Magnetic resonance imaging with gadolinium enhancement in renal failure: a need for caution.

A 36-year-old Asian man with von Hippel-Lindau syndrome was referred to the dermatology clinic in January 2005 with stiff fingers and ankles of 3 months' duration. He had undergone bilateral nephrectomy for renal squamous cell carcinoma in 2002 and had started hemodialysis. Multiple cerebellar hemangiomata were resected in 1991, 2001, and 2003, and consequently he underwent multiple magnetic resonance imaging (MRI) scans with gadolinium-based contrast agents whilst on hemodialysis. The serum creatinine level and calculated glomerular filtration rate (GFR) at the time of his first exposure to g

Gene expression changes during retinal development and rod specification.

PURPOSE: Retinitis pigmentosa (RP) typically results from individual mutations in any one of >70 genes that cause rod photoreceptor cells to degenerate prematurely, eventually resulting in blindness. Gene therapies targeting individual RP genes have shown efficacy at clinical trial; however, these therapies require the surviving photoreceptor cells to be viable and functional, and may be economically feasible for only the more commonly mutated genes. An alternative potential treatment strategy, particularly for late stage disease, may involve stem cell transplants into the photoreceptor layer

Efficacy of CLDN18.2-Targeted Radiotheranostics in Patient-Derived Models of Gastric Cancer.

Gastric cancer remains a major global health challenge with few durable treatments, and claudin-18.2 (CLDN18.2) has emerged as an attractive therapeutic target, highlighted by the modest activity of zolbetuximab in clinical trials. To further leverage CLDN18.2 as a therapeutic target, we developed a radiotheranostic strategy using [89Zr]Zr-zolbetuximab and [177Lu]Lu-zolbetuximab for molecular imaging and targeted radiotherapy, respectively. Methods: We performed [89Zr]Zr-zolbetuximab immuno-PET imaging to quantify and localized CLDN18.2 expression in vivo, comparing uptake with [89Zr]Zr-trastu

Variant-to-Biomarker Pathways in Peripheral Artery Disease: Multiomics Integration and Clinical Translation.

Peripheral artery disease (PAD) is a prevalent, disabling manifestation of systemic atherosclerosis that carries high risks of major adverse cardiovascular and limb events, yet remains incompletely explained by conventional risk factors and haemodynamic indices. Although genome-wide association studies have nominated reproducible susceptibility loci and high-throughput profiling has expanded the landscape of circulating, imaging, vascular and skeletal muscle biomarkers, most signals are noncoding, mechanistic attribution is often uncertain and few biomarkers have demonstrated durable increment

Pharmacological management of osteogenesis.

Osteogenesis and bone remodeling are complex biological processes that are essential for the formation of new bone tissue and its correct functioning. When the balance between bone resorption and formation is disrupted, bone diseases and disorders such as Paget's disease, fibrous dysplasia, osteoporosis and fragility fractures may result. Recent advances in bone cell biology have revealed new specific targets for the treatment of bone loss that are based on the inhibition of bone resorption by osteoclasts or the stimulation of bone formation by osteoblasts. Bisphosphonates, antiresorptive agen

Proteomic-guided targeted treatment of leukoencephalopathy with calcification and cysts.

Precision therapeutic approaches for rare neurological diseases are often hampered by a limited molecular understanding of the disease. Here, we report the first application of spatially-resolved proteomics to guide targeted treatment in a monogenic leukoencephalopathy. We investigated leukoencephalopathy with calcifications and cysts (LCC), a rare ribosomopathy caused by biallelic mutations in SNORD118 that leads to progressive cerebral white matter degeneration and vascular proliferation. LCC is currently incurable, though understanding the pathogenesis at the proteomic level can offer novel

Optic Nerve Head Microcirculation in Eyes with Vogt-Koyanagi-Harada Disease Accompanied by Anterior Ischemic Optic Neuropathy.

