Back pain is a top primary and urgent care complaint; radicular pain can be caused by herniation of the nucleus pulposus (intervertebral disc), spinal stenosis, or degenerative changes to the vertebrae. The focus of this clinical review will be the clinical approach and treatment of lumbar radicular pain, cervical radicular pain, and spinal stenosis. Usually localized through neurological history, exam, and imaging, specific signs and symptoms for lumbar radicular, spinal stenosis, and cervical radicular pain can help determine etiology. Once radicular back pain has been diagnosed, a multitude

An in-vitro experiment was designed to investigate the mode of failure following shear fatigue loading of lumbar motion-segments. Human male lumbar motion-segments (age 32-42 years, n=6) were immersed in Ringer solution at 37°C and repeatedly loaded, using a modified materials testing machine. Fatigue loading consisted of a sinusoidal shear load from 0 N to 1,500 N (750 N±750 N) applied to the upper vertebra of the motion-segment, at a frequency of 5 Hz. During fatigue experiments, several failure events were observed in the dynamic creep curves. Post-test x-ray, CT and dissection revealed tha

We report a case involving an 83-year-old man with interstitial lung disease who developed atypical pneumomediastinum caused by gas replacement of diminished fat. The patient presented with a complaint of worsening symptoms of respiratory difficulty since a diagnosis of chronic eosinophilic pneumonia 5 months back. He had been under observation with no particular treatment for 5 months. Computed tomography performed on admission revealed pneumomediastinum. When the current scan was compared with that obtained 5 months ago, it was evident that the fat surrounding the mediastinum had been replac

Back pain is common in athletes and a source of missed time from practice and competition. Pain generators include muscle (strain), ligament (myofascial sprain and strain), intervertebral disc (herniation and degeneration), nerve (radiculopathy), joint (facet and sacroiliac (SI) joint), and bones (pars interarticularis defect). The goal of treatment of an athlete with back pain is to relieve symptoms and facilitate safe but rapid return to play with no change in performance. Initial conservative treatment includes relative rest, anti-inflammatory medications, and physical therapy. The use of i

"Tug-of-war" may cause a variety of sports injuries, which has rarely been reported previously. This report described an uncommon case of a previously fit 64-year-old male who presented with abrupt onset of loss of consciousness after falling down in a game of massive tug of war including 1,500 participants as the rope snapped apart. Computed tomography (CT) scan of his abdomen revealed liver and spleen rupture. Spinal cord injury due to traumatic herniation of intervertebral disc at C5-6 level and bilateral brachial plexus injury were also noted after exploratory laparotomy with primary repai

Dialysis-related amyloid osteopathy (DRAO) is characterized by local osteoarticular lytic lesions, which sometimes cause a pathological fracture and reduce the quality of life in affected patients. In DRAO, active osteoclastic bone resorption is found at the bone surface facing the invaded synovial tissue and/or intervertebral disc, whereas reactive bone formation is absent. The eroded bone surface is covered with osteoclasts, suggesting the local promotion of osteoclastogenesis and osteoclast activation around DRAO. Inflammatory cells infiltrating the synovial tissue are likely to promote inf

Thirty-seven patients were studied for an average of 32.1 months after canal-expansive laminoplasty for the treatment of multiple-level cervical stenosis caused by spondylosis, ossification of posterior longitudinal ligament, prolapsed intervertebral disc, and other conditions. Short-term and medium-term results were recorded clinically, using the scoring system proposed by the Japanese Orthopedic Association. The canal expansion was also recorded with radiological studies. The improvement rate was good to excellent in 58.3% of the patients. Postoperative neurological deterioration occurred in

This study sought to visualize spinal-cord stimulation activity by infra-red thermography in humans suffering from chronic lumbosciatic pain. All the patients had previously undergone neurosurgery for a herniated intervertebral disc. Temperature changes were evaluated in two defined body areas after starting, stopping, maintaining or not starting the stimulation. In one body area, corresponding to the pain location, the patient experienced stimulation paraesthesia, whereas in the non-painful (second) area no stimulation paraesthesia were present. The patients were studied on four consecutive d

One hundred-ninety-six patients with spinal cord or nerve root compression were operated on by anterior microsurgical discectomy without graft between 1978 and 1987. The patients presented either with spinal cord compression (33%) or radicular symptoms with a duration of 12 days to more than then years (66%). Investigations to confirm these clinical syndromes employed myelography, CT or MRI. Two-hundred-eighty-eight levels were explored; 124 one level, 54 two level, 17 three level and 2 four level operations were done. The follow-up (mean 6.7 years) showed 66% of the patients with excellent or

