Chiari I malformation presenting as downbeat nystagmus: clinical presentation, diagnosis, and management.

INTRODUCTION: Chiari I malformation is a congenital, neurological condition that is characterized by defects of the skull base resulting in herniation of the cerebellum through the foramen magnum into the cervical spinal canal. Because the condition can result in visual symptoms, patients will often search for answers from their eye care providers; CASE REPORT: A 28-year-old Hispanic diabetic male with a 10-year history of nystagmus was referred to the neuro-ophthalmic disease clinic following the initiation of oscillopsia 1 year previous. Downbeat nystagmus, which worsened in right and down

Relationship between oculomotor scanning determined by the DEM test and a contextual reading test in schoolchildren with reading difficulties.

BACKGROUND: The relationship between oculomotor scanning and reading in poor readers of primary school age is not well known. This study was designed to assess this relationship by determining mean Developmental Eye Movement (DEM) test times and reading speeds in a Spanish non-clinical population of children with poor reading skills but without dyslexia. METHODS: We conducted a cross-sectional study on 81 poor readers (8-11 years of age) in the third to fifth grades recruited from 11 elementary schools in Madrid, Spain. In each subject with best spectacle correction, oculomotor scanning was m

Monocular elevation deficiency with contralateral superior oblique muscle tendon laxity.

INTRODUCTION: Monocular elevation deficiency with a tight ipsilateral inferior rectus muscle and fixation preference for the contralateral eye is a well recognized entity in strabismus. Most descriptions of the condition, however, do not document oblique muscle forced duction testing in the contralateral eye. The purpose of this report is to raise awareness that monocular elevation deficiency can be associated with, and may be secondary to, a floppy (lax) superior oblique muscle tendon in the contralateral fixating eye. METHODS: Retrospective case series. RESULTS: All 4 patients were previou

Interesting objects are visually salient.

How do we decide which objects in a visual scene are more interesting? While intuition may point toward high-level object recognition and cognitive processes, here we investigate the contributions of a much simpler process, low-level visual saliency. We used the LabelMe database (24,863 photographs with 74,454 manually outlined objects) to evaluate how often interesting objects were among the few most salient locations predicted by a computational model of bottom-up attention. In 43% of all images the model's predicted most salient location falls within a labeled region (chance 21%). Furthermo

Object-based anisotropic mislocalization by retinotopic motion signals.

The relative visual positions of briefly flashed stimuli are systematically modified in the presence of motion signals. We have recently shown that the perceived position of a spatially extended flash stimulus is anisotropically shifted toward a single convergent point back along the trajectory of a moving object without a significant change in the perceived shape of the flash [Watanabe, K., & Yokoi, K. (2006). Object-based anisotropies in the flash-lag effect. Psychological Science, 17, 728-735]. In the previous experiment, the moving stimulus moved in both retinotopic and environmental coord

Biological organization of the extraocular muscles.

Extraocular muscle is fundamentally distinct from other skeletal muscles. Here, we review the biological organization of the extraocular muscles with the intent of understanding this novel muscle group in the context of oculomotor system function. The specific objectives of this review are threefold. The first objective is to understand the anatomic arrangement of the extraocular muscles and their compartmental or layered organization in the context of a new concept of orbital mechanics, the active pulley hypothesis. The second objective is to present an integrated view of the morphologic, cel

Inferior rectus muscle transection: a cause of diplopia after non-penetrating orbital trauma.

BACKGROUND: Vertical diplopia after blow out fracture repair is not uncommon; we report an unusual case of inferior rectus muscle (IR) transection presenting as a persistent infra-duction deficit after uncomplicated blow out fracture repair. METHODS: We used multi-positional MRI to diagnose a transected IR with a contracted and posteriorly displaced muscle belly. RESULTS: Infra-duction improved after surgical repair of the transected IR. CONCLUSION: Multipositional MRI is a novel technology that can be used to assist in the decisive management of persistent post-operative infra-duction defi

Anterior transposition of the inferior oblique muscle for a snapped inferior rectus muscle following functional endoscopic sinus surgery.

A 55-year-old woman was referred with a 4-month history of diplopia following functional endoscopic ethmoidectomy for chronic sinusitis. She had right hypertropia of 14 prism diopters in the primary position that increased to 30 prism diopters in down gaze. Her right eye showed a moderate limitation of motion in down gaze. Orbital imaging demonstrated a snapped right inferior rectus muscle. The inferior oblique muscle was transposed as an initial treatment for the snapped inferior rectus muscle. After surgery, the patient was orthophoric and obtained fusion in the primary position.

Quantitative evaluation of ocular motility in blow-out fractures for selection of nonsurgically managed cases.

