Peritoneoscopic placement of Swan neck peritoneal dialysis catheters.

Peritoneoscopic placement of peritoneal dialysis catheters, although accomplished in only about 10% of dialysis centers, is a nonsurgical technique that fulfills requirements for safety and dependability. Over a 40-month period, 136 catheters were placed with the peritoneoscope, 135 of which were double-cuffed, Swan neck curled catheters, with a uniform radiopaque stripe. Patients were followed longitudinally for outcome. Catheters were placed in 44 diabetic patients, 1 human immunodeficiency virus (HIV)-positive patient, and 18 morbidly obese patients. No complications occurred as a direct re

The effect of joint angles on shot performance during the swing with different clubs among female professional golfers.

OBJECTIVE: This study aimed to investigate the differences in body joint angles during driver, 5-iron, and 7-iron swings in female professional golfers and their relationships with shot performance, providing a biomechanical basis for technical training. METHODS: Twelve female professional golfers were recruited. A Qualisys three-dimensional motion capture system (250 Hz) and a FlightScope Xi launch monitor were used to collect data at four key swing phases: half backswing (HB), top of backswing (TOP), half downswing (HD), and impact (IMP). Joint angles of the head, neck, torso, pelvis, elbow

Hyper-buoyancy flotation increases cervical disc height and reduces vertebral stiffness, with only partial reversal after acute 1 g axial loading.

Exposure to microgravity is associated with stature increases, moderate-to-severe back/neck pain and elevated lumbar and cervical intervertebral disc (IVD) herniation risk post-flight. Whilst lumbar pathophysiology has been investigated, little attention has been placed on the cervical spine. Thus, the aim of this study was to determine the effect of 4 h of hyper-buoyancy flotation (HBF) and subsequent 15 min of upright 1 g exposure on stature, cervical IVD height, cervical muscle cross-sectional area (CSA) and thickness, passive cervical vertebral stiffness, and their association with stature

[Restoration of upper-limb function in patients with cervical spinal cord lesion].

PURPOSE OF THE STUDY: Injury to the spinal chord at the C5 level results in a severe handicap to the patient because of a loss of active motion of the wrist and hand. Selected surgical procedures can effectively restore active extension of the wrist and provide conditions for some vital pinching and gripping abilities. The deltoid muscle also has a potential for restoration of upper limb function, because its posterior portion can be used to restore elbow extension. The aim of this clinical retrospective study was to describe and evaluate the techniques used to improve the quality of life in p

Open Release of Pediatric Trigger Thumb.

UNLABELLED: Open release of a trigger thumb has been shown to be the most reliable option to restore full interphalangeal (IP) joint extension and thus normal thumb-joint motion in children1. The aim of this procedure is to restore free gliding of the flexor pollicis longus (FPL) tendon in its canal in children with fixed IP joint flexion contractures or those in whom nonoperative treatment modalities have failed. DESCRIPTION: The surgical procedure is easy to perform and straightforward; however, attention must be given to several details in order to avoid surgical failure and complications.

Paediatric Sternoclavicular Joint Pseudodislocation Fixation Technique.

BACKGROUND: Posterior sternoclavicular joint (SCJ) injuries in children are rare and frequently misdiagnosed. Due to the anatomy of the immature medial clavicle, many of these injuries are Salter-Harris type I/II physeal fractures rather than true joint dislocations-termed pseudodislocations. These injuries require careful diagnosis and tailored management to ensure safe and functional outcomes. INDICATIONS: Surgical intervention is indicated in paediatric SCJ pseudodislocations with significant posterior displacement of the medial clavicular fragment, particularly when the posterior metaphys

Volar approach combined with small dorsal incision in the treatment of AO type C distal radius fracture: A retrospective cohort study.

This study aims to explore the clinical efficacy of volar approach combined with small dorsal incision in the treatment of AO type C fracture of the distal radius. From January 2020 to December 2022, a total of 126 patients with AO type C fracture of the distal radius were treated. The treatment was conducted using a volar approach combined with a small dorsal incision. Initially, the Henry approach was employed from the volar side to expose the fracture site. Following reduction, Kirschner wires were used for temporary fixation. Subsequently, a small incision was made on the dorsal side to ex

A Prospective Study on Deep Inspiration Breath Hold Thoracic Radiation Therapy Guided by Bronchoscopically Implanted Electromagnetic Transponders.

BACKGROUND: Electromagnetic transponders bronchoscopically implanted near the tumor can be used to monitor deep inspiration breath hold (DIBH) for thoracic radiation therapy (RT). The feasibility and safety of this approach require further study. METHODS: We enrolled patients with primary lung cancer or lung metastases. Three transponders were implanted near the tumor, followed by simulation with DIBH, free breathing, and 4D-CT as backup. The initial gating window for treatment was ±5 mm; in a second cohort, the window was incrementally reduced to determine the smallest feasible gating window

Accuracy and complications of CT-guided pulmonary core biopsy in small nodules: a single-center experience.

BACKGROUND: Computed tomography (CT)-guided pulmonary core biopsies of small pulmonary nodules less than 15 millimeters (mm) are challenging for radiologists, and their diagnostic accuracy has been shown to be variable in previous studies. Common complications after the procedure include pneumothorax and pulmonary hemorrhage. The present study compared the diagnostic accuracy of small and large lesions using CT-guided core biopsies and identified the risk factors associated with post-procedure complications. METHODS: Between January 1, 2016, and December 31, 2017, 198 CT-guided core biopsies

Design and preliminary analysis of a vaginal inserter for speculum-free cervical cancer screening.