Twenty-six patients with various brain tumors or carcinomatous meningitis were examined for alkaline phosphatase (ALP) in the cerebrospinal fluid. ALP enzyme levels were compared with the respective levels in control groups of 75 patients with epilepsy, stroke, bacterial and viral meningitis and intervertebral disc prolapse. Extremely high ALP levels in CSF (9516 mu/l, 1425 mu/l, and 871 mu/l) were found in patients with pulmonary carcinomatous meningitis. Among all other patients with brain tumors, ALP levels in CSF were in the normal range. Examination of ALP in serum yielded normal results

BACKGROUND: Lumbar degenerative disc disease (LDD) is a disabling condition which was associated with progressive degeneration of the intervertebral discs. The miR-766-3p and its modulation of SIRT6 remains unclear in LDD.
PURPOSE: This study aims to explored the regulated function of miR-766-3p and SIRT6 during LDD.
METHODS: Human nucleus pulposus cells (HNPCs) were treated with lipopolysaccharide (LPS) to construct a model of LDD to simulate inflammatory conditions. RT-qPCR and ELISA assays were employed to measure the expression of miR-766-3p and SIRT6. Additionally, we evaluated the leve

Purpose: The purpose of this study was to investigate dynamic responses of Lenke1B+ spines of adolescent scoliosis patients to different frequencies. Methods: Modal analysis, harmonic response analysis and transient dynamics of a full spine model inverted by the finite element method using Abaqus. Results: The first-order axial resonance frequency of 4.51 Hz produced a maximum axial displacement of 30.15 mm. Comparison of the five frequencies indicated that the 10 Hz frequency response curve was smoothest, while the amplitude-frequency curve at 4 Hz showed the greatest fluctuations accompanied

Intervertebral disc degeneration (IDD), osteoarthritis (OA), and osteoporosis (OP) are common musculoskeletal disorders (MSDs) with similar age-related risk factors, representing the leading causes of disability. However, successful therapeutic development and translation have been hampered by the lack of clinically-relevant animal models. In this study, we investigated the potential suitability of the tree shrew, a small mammal with a close genetic relationship to primates, as a new animal model for MSDs. Age-related spontaneous IDD in parallel with a gradual disappearance of notochordal cell

INTRODUCTION: Discal cysts have been reported as intraspinal extradural cysts connected with corresponding intervertebral discs. The resection of the discal cyst has illustrated the good result. However, we report a much larger discal cyst, which was seemed to be difficult to resect all of them under the microendoscope safety.
CASE REPORT: A 21-year-old man had experienced pain in the right lower limb for at least a month and was diagnosed with radiculopathy of the right S1 nerve root. Magnetic resonance image showed that mild disc degeneration and cyst at L5/S1, which was measured 12.0 mm on

INTRODUCTION: The risk factors for the development of progressive segment degeneration (PSD) after decompression surgery are still unknown. In this study, the risk factors for PSD in patients who undergo decompression surgery for lumbar spinal stenosis with and without coexisting spondylolisthesis and scoliosis were examined, focusing on decompression and non-decompression segments.
METHODS: We reviewed the data of patients with >5 years of postoperative follow up. Radiographic PSD was defined as either the development of an anterolisthesis or retrolisthesis of >3 mm or a decrease in disc hei

BACKGROUND: Chronic low back pain secondary to degenerative changes in the spine is a common cause of disability, and disc degeneration is one of the most frequent imaging findings. Intervertebral vacuum phenomenon (IVP) is usually observed in advanced degeneration. Recently, this phenomenon has gained interest due to a relatively new surgical technique called percutaneous discoplasty, aimed at treating low back pain secondary to degenerative disc disease in elderly patients.
PURPOSE: To analyze the prevalence and related factors of the vacuum phenomenon in adult patients.

Epidemiological and clinical studies show that the magnitude and scope of cervical disease are on the rise, along with the world's rising aging population. From a biomechanical perspective, the cervical spine presents a wide inter-individual variability, where its motion patterns and load sharing strongly depend on the anatomy. This study aimed to first develop and validate a geometrically patient-specific model of the lower cervical spine for clinical applications, and secondly to use the model to investigate the spinal biomechanics associated with typical cervical disorders. Based on measure