PURPOSE: To clarify in which cases of blow-out fractures ocular motility will naturally recover. DESIGN: A retrospective review of observational cases. METHODS: Involved ocular motility ranges were measured by the Hess screen test at 15 or 30 degrees in 23 nonsurgical blow-out fracture patients without muscle entrapment or strong mechanical ocular restriction and in eight surgical patients with the above. RESULTS: In nonsurgical patients, the eyes moved more than 10 or 20 degrees respectively on a 15- or 30-degree Hess screen test in the early period, whereas in the surgical patients, the e

The time course of attentional focusing in dyslexic and normally reading children.

A cue size procedure was used to evaluate the time course of visuo-spatial attention in dyslexic and normally reading children. When a stimulus target is presented inside a large cue vs a small cue the identification time is slower. In the present study two cue-target delays (100 and 500 ms) were used. Results showed a slower time course of attentional focusing in dyslexics vs normal readers. Indeed, dyslexics exhibited no cue size effect at a shorter cue-target delay (100 ms), while it was present at a longer cue-target delay (500 ms). In contrast, a cue size effect was found at both cue-targ

Neuro-ophthalmologic manifestations of systemic disease and neurologic disease.

Neuro-ophthalmology is a difficult and challenging field for ophthalmologists, orthoptists, and neurologists. Unfortunately, signs and symptoms of these patients can be very subtle and easily overlooked. Although neuro-ophthalmologic cases are rare in the practice of a general ophthalmologist, missing the diagnosis can have severe impact on the patient's health. The authors review last year's publications of neuro-ophthalmologic manifestations of systemic and neurologic disease.

Fresnel prism treatment of sensory exotropia with restoration of sensory and motor fusion.

Anterior segment surgeons may treat patients with long-standing media opacities or uncorrected aphakia who have developed sensory strabismus. These patients are at risk for diplopia after surgery to clear the visual axis and restore emmetropia. This report describes 2 patients who regained comfortable single binocular vision without strabismus surgery. Sensory fusion was restored with Fresnel prisms, which were weaned and ultimately discarded as the patients' motor fusion was re-engaged after decades of disuse. Surgeons who restore vision in an eye with manifest sensory strabismus should be aw

Loss of vision alone may result in seesaw nystagmus.

A patient is described who developed seesaw nystagmus (SSN) associated with progressive severe vision loss due to cone-rod dystrophy. She was otherwise neurologically normal, and findings on magnetic resonance imaging of the brain were normal, including optic chiasm, meso-diencephalic junction, and brainstem. The literature is reviewed on neurologically normal patients with SSN and ocular vision loss, and the hypothesis is presented that SSN may become manifest as a result of vision loss alone, even in patients with chiasmal lesions, without disruption of central ocular motility pathways.

[Dizziness from a neuro-otological viewpoint].

The symptom of vertigo can be due to many different causes. Differential diagnosis will be discussed primarily from a neuro-otologic point of view. Vertigo can be thought of as a subjective disturbance of the integration of different sensory inputs. The history and subjective characterisation of vertigo often provide enough information for initial differential diagnosis and recommendation for a specific evaluation. The evaluation includes simple tests of posture and gait, tests of ocular motility, and examination of nystagmus. Instability and nystagmus towards a specific direction point to a v

Reflections on visual evoked cortical potentials and selective attention: methodological and historical.

Details of research in which Russ Harter participated in his formative years are reviewed. Harter's participation in the early work on Visual Selective Attention (Eason, Harter & White, 1969) probably contributed most importantly to his career. While reviewing Russ's early experience with basic vision research and pioneer visual evoked potentials (VEPs), there is reference to extensive information regarding stimulus effects on VEPs (exogenous components of the ERP). There can be complex early and late waveforms and waveform changes with changes in certain stimulus conditions. There are critica

[Exophthalmometry and thyroid disease. The value of the Hertel measurement evaluated by a group of patients with thyroid diseases and a control group].

Exophthalmometry values from routine referrals to a hospital eye clinic are analysed, including adult patients with and without thyroid disease. In thyroid disease without eye motility complaints (n = 90) the median exophthalmometry value was 18 mm (range 12-28 mm). Thyroid patients with eye muscle involvement (diplopia, n = 47) had significantly higher values (median 22 mm, range 15-31 mm) but did not differ significantly from a subgroup of 51 patients without thyroid disease, but with "big eyes" as a (presumably) physiological finding (median value 20 mm, range 16-25 mm). Early visual deprivation results in persistent strabismus and nystagmus in monkeys.

To understand to what extent visual-pattern deprivation during infancy results in strabismus and nystagmus, the authors examined the long-term consequences of this type of deprivation in monkeys during the first 50 days of life. Three cynomolgus and three rhesus monkeys had the eyelids sutured closed within 24 hr of birth. At 25 days of age, the eyelids were opened, and the eyelids of the fellow eye were sutured closed for an additional 25 days (reverse-eyelid suture). When the eyelids were opened at 50 days of age, each monkey was found to have 20-30 delta of exotropia and nystagmus, which pe