OBJECTIVE: Cervical cancer screening usually requires use of a speculum to provide a clear view of the cervix. The speculum is one potential barrier to screening due to fear of pain, discomfort and embarrassment. The aim of this paper is to present and demonstrate the feasibility of a tampon-sized inserter and the POCkeT Colposcope, a miniature pen sized-colposcope, for comfortable, speculum-free and potentially self-colposcopy. STUDY DESIGN: We explored different designs using 3D computer-aided design (CAD) software and performed mechanical testing simulations on each. Designs were rapid pro

Intratympanic dexamethasone injection effects on transient-evoked otoacoustic emission.

PURPOSE: To investigate the effects of intratympanic dexamethasone injection, which is done because of tinnitus, on transient evoked otoacoustic emission (TEOAE) and so determine whether given dexamethasone cause any damage in the inner ear. PATIENTS AND METHODS: Twenty-six patients, aged between 32 and 75, with subjective tinnitus, were randomly selected. The selected patients were the ones whose improvement had not been achieved through minimum 6 months' medical therapy (eg, Ginkgo biloba extract EGb 761, betahistidine, and trimetazidin) and who were free of systemic or otolaryngologic dise

Type V osteogenesis imperfecta: a new form of brittle bone disease.

Osteogenesis imperfecta (OI) is commonly subdivided into four clinical types. Among these, OI type IV clearly represents a heterogeneous group of disorders. Here we describe 7 OI patients (3 girls), who would typically be classified as having OI type IV but who can be distinguished from other type IV patients. We propose to call this disease entity OI type V. These children had a history of moderate to severe increased fragility of long bones and vertebral bodies. Four patients had experienced at least one episode of hyperplastic callus formation. The family history was positive for OI in 3 pa

Value of the plasma norepinephrine/3,4-dihydroxyphenylglycol ratio for the diagnosis of pheochromocytoma.

PURPOSE: The purpose of this study was to assess whether the plasma norepinephrine/3,4-dihydroxyphenylglycol ratio (NE/DHPG) is of diagnostic relevance for patients with a pheochromocytoma. SUBJECTS AND METHODS: In 18 patients with a histologically proven pheochromocytoma and in nine patients with congestive heart failure, plasma levels of NE, epinephrine (EPI), and DHPG (radioenzymatic method) were determined after 20 minutes of supine rest. In 10 healthy subjects, the plasma catecholamine responses to active standing (5 minutes) and mental arithmetic (5 minutes) were measured. From the plas

Repositioning for pressure injury prevention in adults.

RATIONALE: Pressure injuries (or pressure ulcers) are localised damage to skin or tissue (or both) occurring over bony prominences, resulting from prolonged pressure or shear forces (or both). Adults receiving care in any healthcare setting can develop pressure injuries. Immobility, malnutrition, and reduced sensation are some known risk factors. Regular repositioning is a theoretically sound prevention strategy that is part of standard patient care. This is the second update of a review published in 2014 and updated in 2020. OBJECTIVES: To evaluate the benefits and harms, and cost-effectiven

Optic nerve sheath diameter increases during 12◦ head-down tilt in healthy volunteers: a crossover pilot study informing the design of a spaceflight countermeasure trial.

BACKGROUND: Spaceflight-associated neuro-ocular syndrome (SANS) is hypothesized to arise from a headward fluid shift that elevates intracranial pressure and distends the optic nerve sheath. Head-down tilt (HDT) is the ground-based analog of microgravity, and optic nerve sheath diameter (ONSD) measured by transorbital B-mode ultrasound is a candidate non-invasive marker of intracranial pressure. Before a definitive trial in which lower body negative pressure (LBNP) is added to 12◦ HDT as a putative countermeasure, we conducted a pilot study to (i) confirm that 12◦ HDT detectably elevates ONSD w

Resting beat-to-beat blood pressure variability is preserved in young adults with major depressive disorder.

Major depressive disorder (MDD) often first emerges during young adulthood and is associated with an increased risk of future hypertension, but our understanding of blood pressure (BP) regulation in young adults with MDD who are otherwise clinically healthy remains limited. We tested the hypothesis that beat-to-beat BP variability (BPV) would be greater in young unmedicated adults with MDD compared with nondepressed healthy adults (HA). Because the arterial baroreflex is essential for beat-to-beat BP regulation, we also hypothesized that 1) sympathetic baroreflex sensitivity would be reduced i

Effects of Fibular Plate Fixation and Deep Posterior Tibiotalar Ligament Repair on Ankle Stability in a Weber B Fracture Model with Complete Deltoid Ligament Sectioning.

BACKGROUND: Weber B (supination external rotation) ankle fractures with complete deltoid ligament rupture (SER4b) are typically considered unstable and treated with plate fixation of the fibula. Recent studies have suggested a significant ankle-stabilizing role of the deep posterior tibiotalar ligament (dPTTL), which questions current practice. To our knowledge, there are few studies on the biomechanical effects of plate fixation and none on dPTTL repair. Therefore, this study aims to evaluate the effects of fibular plate fixation and dPTTL repair on ankle stability in a Weber B fracture model

A novel retractor-assisted closed reduction combined with percutaneous pinning fixation for the treatment of elderly distal radius fractures: a retrospective cohort study.

BACKGROUND: Percutaneous pinning fixation (PCP) has been used for the treatment of distal radius fractures for decades, especially in the elderly with fragile soft tissue. However, achieving and maintaining a sound anatomic reduction before PCP is difficult if we use the manipulative reduction method alone. Our study innovatively applied the Steinmann pin retractor for closed reduction combined with PCP, to provide a new protocol for the treatment of distal radius fractures. METHODS: From March 2017 to July 2018, 49 patients out of 57 that met the inclusion criteria but not the exclusion crit
