Orbital mucormycosis with retinal and ciliary artery occlusions.

A 61-year-old man presented with acute, painful loss of vision in the left eye due to a central retinal artery occlusion. Fluorescein angiography confirmed the central retinal artery occlusion and also identified a nasal posterior ciliary artery occlusion. CT scanning revealed a left medial orbital mass with adjacent ethmoid sinusitis. Transnasal ethmoid biopsy disclosed mucormycosis. A left external ethmoidectomy, maxillectomy, and orbital exploration were performed, after which the patient was treated with daily intravenous amphotericin B for six weeks. Coexistence of retinal and nasal poste

Hospitalization of children with status asthmaticus: a review.

A review of hospital admissions of patients with status asthmaticus at the Childrens Hospital of Los Angeles showed a marked increase in admissions in recent years. Asthma mortality did not increase. Patients tended to be young, boys, and black as compared with patients admitted to a general hospital. Interviews with 100 patients and/or their parents admitted between February and June 1988 indicated that the majority of patients had frequent, severe, and/or disabling symptoms and a significant number were undertreated. Forty-five percent of these patients and 46% of all patients admitted betwe

How to stop a nosebleed.

Epistaxis is a common emergency usually caused by local trauma to the nasal mucosa. Occasionally, it may result from systemic diseases such as atherosclerosis, hypertension, or coagulopathy. Anterior nosebleed often can be controlled with topical vasoconstriction and cautery. Intranasal packing may be necessary to tamponade bleeding vessels. Occlusion of the sinus ostia by anterior nasal packing may necessitate prophylactic use of antibiotics to prevent sinusitis. Posterior nosebleed requires nasopharyngeal packing. Patients must be closely monitored and given supplemental high-humidity oxygen

[Common variable hypogammaglobulinemia. Apropos of a new case].

The authors report the case of a 17 year-old girl who presented with typical clinical features (pneumonia, sinusitis, otitis and diarrhoea) of Variable Primary Hypogammaglobulinemia for over 15 years. Immunodeficiency investigations showed a defective antibody response with a panhypogammaglobulinemia but normal cell-mediated immunity. Serum protein electrophoresis and detection of natural and acquired antibodies in the blood are the basic biological tests to run for a quick diagnosis. The main long term complication consists of the risk of neoplasia. The patient recovered a good clinical statu

Benefit of intravenous IgG replacement in hypogammaglobulinemic patients with chronic sinopulmonary disease.

Seven patients with hypogammaglobulinemia and chronic sinopulmonary infections were treated with a preparation of intravenous gammaglobulin. In order to maintain levels of serum IgG at greater than 500 to 750 mg/dl four weeks after infusion, 0.6 g/kg was administered every month. Stable serum levels were achieved after three to eight months. After six to 12 months of this regimen, there was significant reduction in acute infections requiring hospitalization, amelioration of clinical and radiographic evidence of chronic maxillary sinusitis, and improvement in pulmonary symptoms and pulmonary fu

Activation of the impaired nasal mucociliary transport in children: preliminary report.

The effect of HR-6 containing adenosine triphosphate on the impaired mucociliary transport was studied in 13 children. The main symptoms in the children were chronic non-allergic rhinitis, recurrent otitis media, recurrent or chronic maxillary sinusitis and infections of the lower respiratory tract. Initially, the mucociliary transport rate ranged from 0 to 5.6 mm/min, mean 1.8 mm/min. HR-6 increased the mucociliary transport rate by 1.3-3.5, mean 2.0 mm/min. The corresponding figures for the placebo were from 0 to 0.4, mean 0.2 mm/min. These preliminary results indicate further studies with H

Wegener's granulomatosis in a 7 year old child.

A 7 year old boy presented with a history of recurrent episodes of sinusitis and otitis. He developed paresis of the external muscle of his right eye with exophthalmos. A biopsy of his sinus mucosa demonstrated necrotizing granulomatous involvement of the membranes with the presence of multiple giant cells interspersed in zones of fibrosis. A diagnosis of Wegener's granulomatosis was entertained. However the absence of pulmonary and renal manifestations did not allow for a definitive diagnosis to be made. He later developed hematuria and a renal biopsy demonstrated a renal lesions consistent w

Upper and lower airway disease in penicillamine treated patients with rheumatoid arthritis.

