Aromatase inhibitors for the treatment of infertility.

Ovarian stimulation during infertility treatment is used either alone or in conjunction with intrauterine insemination and assisted reproductive technologies. At the present time, the two main medications used for ovarian stimulation include an oral antioestrogen, clomiphene citrate and injectable gonadotrophins. In spite of the high ovulation rate, the use of clomiphene citrate is associated with adverse side effects and low pregnancy rates. In clomiphene citrate failures, gonadotrophin injections are generally the next treatment option but, especially in polycystic ovarian syndrome, are asso

Review of the Registration and Establishment of a Database for Pregnant Mothers With Cardiovascular Diseases.

BACKGROUND AND AIMS: Cardiovascular diseases (CVDs) cause most maternal deaths, mainly from cardiomyopathies, pulmonary hypertension, aortic dissections, and MIs. We examined CVD prevalence in pregnancy, its links to risk factors/demographics, and identified high-risk cases for prevention programs. METHODS: This descriptive study was conducted on 104 pregnant patients with heart conditions referred to Shahid Sayyad Shirazi Educational and Medical Center. The data and information for this project were obtained by reviewing the medical records of pregnant patients with heart conditions and reco

Association of co-exposure to EDCs in early pregnancy with threatened abortion: The mediation effect of progesterone.

Endocrine-disrupting chemicals (EDCs) are ubiquitous environmental pollutants linked to adverse pregnancy outcomes, but their combined effects on threatened abortion (TA) remain unclear. Progesterone (P4) plays a critical role in maintaining pregnancy, yet its potential modifying effect on EDC-associated TA is underexplored. This study aimed to investigate the association between patterns of EDC co-exposure and TA, and to evaluate the potential mediating effect of P4. A prospective cohort study was conducted among 1,122 pregnant women in China. Urine concentrations of 24 EDCs (including bisphe

The Importance of Inflammatory and Angiogenic Markers in the Evaluation of Early Cardiovascular Disease Risk in Women with Hypertensive Disorders of Pregnancy.

BACKGROUND: Women with hypertensive disorders of pregnancy (HDP) have a significantly higher risk of developing cardiovascular diseases later in life. The stratification of this risk using biomarkers during pregnancy can help to identify these women and apply early prevention. OBJECTIVE: We aimed to determine proinflammatory cytokines and angiogenic markers, echocardiographic parameter changes after delivery and predict early cardiovascular disease risk in women with arterial hypertension and its complications during pregnancy. METHODS: We conducted a literature search using the PubMed datab

Urinary and anal incontinence in pregnancy in a Nigerian population: A prospective longitudinal study.

OBJECTIVES: There are several studies from sub-Saharan Africa on postpartum urinary incontinence and anal incontinence, but very rare in pregnancy. Such data will guide obstetric caregivers in providing appropriate counseling to the women as well as in minimizing the risk factors. This study aimed to determine the comparative effects of different trimesters of pregnancy on urinary incontinence and anal incontinence, and their possible risk factors. METHODS: The study was longitudinal in design, and the study population consisted of 223 pregnant women receiving care at the two largest tertiary

Planning for Outcomes (P4O) Modeling Tool: Estimating the Impact of Changing the Proportion of Injectable Progestins in the Contraceptive Method Mix.

BACKGROUND: Observational studies raise concern about a potential link between injectable progestin contraceptive use and HIV acquisition risk. This possible link is particularly relevant in sub-Saharan Africa where HIV risk is high and the method mix is skewed toward injectables. We developed the Planning for Outcomes (P4O) model (https://planning4outcomes.ctiexchange.org/) to predict changes in maternal and child health (MCH) and HIV outcomes that could occur if the proportion of injectables in the method mix is changed. METHODS: P4O incorporates evidence-based assumptions to predict yearly

Effectiveness of cold coagulation in treating high-grade cervical intraepithelial neoplasia: the human papillomavirus evidence of cure.

