Nutraceutical and functional food: efficacy against metabolic disorders and underlying mechanistic insights.

BACKGROUND: The growing global emphasis on health consciousness has significantly increased demand for traditional and complementary medicine products, including functional foods, nutraceuicals, and medicine and food homologous substances (MFHs). The rising prevalence of chronic diseases and the shift toward preventive healthcare have accelerated this market expansion. Although MFHs have been applied for hundreds of years in traditional healthcare systems, contemporary research faces a critical knowledge gap in the systematic organization of scientific evidence. AIM OF THE REVIEW: This review

Adipose-Derived Stem Cells in Ophthalmic Regenerative Medicine: From Cell Replacement to Precision Engineering.

Adipose‑derived stem cells (ASCs) have emerged as a promising cell source in ophthalmic regenerative medicine, attributed to their accessibility, differentiation potential, and paracrine functions. Despite compelling preclinical evidence, clinical outcomes remain unsatisfactory. Departing from the conventional disease‑oriented categorization, this review adopts the cognitive evolution of ASC therapeutic mechanisms as its organizational thread, synthesizing evidence from 158 studies across six ophthalmic subspecialties. Four sequential paradigms are identified: (1) multilineage differentiation

Kinematic features of dysphagia in inclusion body myositis.

Dysphagia often threatens the health of individuals with inclusion body myositis (IBM), significantly contributing to premature mortality. Quality evidence defining IBM-associated dysphagia remains scant and restricted to reports of limited utility. To address this gap, our group recently published a preliminary dysphagia profile for IBM using semi-quantitative impression-based kinematic analysis. In the current study, we offer updated evidence that combines our prior approach with quantitative pharyngeal kinematic analysis to better delineate swallowing features associated with dysphagia in I

Precision nutrition testing is the future of personalized care: evidence and coverage policies for equitable access.

The one-size-fits-all approach to dietary and metabolic interventions often fails to improve population health outcomes. This problem has been made more urgent by the rise of GLP-1 receptor agonist medications for obesity, which are high-cost but have variable patient outcomes. Precision nutrition (PN) testing has the potential to address this gap by using biomarkers, such as genetics and metabolic profiles, to predict individual responses to foods and drug therapies. Although some PN tests are on the market, most are not FDA-approved or covered by insurers, requiring out-of-pocket payment and

Personalizing Peri-Intubation Oxygenation in Patients at Risk of Acute Hypoxemic Respiratory Failure: A Phenotype-Driven Narrative Review of High-Flow Nasal Oxygen and Non-Invasive Ventilation.

Tracheal intubation in patients at risk of acute hypoxemic respiratory failure (AHRF) carries a high risk of life-threatening desaturation, and the choice of peri-intubation oxygenation strategy critically influences patient safety. This narrative review synthesizes current evidence on non-invasive oxygenation techniques-high-flow nasal oxygen (HFNO), non-invasive ventilation (NIV), and their combination-across the pre-oxygenation, apneic, and awake-intubation phases of airway management. We examine the physiological mechanisms underlying each modality, appraise landmark randomized trials and

Movement Through Time: The Evolution of Treatment for People With Parkinson Disease.

BACKGROUND AND PURPOSE: Parkinson disease (PD) is the fastest-growing neurological disorder worldwide, projected to affect approximately 25 million people by 2050. Despite effective pharmacologic management of motor symptoms, patients consistently identify walking, mobility, and balance as their greatest challenges. This Special Interest paper, based on the 2026 Anne Shumway-Cook Lectureship, summarizes a program of research aimed at building the evidence base for rehabilitation in PD and translating that evidence into improved real-world outcomes. BACKGROUND AND PURPOSE: Four interrelated th

Digital-Supported Delivery of Behavioural Therapy for Patients with Tic Disorders: A Narrative Review.

BACKGROUND: Behavioural therapy (BT), including Comprehensive Behavioural Intervention for Tics (CBIT), is an evidence-based first-line treatment for patients with tic disorders. However, access remains limited due to a shortage of trained providers, geographical barriers, costs, and high treatment burden for patients and families. Rapid advances in digital health technologies including telemedicine, web-based treatment platforms, and mobile applications offer new opportunities to expand access to BT for individuals with tic disorders across the lifespan. METHODS: For the purpose of this narr

The single-cell immune atlas of bacterial pneumonia: from inflammatory cell infiltration to treatable cell states.