Four of 133 patients with rheumatoid arthritis suddenly developed progressive respiratory symptoms while receiving penicillamine therapy. None had previous disease. None of 89 patients treated with gold alone and 0 of 380 not taking gold or penicillamine developed respiratory symptoms. Two of the 4 developed simultaneous severe, unresponsive sinusitis. Lung biopsy demonstrated obliterative bronchiolitis (OB) in 2 patients, fibrosing alveolitis in one. A clinical course characteristic of OB was noted in the 4th patient. Sinus biopsy in 2 demonstrated lymphocytic and plasmacytic mucosal infiltra

[Focal infections in the head and neck region. An interdisciplinary problem].

The concept of focal infection is still valid. Besides hematogenous spread of bacteriae and their toxins also the proliferation of infection along the mucosal inner surfaces of the upper aero-digestive tract may be the origin of remote secondary manifestations. The diagnostic of chronic tonsillitis or occult sinusitis is sometimes difficult. While the ENT-specialist is responsible for both the detection of a focus in the head and neck region and its safe removal, the interrelationship between a focus and its actual secondary disease must be diagnosed by the inquiring discipline (general practi

Antibiotics and nasal surgery.

Despite the widespread use of prophylactic antibiotics in nasal surgery, the subject has been paid little attention in the literature. In a retrospective analysis of 210 patients who underwent septal surgery or a rhinoplasty, only 5 developed minor infections. Patients undergoing septal surgery or rhinoplasty (174) were evaluated in a randomized sequence, prospectively, to study the efficacy of systemic antibiotic prophylaxis. Only 4 patients developed minor infections; and 173 patients demonstrated no clinical nor radiographic evidence of subsequent sinusitis. The incidence of infectious comp

Hyperimmunoglobulinemia E syndrome: radiographic observations.

Susceptibility to recurrent staphylococcal cutaneous and respiratory infections beginning in infancy associated with extreme hyperimmunoglobulinemia E is a recently described primary immunodeficiency syndrome. Other clinical features include depressed cellular immunity and deficient antibody formation. Recurrent pneumonia and cyst formation with variable persistence and expansion characterized the radiographic couse in 11 patients. Five cysts resolved with continuous antistaphylococcal therapy; 2 were resected without recurrence; and 4 persisted after surgery and/or antibiotics (2--8 years). T

Invasive sinus aspergillosis with perineural spread in an immunocompetent patient.

Invasive aspergillosis of paranasal sinuses is usually seen in immunocompromised patients and is associated with a very high mortality rate. The disease is distinctly uncommon in immunocompetent patients. A rare case of invasive aspergillosis of paranasal sinus and orbit with intracranial extradural extension via perineural spread along the maxillary division of the trigeminal nerve in an immunocompetent patient is presented.

Rosacea fulminans associated with pegylated interferon alpha-2B and ribavirin therapy.

Rosacea fulminans is characterized by the sudden onset of large, coalescing nodules and draining sinuses on the face. A few reports have linked medications to this condition, but none have described the onset of rosacea fulminans with pegylated interferon or ribavirin therapy. We report a patient who presented with rosacea fulminans after initiation of therapy for Hepatitis C.

Sphenochoanal polyp: diagnose and treatment.

Choanal polyps (CP) are benign mucous tumours that usually originate in the mucosa of the maxillary sinus, and hence are termed antrochoanal polyps. CP rarely originates in the mucosa of other nasal structures and paranasal sinuses. The purpose of this brief clinical communication is to present six cases of CP originating in the sphenoidal sinus and termed sphenochoanal polyps (SCP).

Inferior concha bullosa--a radiological and clinical rarity.

Two cases of inferior concha bullosa (ICB) are reported. The condition was bilateral in one patient and unilateral in the other. Unilateral ICB was associated with marked septal deviation. The diagnosis was made in patients being investigated for chronic rhinosinusitis. ICB is diagnosed by computed tomography (CT) of the sinuses in the coronal plane. It may also be seen in axial views.

Chronic hypertrophic polypoid rhinosinusitis.

Chronic hypertrophic polypoid rhinosinusitis is a condition in which the mucous membrane lining the nose and paranasal sinuses becomes markedly hypertrophied, and nasal polyps become evident. Superimposed infection is common. Bony erosion, sometimes marked, may also result. Because of the alterations in bone, the condition may be mistaken for other more serious lesions.

Atypical dermoid sinus in a chow chow dog.

A case of multiple dermoid sinuses in the dorsal cervical and craniothoracic regions in an adult chow chow dog is described. One sinus did not open on the skin surface. This is the first reported case of the condition in this breed and the first time absence of the sinus opening on the skin is described. The use of the term pilonidal sinus to describe this condition is challenged.