Anterior ischemic optic neuropathy (AION) is infrequently complicated with Vogt-Koyanagi-Harada (VKH) disease. We quantitatively examined sequential changes in the morphology and circulation hemodynamics, using a C-scan of optical coherence tomography (OCT) and laser speckle flowgraphy (LSFG) in a patient with VKH disease accompanied by AION. A 65-year-old female complained of blurred vision in both of her eyes. The patient presented with optic disc swelling and remarkable choroidal thickening detected by OCT bilaterally. The diagnosis of VKH disease was established based on the presence of pl

From Single Batch to Mass Production-Automated Platform Design Concept for a Phase II Clinical Trial Tissue Engineered Cartilage Product.

Advanced Therapy Medicinal Products (ATMP) provide promising treatment options particularly for unmet clinical needs, such as progressive and chronic diseases where currently no satisfying treatment exists. Especially from the ATMP subclass of Tissue Engineered Products (TEPs), only a few have yet been translated from an academic setting to clinic and beyond. A reason for low numbers of TEPs in current clinical trials and one main key hurdle for TEPs is the cost and labor-intensive manufacturing process. Manual production steps require experienced personnel, are challenging to standardize and

Treatment of primary lymphoma of the lacrimal gland by surgical excision alone: A 5-year follow-up study.

PURPOSE. To evaluate the long-term efficacy of surgical excision alone in the treatment of non-Hodgkin(1)s lymphoma localized to the lacrimal gland. MATERIALS AND METHODS. Thirteen patients with primary lacrimal gland lymphoma were included. The presumptive diagnosis was based on: (1) The painless and relatively slow onset of unilateral lacrimal gland swelling, mostly in elderly adults; (2) The CT pattern of a soft tissue mass in the lacrimal gland region with defined margins and molds to the globe; (3) The negative results of systemic work-up. The involved lacrimal gland was excised via an an

Designing In-situ Gels for Periodontal Repair: A Review of Sustained Release Strategies, Advanced Manufacturing Approaches, and Bio-Fabrication.

Periodontitis is a chronic biofilm-mediated inflammatory disease that progressively destroys the gingiva, periodontal ligament, and alveolar bone. Although scaling and root planing remain the cornerstone of care, incomplete pocket access, rapid microbial recolonization, and the limited residence time of many conventional local formulations can reduce therapeutic effectiveness. The development of in-situ gelling drug delivery systems, administered as low-viscosity liquids that transform within periodontal pockets into mucoadhesive gels, improves retention, prolongs drug contact time, provides s

Bioinspired hydrogel remodels the niche after spinal cord injury for neural reconnection by promoting angiogenesis and reducing inflammation.

Spinal cord injury (SCI) disrupts the local microenvironmental niche, resulting in persistent neuroinflammation, vascular dysfunction, and impaired neuronal regeneration, presenting a significant challenge for tissue repair. In this context, hydrogels offer a versatile strategy for targeted and sustained therapeutic delivery and localized microenvironmental modulation. To facilitate niche restoration following SCI, we developed a bioinspired platform by integrating KTPSLEQRTVYAKGGIKRGK-brain-derived neurotrophic factor mimetic peptides (TPS-BDNF) into a dopamine-modified gelatin methacryloyl h

Fabrication of vesicle-incorporated etoposide-loaded nanocochleates for improved anticancer efficacy.

Cancer is a major global health concern, characterized by uncontrolled cell proliferation and high mortality rates. Conventional chemotherapeutic agents often suffer from poor selectivity and systemic toxicity, limiting their clinical utility. Etoposide, a potent topoisomerase II inhibitor, is widely used to treat various types of cancers such as lung, testicular, and lymphomas. However, its clinical application is hampered due to low bioavailability and rapid clearance. To address, these limitations, nanocarrier-based systems such as liposomes and nanocochleates have been fabricated and explo

BACKGROUND: Metabolic dysfunction-associated steatohepatitis is a progressive liver disease characterized by inflammation and fibrosis, for which effective pharmacological treatments remain limited. Hormones that regulate metabolism, such as glucagon-like peptide-1 and fibroblast growth factor-21, have shown therapeutic promise through complementary metabolic and hepatoprotective actions. This study aims to develop and evaluate a single molecular therapy that engages both pathways to address multiple drivers of disease progression. METHODS: We designed a unimolecular dual agonist by linking g