Bacterial pneumonia remains a major cause of infectious mortality, and antimicrobial resistance has intensified interest in host-directed therapy (HDT). Recent bronchoalveolar lavage fluid (BALF) and peripheral blood atlases (2024-2025) provide single-cell evidence from human disease, but the main translational gap is prioritization rather than catalog expansion. In this review, we reorganize published datasets from bacterial pneumonia and related lung infection cohorts (>500,000 cells; 2020-2026) into a state-prioritization scheme termed "treatable cell states." Evidence is currently stronges

Epitranscriptomics, immunometabolism, and NETosis: unlocking novel mechanisms of traditional Chinese medicine in COPD.

Chronic obstructive pulmonary disease (COPD) is driven by self-perpetuating inflammation that resists conventional therapies. Recent evidence reveals three interconnected pathogenic layers: immunometabolic reprogramming, m6A epitranscriptomic dysregulation, and excessive NETosis with defective resolution. Pathogenic immune cells shift toward aerobic glycolysis and impaired fatty acid oxidation, sustaining pro-inflammatory phenotypes and corticosteroid resistance. m6A modifications may control the stability of key transcripts such as MALT1, IL-17, and PADI4, as suggested by emerging evidence. Development and psychometric validation of the soft skills scale for primary school students.

This study aimed to develop a Soft Skills Scale for primary school students and to determine its psychometric properties. A total of 2,418 primary school students participated in the scale development process. Principal component analysis revealed a six-factor, 19-item structure explaining 41.37% of the total variance. Confirmatory factor analysis provided additional evidence supporting the proposed six-factor structure, with acceptable model fit indices. Criterion-related validity was supported by a moderate positive correlation. Test-retest reliability, assessed over a four-week period, yiel

Type 1 Diabetes Mellitus and Alexithymia: A Systematic Review.

BACKGROUND: Alexithymia has been recognised as a predictor of negative outcomes in various chronic conditions, including type 2 diabetes mellitus (T2DM). However, evidence concerning its role in type 1 diabetes mellitus (T1DM) remains limited. This systematic review aims to explore the relationship between alexithymia and T1DM. METHODS: In June 2025, following PRISMA guidelines, a systematic review was conducted using Scopus, PubMed, and Web of Science databases. Studies specifically addressing the relationship between alexithymia and type 1 diabetes mellitus were analysed. The search strateg

Developed a knowledge base of risk factors for postoperative cognitive dysfunction: a retrospective database study.

Postoperative cognitive dysfunction (POCD), a heterogeneous spectrum of surgery/anesthesia-associated neurocognitive impairments, represents a critical clinical challenge due to its associations with prolonged hospitalization, increased mortality, and accelerated long-term cognitive deterioration. The development of targeted perioperative management strategies fundamentally depends on systematic risk factor identification, though current research remains hampered by significant heterogeneity and incomplete evidence synthesis across observational studies. To address this knowledge gap, we const

Treatment of Osteoporosis in Patients with Chronic Kidney Disease.

PURPOSE OF REVIEW: To discuss current evidence on the diagnosis and management of osteoporosis in patients with chronic kidney disease (CKD). RECENT FINDINGS: Osteoporosis and fractures are prevalent in older CKD patients and associated with poor process indicators and outcomes. While osteoporosis treatment is generally similar in patients without or with CKD up to stage 3, there is still a lack of evidence to guide many areas of osteoporosis management in CKD stages 4-5. There is an urgent need to establish local multidisciplinary care pathways for CKD and dialysis patients with osteoporosis

Sex Differences in Bleeding Risk Associated With Antithrombotic Therapy Following Percutaneous Coronary Intervention.

Background: Antithrombotic therapy is crucial for secondary prevention of cardiovascular disease (CVD), but women with CVD may face increased bleeding complications post-percutaneous coronary intervention (PCI) under antithrombotic therapy. However, women are often underrepresented in clinical trials in this field, so evidence for sex-specific recommendations is lacking. Methods and Results: A search on PubMed was conducted for English-language articles addressing bleeding complications and antithrombotic therapy in women.
