Preoperative lower esophageal sphincter pressure affects outcome of laparoscopic esophageal myotomy for achalasia.

The primary aim of this study was to identify factors that influence outcome of the surgical treatment of achalasia. A secondary aim was to compare outcomes after laparoscopic Heller myotomy and partial fundoplication using either a Dor or Toupet hemifundoplication. Between 1994 and 2002, a total of 78 patients underwent laparoscopic Heller myotomy and partial fundoplication. Preoperative investigations included esophageal manometry, a videoesophogram, and upper gastrointestinal endoscopy with biopsy. In 64 patients (35 males and 29 females), telephone contact was possible at a median 24 month

[Nineteen years experience with posterior sagittal anorectoplasty as a treatment of anorectal malformation].

UNLABELLED: Posterior sagittal anorectoplasty (PSARP) is considered today the best surgical technique for the treatment of the anorectal malformations. With the aim of evaluating the results of the PSARP in our patients, the charts of 39 children (27 male, 12 female) were reviewed. Mean age was 11 years (3 to 20 years) and the mean follow-up period from the closure of the colostomy was 9.4 years (1 to 18 years). Patients with any alteration of the extrinsic innervation or those with rare malformations were excluded of the study. According to the Wingspread classification, 14 patients had a hig

Influence of the L-arginine-nitric oxide pathway on vasoactive intestinal polypeptide release and motility in the rat stomach in vitro.

Endogenous nitric oxide (NO) plays an important role as non-adrenergic, non-cholinergic inhibitory transmitter in the gastrointestinal tract, especially in sphincter regions. The aim of this study was to investigate the influence of NO on pyloric motility and on the release of vasoactive intestinal polypeptide (VIP) in the isolated perfused rat stomach in vitro. Therefore, pyloric motility was continuously recorded by a special sleeve manometry catheter placed in the pyloric region and the concentration of VIP was determined in the venous effluent of the portal vein. Arterial perfusion with th

Patterns of gastroesophageal reflux in health and disease.

Twenty-four pH monitoring the distal esophagus quantitates gastroesophageal reflux in a near physiologic setting by measuring the frequency and duration of acid exposure to the esophageal mucosa. Fifteen asymptomatic volunteers were studies with 24-hour pH and esophageal manometry. The normal cardia was more competent supine than in the upright position. Physiologic reflux was unaffected by age, rarely occurred during slumber, and was the rule after alimentation. One hundred symptomatic pateitns with an abnormal 24-hour pH record (2 S.D. above the mean of controls) could be divided into three

Supragastric belching: Evaluating the efficacy of short-term Ryle's tube placement and diaphragmatic breathing technique.

BACKGROUND: Supragastric belching (SGB) is a behavioral disorder involving rapid air influx into the esophagus through a transiently relaxed upper esophageal sphincter, followed by immediate expulsion. Unlike gastric belching, it is not related to lower esophageal sphincter (LES) dysfunction or gastroesophageal reflux disease (GERD). Often associated with psychological stress, SGB can significantly impair quality of life and some patients remain refractory to conventional therapies. METHODS: In this retrospective study, clinical records of patients diagnosed with refractory supragastric belch

The impact of stent length on stent-adjacent stenosis following transverse sinus stenting in idiopathic intracranial hypertension.

OBJECTIVE: Cerebral venous sinus stenting (VSS) is an established treatment for transverse sinus (TS) stenosis associated with idiopathic intracranial hypertension (IIH). The authors' early practice favored shorter stents that only spanned the focal narrowing to reduce the metal burden. However, stent-adjacent stenosis (SAS) emerged as a significant failure mode prompting the use of longer constructs. In this study, the authors sought to compare the incidence of SAS, clinical outcomes, and angiographic findings between shorter and full-length stent constructs. METHODS: The authors conducted a

IR-780 improves urination function and complications in rats with partial bladder outlet obstruction by protecting bladder smooth muscle cell mitochondria from oxidative stress.

INTRODUCTION: Partial bladder outlet obstruction (pBOO) is the most common cause of lower urinary tract symptoms (LUTS). Prolonged BOO induces bladder remodeling, which can lead to severe bladder dysfunction and refractory LUTS in some patients, even after obstruction resolution. This condition significantly impairs patients' quality of life, and no effective treatment is currently available. This study investigated a pBOO rat model using IR-780, a novel near-infrared lipophilic dye with potential targeted antioxidant effects. METHODS: A partial ligation of the rat bladder neck was performed

Early outcomes of transverse perineal support in obstructed defecation syndrome with pathological perineal descent; a prospective cohort study.

