Stronger back muscles reduce the incidence of vertebral fractures: a prospective 10 year follow-up of postmenopausal women.

The long-term protective effect of stronger back muscles on the spine was determined in 50 healthy white postmenopausal women, aged 58-75 years, 8 years after they had completed a 2 year randomized, controlled trial. Twenty-seven subjects had performed progressive, resistive back-strengthening exercises for 2 years and 23 had served as controls. Bone mineral density, spine radiographs, back extensor strength, biochemical marker values, and level of physical activity were obtained for all subjects at baseline, 2 years, and 10 years. Mean back extensor strength (BES) in the back-exercise (BE) gr

Low back pain: development and five-year prospective application of a computerized quality-based diagnostic and treatment protocol.

A standardized protocol for low back pain was developed and computerized so that it could be used as a concurrent monitoring system for large patient populations. The software incorporated a relational database management system (RDBMS) and C language, a flexible, general-purpose programming language that is fast and portable. The protocol was then applied to a uniform group of industrial patients for 5 years. The results demonstrated that a quality-based protocol could be successfully computerized and applied to a large group of patients as a concurrent monitoring system. Quality care was ins

The efficacy of a multimodal physical activity intervention with supervised exercises, health coaching and an activity monitor on physical activity levels of patients with chronic, nonspecific low back pain (Physical Activity for Back Pain (PAyBACK) trial): study protocol for a randomised controlled trial.

BACKGROUND: Physical activity plays an important role in the management of chronic low back pain (LBP). Engaging in an active lifestyle is associated with a better prognosis. Nevertheless, there is evidence to suggest that patients with chronic LBP are less likely to meet recommended physical activity levels. Furthermore, while exercise therapy has been endorsed by recent clinical practice guidelines, evidence from systematic reviews suggests that its effect on pain and disability are at best moderate and not sustained over time. A limitation of current exercises programmes for chronic LBP is

Effectiveness of dynamic neuromuscular stabilization-based interventions for nonspecific low back pain: a systematic review and meta-analysis.

INTRODUCTION: Nonspecific low back pain (NLBP) is a leading cause of disability and imposes substantial public health and socioeconomic burdens. Dynamic Neuromuscular Stabilization (DNS) is increasingly used in NLBP rehabilitation, but quantitative evidence of its effects remains limited. We systematically reviewed and meta-analyzed controlled trials evaluating DNS-based interventions for NLBP. METHODS: PubMed, Web of Science, Scopus, the Cochrane Library, and CNKI were searched. Two reviewers independently screened studies and extracted data. Random-effects meta-analysis, subgroup analyses,

From information to action; co-designing pain neuroscience education for adolescents with low back pain: Focus group insights.

INTRODUCTION: Low back pain (LBP) is a common health problem among adolescents. Adolescents with pain report higher stress, more school absenteeism, poorer academic performance, and frequent healthcare use. LBP during adolescence may increase the risk of pain in adulthood, underscoring the importance of supporting young people in developing strategies to understand and manage pain. OBJECTIVE: To develop educational material based on pain neuroscience education (PNE) through co-design with adolescents experiencing LBP. METHODS: Adolescents 15-19 years old were recruited to review an initial p

Investigating the effects of back-support exoskeletons on work posture: An automated AI-based NIOSH measurement using YOLO.

Work-related musculoskeletal disorders (WMSDs) are prevalent among masons with 58% reporting lower back pain from repetitive lifting and sustained trunk flexion. Back-support exoskeletons (BSEs) reduce muscle activation and claim to improve posture, yet evidence of their effectiveness in improving posture remains limited. This study investigated whether BSEs improve posture among 23 experienced masons performing masonry tasks, comparing rigid and soft designs. Participants lifted 17.2 kg concrete masonry units with and without exoskeletons. Using YOLO v11 pose estimation to analyze 351,615 vid

The hidden architecture of back pain: ultrasound-based lumbar multifidus pennation angle analysis - a cross-sectional pilot study.

BACKGROUND: Chronic low back pain (CLBP) is associated with structural and functional alterations in the lumbar-multifidus (LM) muscle, a key-stabilizer of the spine. In this pilot study, Pennation-angle, a well-established muscle architecture marker not previously assessed in CLBP, was evaluated using ultrasound to compare LM structure between CLBP-patients and healthy-controls and examine its relationship with muscle thickness, pain-intensity and activity-levels. METHODS: This cross-sectional study included 40 adults aged 18–35 years (20 CLBP-patients, 20 healthy-controls) at AIIMS Jodhpur.

Intelligence without intuition: a mixed-methods pilot study on reasoning models in musculoskeletal physiotherapy for low-back pain.

Musculoskeletal pain, especially low-back pain, is highly prevalent and often challenging to manage due to its multifactorial nature. Effective diagnosis and therapy require clinicians to integrate biopsychosocial information within an evidence-based clinical reasoning framework. Large language models that "think" before responding, so-called reasoning models, show promise to support such complex decision-making, yet their validity and reliability in this setting remain unclear. In our work, we present a comprehensive human evaluation of reasoning models for clinical reasoning. Our results ind
