Onychomycosis is a fungal disease of the nail that is found worldwide and is difficult to diagnose accurately. This study used metagenomics to investigate the microbiology of 18 clinically diagnosed mycotic nails and two normal nails for fungi and bacteria using the ITS2 and 16S loci. Four mycotic nails were from Bass Coast, six from Melbourne Metropolitan and eight from Shepparton, Victoria, Australia. The mycotic nails were photographed and metagenomically analysed. The ITS2 sequences for T. rubrum and T. interdigitale/mentagrophytes averaged over 90% of hits in 14/18 nails. The high abundan

Background: Posterior sagging (PS) of the proximal fragment after cephalomedullary nailing (CMN) of intertrochanteric fractures leads to loss of anteromedial cortical support. Methods: We retrospectively reviewed 118 patients with AO/OTA 31-A2 intertrochanteric fractures (2007 classification). After an institutional protocol change, standard-length constructs with one distal screw were used until March 2019 (n = 65) and long constructs (≥300 mm) with two distal screws from June 2019 (n = 53); the cohorts were consecutive, not concurrent. Results: PS occurred in 23/118 patients (19.5%): 21/65 (

INTRODUCTION: Pediatric femoral shaft fractures are one of the commonest injuries in early childhood and school-going children, with operative management increasing preferred in the age group of 6-13 years. The titanium elastic nailing system (TENS) is the most preferred modality in this age group, providing stable fixation and biological preservation; however, its effect on mechanical axis alignment and early functional outcome needs evaluation. In this study, we aim to analyze the effect of femur shaft fractures being managed by TENS on the mechanical axis of the lower limb and its effect on

PURPOSE: To determine whether cephalomedullary nail (CMN) angle was associated with malreduction after intertrochanteric (IT) fracture fixation. Secondarily, to explore if CMN angle was associated with fixation failure (FF).
METHODS: A retrospective comparative review was conducted of patients ≥ 65 years old with low energy falls and OTA/AO 31-A fractures treated with a CMN from 12/01/2016-06/30/2024 at a single academic institution (four hospitals). The primary outcome was immediate postoperative malreduction (IPM)-the absolute difference between post-fixation neck-shaft angle (NSA) and nati

BackgroundDespite advances in motorized internal lengthening nails, the lengthening over an intramedullary nail (LON) technique remains relevant in selected clinical scenarios. In femoral LON, both antegrade and retrograde nailing directions may be considered according to osteotomy level, deformity characteristics, and planned lengthening. Although antegrade entry is more commonly preferred, retrograde entry may offer technical advantages in selected cases. This study compared outcomes and complications between antegrade and retrograde femoral LON in a single-surgeon cohort.MethodsA retrospect

BACKGROUND: From a biomechanical perspective, intramedullary fixation offers potential benefits, including a central load-sharing axis that may better withstand axial and bending forces, while preserving the periosteal blood supply and reducing soft tissue damage. This review aimed to determine whether intramedullary nailing in displaced intra-articular fractures provides comparable or superior biomechanical stability to plate fixation.
METHODS: A literature search identified studies comparing the biomechanical stability of intramedullary fixation with plate fixation, screw-only fixation, or

Nail psoriasis is a chronic inflammatory manifestation frequently associated with cutaneous psoriasis and psoriatic arthritis (PsA). Fingernails and/or toenails may be affected by psoriasis. The management of nail psoriasis can be challenging due to marked variability in clinical presentation, its chronicity, its frequent refractory nature despite a variety of therapies, and the slow rate of nail plate growth over several months. Unfortunately, the common clinical misdiagnosis as onychomycosis often leads to delay in an accurate diagnosis of nail psoriasis as well as exposure of the patient to

BACKGROUND: Cephalomedullary nail (CMN) fixation is commonly used to treat extracapsular femoral fractures but is associated with mechanical complications, including implant cut-out, cut-in, and material failure. Identifying clinical and radiographic predictors of these complications is critical for improving patient outcomes. METHODS: This retrospective cohort study analyzed 401 patients treated with CMN for per-, sub-, or intertrochanteric fractures between 2019 and 2024. Clinical, laboratory, and radiographic parameters were evaluated using logistic regression analyses to identify predictor

Bowen disease (BD), or squamous cell carcinoma (SCC) in situ, represents a histologically defined but biologically heterogeneous group of intraepithelial neoplasms arising across different epithelial compartments. Human papillomavirus (HPV) plays a well-established causal role in anogenital squamous intraepithelial neoplasia, whereas its contribution to extragenital BD, including nail apparatus and general cutaneous lesions, has remained controversial. We performed a narrative review of the literature to synthesize current evidence on HPV prevalence, genotype distribution, and pathogenetic rel

BACKGROUND: Varus malalignment is a risk factor for nonunion and mechanical complications in subtrochanteric femoral fractures (SFFs). Although various reduction techniques have been reported to avoid varus malalignment in SFFs, achieving anatomic reduction remains challenging, often resulting in residual varus alignment (RVA) after reduction. This study aimed to investigate the radiographic and clinical outcomes of a novel method resolving RVA by abducting the ipsilateral hip after cephalomedullary fixation with an intramedullary nail and subsequently inserting distal interlocking screws.