BACKGROUND: While current surgical approaches for obstructed defecation syndrome (ODS) primarily target rectocele and rectal intussusception, excessive perineal descent (PD) may contribute to persistent symptoms and suboptimal surgical outcomes. This study aimed to evaluate early outcomes of transverse perineal support (TPS), a procedure designed to reinforce the perineum, in patients with ODS and pathological PD. PATIENTS AND METHOD: All consecutive patients with ODS and pathological PD were assessed for eligibility. After at least one year of conservative management (dietary fiber, biofeedb

Life-threatening lower gastrointestinal bleeding after routine anorectal procedures: a case series and management implications.

BACKGROUND: This case series characterizes enema- and catheter-induced rectal injuries as an underrecognized iatrogenic cause of life-threatening lower gastrointestinal bleeding (LGIB). By focusing on elderly, high-risk patients, we highlight the diagnostic complexities and management strategies associated with these routine yet potentially catastrophic anorectal procedures. METHODS: We analyzed three cases of high-risk elderly patients who experienced acute, massive hemorrhage following routine anorectal procedures, specifically glycerin enemas and anorectal manometry. To contextualize these

Peroral endoscopic myotomy for achalasia and patients with normal lower-esophageal-sphincter integrated relaxation pressure: A propensity-score-matched retrospective study.

BACKGROUND: Most patients who were included in previous studies on achalasia had increased lower esophageal sphincter (LES) pressure. Peroral endoscopic myotomy (POEM) has been confirmed to be effective at relieving the clinical symptoms of achalasia associated with increased LES pressure. AIM: To identify the safety and efficacy of POEM for patients with normal LES integrated relaxation pressure (LES-IRP). METHODS: The clinical data of patients who underwent POEM successfully in The First Medical Center of Chinese PLA General Hospital were retrospectively analyzed. A total of 481 patients w

Non-invasive versus arterial pressure monitoring in the pre-hospital critical care environment: a paired comparison of concurrently recorded measurements.

BACKGROUND: Blood pressure monitoring is important in the pre-hospital management of critically ill patients. Non-invasive blood pressure (NIBP) measurements are commonly used but the accuracy of standard oscillometric cuff devices may be affected by extremes of physiology and adverse conditions (e.g. vibration) during transport. This study aimed to quantify the accuracy of NIBP measurements amongst patients requiring pre-hospital critical care. METHODS: A retrospective cohort study was undertaken using data from patients treated by a pre-hospital critical team between 1st May 2020 and 30th A

Effects of an asymmetrical high flow nasal cannula interface in hypoxemic patients.

BACKGROUND: Optimal noninvasive respiratory support for patients with hypoxemic respiratory failure should minimize work of breathing without increasing the transpulmonary pressure. Recently, an asymmetrical high flow nasal cannula (HFNC) interface (Duet, Fisher & Paykel Healthcare Ltd), in which the caliber of each nasal prong is different, was approved for clinical use. This system might reduce work of breathing by lowering minute ventilation and improving respiratory mechanics. METHODS: We enrolled 10 patients ≥ 18 years of age who were admitted to the Ospedale Maggiore Policlinico ICU in

Peroral endoscopic myotomy is equally safe and highly effective treatment option in achalasia patients with both lower and higher ASA classification status.

INTRODUCTION: The American Society of Anesthesiologists (ASA) physical status classification system was developed as a simple categorization of patients' physiological status that predicts the operative risk. Peroral endoscopic myotomy (POEM) is a less invasive alternative to surgical myotomy in achalasia. As such, POEM seems to be an appealing option for high-risk patients with achalasia. However, there are no studies which systematically analyzed the outcomes of POEM among patients with different ASA classes. Hence, we aimed to compare the safety and efficacy of POEM in patients with lower a

Endoscopic ultrasound: a powerful tool to modify treatment algorithms in opioid-induced achalasia.

BACKGROUND: Opioid use in the U.S. has increased dramatically over the last 15 years, recently being declared a public health emergency. Opioid use is associated with esophageal dysmotility lending to a confusing clinical picture compared to true achalasia. Patients exhibit symptoms and elicit diagnostic results consistent with esophageal motility disorders, in particular type III achalasia. Modified therapeutic strategies and outcomes become challenging. Differentiating true achalasia from opioid-induced achalasia is critical. Conventional surgical interventions, i.e., myotomy, are ineffectiv

Effect of positive end-expiratory pressure on gastric insufflation during induction of anaesthesia when using pressure-controlled ventilation via a face mask: A randomised controlled trial.

BACKGROUND: Face mask ventilation (FMV) during induction of anaesthesia is associated with risk of gastric insufflation that may lead to gastric regurgitation and pulmonary aspiration. A continuous positive airway pressure (CPAP) has been shown to reduce gastric regurgitation. We therefore hypothesised that CPAP followed by FMV with positive end-expiratory pressure (PEEP) during induction of anaesthesia would reduce the risk of gastric insufflation. OBJECTIVE: The primary aim was to compare the incidence of gastric insufflation during FMV with a fixed PEEP level or zero PEEP (ZEEP) after anae
