrolene may not be protective against the hypermetabolic crisis. Eight patients at risk for MH undergoing tonsillectomy, adenoidectomy, and/or myringotomy with ventilation tube insertion were anesthetized with nitrous oxide, barbiturates, opiates, tranquilizers, and nondepolarizing muscle relaxants without dantrolene and without complication. Cardiac

Severe cardiopathy in branching enzyme deficiency.

A 7 1/2-year-old girl had exercise intolerance and exertional dyspnea. Four months later, congestive heart failure developed, with recurrent chylous pleural effusions, and she died at age 8 1/2 years. Endomyocardial biopsy tissue showed abundant PAS-positive, diastase-resistant cytoplasmic deposits. Similar inclusions were seen in muscle, skin, and liver specimens. Postmortem studies showed that the abnormal polysaccharide was especially abundant in heart and muscle, but was also present in all other tissues, including the central nervous system. Glycogen isolated from heart, muscle, and spina

Disseminated intravascular coagulation induced by generalized cytomegalic inclusion disease during steroid therapy for polymyositis.

A 56-year-old woman came to the hospital with fever and skin eruptions. A rise in myogenic enzyme and the presence of antileucocyte antibody were noticed, along with the gradual appearance of myalgia in both lower extremities, and muscle weakness. Steroid therapy was started under the diagnosis of polymyositis. The steroid was reduced because of mental disturbance but immediately the patient developed high fever. Various forms of treatment were carried out but there was no improvement, and the patient died. At autopsy there were scattered purpura on the skin, and the muscles were atrophic and

Endocrine disorders associated with obesity.

Several endocrine disorders, including diabetes, insulinoma, Cushing syndrome, hypothyroidism, polycystic ovarian syndrome, and growth hormone deficiency, are associated with obesity. The mechanisms underlying the development of obesity vary according to the abnormalities of endocrine function. The primary actions of insulin, glucocorticoids (GCs), thyroid hormone, and growth hormone are associated with energy metabolism in the liver, muscle, adipose tissue, and other tissues. This chapter describes the pathogenesis of obesity and metabolic dysfunction associated with excess insulin or GCs and

Heart failure in diabetes.

Heart failure and cardiovascular disorders represent the leading cause of death in diabetic patients. Here we present a systematic review of the main mechanisms underlying the development of diabetic cardiomyopathy. We also provide an excursus on the relative contribution of cardiomyocytes, fibroblasts, endothelial and smooth muscle cells to the pathophysiology of heart failure in diabetes. After having described the preclinical tools currently available to dissect the mechanisms of this complex disease, we conclude with a section on the most recent updates of the literature on clinical manage

Fibro-adipogenic progenitors enhance functional and structural properties of human 3D tissue engineered skeletal muscles.

Human skeletal muscle models often lack important supportive cell types. Here we developed a co-culture three-dimensional tissue engineered skeletal muscle (3D-TESM) model by combining myogenic progenitors (MPs) with genetically-matched immortalized fibro-adipogenic progenitors (iFAPs). FAPs play a crucial physiological role in myogenesis, tissue remodeling and extracellular matrix (ECM) formation. We demonstrate that co-culture 3D-TESMs effectively recapitulate these processes under controlled conditions, thereby enhancing contractile force, muscle tissue integrity and longevity, as well as i

Orbital Floor Fracture With Haller Cells and Inferior Rectus Injury Post-Endoscopic Sinus Surgery.

A 27-year-old male developed diplopia and gaze limitation after endoscopic sinus surgery (ESS) due to an iatrogenic inferior rectus (IR) muscle injury caused by an orbital floor fracture involving Haller cells. Preoperative CT and MRI confirmed this injury. Intraoperatively, a fracture and soft tissue herniation were found and their reduction improved upward extraocular movement (EOM), although downward EOM remained limited. Although initial postoperative improvement was observed, at the 1-year follow-up, the patient showed overall recovery compared with the initial injury but declined from th

Dengue fever complicated with bilateral Psoas hematoma and superadded infection: A case report.

Dengue hemorrhagic fever (DHF) typically presents with various bleeding manifestations such as epistaxis, gum bleeding, and gastrointestinal bleeding. However, spontaneous large muscle hematoma formation is a rare complication. This case report discusses a patient with DHF who developed bilateral psoas muscle hematomas, a very uncommon presentation. The patient experienced acute abdominal pain and was diagnosed with psoas muscle hematomas complicated by bacterial infection. Conservative treatment, including fluid management, blood transfusions, and antibiotics, led to the patient's full recove

Muscular and neuronal control of voice production - forgotten findings, current concepts, and new developments.

Voice production has been an area of interest in science since ancient times, and although advancing research has improved our understanding of the anatomy and function of the larynx, there is still little general consensus on these two topics. This review aims to outline the main developments in this field and highlight the areas where further research is needed. The most important hypotheses are presented and discussed highlighting the four main lines of research in the anatomy of the human larynx and their most important findings: (1) the arrangement of the muscle fibers of the thyroaryteno

Delayed-Onset Muscle Soreness of the Psoas Major Muscle Following Abdominal Training: Case Report.

Edema of the psoas major muscle can indicate various pathologies, such as infection, malignancy, and trauma, but it can also result from benign causes like delayed-onset muscle soreness (DOMS). This article presents the case of a 44-year-old female patient who developed DOMS in the psoas major after participating in an intense abdominal workout. The patient reported hip pain that began a day after the workout, which was confirmed by magnetic resonance imaging (MRI) revealing significant edema in the psoas major muscles, particularly on the right side. Conservative treatment with rest and analg

Single cell sequencing maps skeletal muscle cellular diversity as disease severity increases in dystrophic mouse models.

Duchenne muscular dystrophy (DMD) is caused by out-of-frame mutations in the DMD gene resulting in the absence of a functional dystrophin protein, leading to a devastating and progressive lethal muscle-wasting disease. Little is known about cellular heterogeneity as disease severity increases. Advances in single-cell RNA sequencing (scRNA-seq) enabled us to explore skeletal muscle-resident cell populations in healthy, dystrophic, and severely dystrophic mouse models. We found increased frequencies of activated fibroblasts, fibro-adipogenic progenitor cells, and pro-inflammatory macrophages in

New phenotype of severe neonatal episodic laryngospasm due to a missense mutation in SCN4A: A case report and literature review.

Severe neonatal episodic laryngospasm (SNEL) is an ion channel disease characterized by recurrent life-threatening myotonia of respiratory muscle due to mutations in the voltage-gated sodium channel genes. Here we reported a newborn manifested as paroxysmal cyanosis and limb myotonia after birth. The neonate also developed muscle hypertrophy and stunted growth during the follow-up. Whole exome sequencing confirmed c.2395G>A, p.Ala799Thr heterozygous mutation of SCN4A. Carbamazepine was found to be effective on treating the disease. This case expands our understanding of the phenotype resulting

A syringe adapter for reduced muscular strain and fatigue.

Workers in hospitals, clinics, and contract research organizations who repetitively use syringes have an increased risk for musculoskeletal disorders. This study developed and tested a novel syringe adapter designed to reduce muscle strain associated with repetitive fluid draws. Three syringe plunger extension methods (ring-finger, middle-finger, and syringe adapter) were studied across twenty participants. Electromyogram signals for the flexor digitorum superficialis and extensor digitorum muscles were recorded. The syringe adapter required 31% of the 90th percentile flexor muscle activity as