We retrospectively analysed post-treatment human papillomavirus (HPV) results in women treated for cervical intraepithelial neoplasia (CIN) to establish if 'virological cure' rates achieved by cold coagulation matched those for large-loop excision of the transformation zone (LLETZ) in the treatment of CIN. The main outcome measure was the rate of non-detection of HPV at 6- and 18-month follow-up. HPV was not detected in 82.0% and 86.0% of women at 6- and 18-month follow-up, respectively. In women with high-grade CIN (CIN 2 or worse), there was no difference in HPV non-detection rates among the

Placental Chronic Histiocytic Intervillositis: Pregnancy Outcomes and Complications, a Systematic Review and Meta-Analysis.

Chronic histiocytic intervillositis (CHI) is a rare placental inflammatory condition characterized by the infiltration of monocytes into the intervillous space. CHI is associated with serious neonatal complications. In the present systematic review and meta-analysis, we assessed the relationship between CHI and maternal and neonatal outcomes. The meta-analysis involved searching databases such as EMBASE, PubMed, Web of Science, and SCOPUS for studies on idiopathic CHI up to May 1, 2024. Included studies assessed pregnancy outcomes, including gestational age and live birth status, while excludi

Hypertensive Disorders of Pregnancy in Saudi Arabia: Evaluating Maternal and Neonatal Risks, Outcomes, and Aspirin Prophylaxis: A Review Article.

Hypertensive disorders during pregnancy, including pre-eclampsia and eclampsia, pose significant risks to both maternal and neonatal health. This review article evaluates the prevalence, maternal and neonatal outcomes, and the efficacy of aspirin prophylaxis in managing these conditions in Saudi Arabia. Utilizing data from multiple retrospective studies and recent guidelines, we highlight the regional variations in the outcomes of hypertensive disorders of pregnancy. Severe complications such as Hemolysis, Elevated Liver enzymes and Low Platelets (HELLP) syndrome occurred in 6.6% of cases, whi

Machine Learning for Fetal Growth Prediction.

Birthweight is often used as a proxy for fetal weight. Problems with this practice have recently been brought to light. We explore whether data available at birth can be used to predict estimated fetal weight using linear and quantile regression, random forests, Bayesian additive regression trees, and generalized boosted models. We train and validate each approach using 18,517 pregnancies (31,948 ultrasound visits) from the Magee-Womens Obstetric Maternal and Infant data and 240 pregnancies in a separate dataset of high-risk pregnancies. We also quantify the relation between smoking and small-

Gestational choriocarcinoma after term pregnancy: a case report.

Choriocarcinoma coexisting with or after a "normal" pregnancy has an incidence of one per 160 000 pregnancies. In case of choriocarcinoma after term pregnancy, early diagnosis by histopathological examination of the placenta is very important, the precocity of the diagnosis influencing the prognosis and tumor response to chemotherapy. In this paper, we report the case of a 29-year-old woman gravidity 2, parity 2, with metastatic choriocarcinoma after term pregnancy, diagnosed at four months after the delivery of a healthy baby. An episode of abundant vaginal bleeding occurred after four months

[Syphilis and pregnancy].

Syphilis is a bacterial infection caused by the species Treponema pallidum subsp. pallidum, transmitted both sexually and vertically. The disease progresses in three distinct stages, with a high risk of contagion in the early stages. The infection has seen a resurgence in several countries around the world and is particularly concerning during pregnancy.Indeed, from the 16th week of gestation, the treponema can cross the placenta and infect the fetus and it can cause placental and/or fetal lesions. Maternal-fetal transmission is more common in the early stages of the disease. The consequences

Complications in pregnant women with sickle cell disease.

Pregnancy in women with sickle cell disease (SCD) is associated with increased maternal and fetal morbidity and mortality. Outcomes vary widely owing to methodological limitations of clinical studies, but overall, hypertensive disorders of pregnancy, venothromboembolism, poor fetal growth, and maternal and perinatal mortality are increased globally. Few therapeutic interventions have been explored other than prophylactic and selective transfusion therapy. Unfortunately, existing data are limited, and it remains unclear whether prophylactic use of chronic transfusions will improve pregnancy out

Effect of Adherence to the Mediterranean Diet on Maternal Iron Related Biochemical Parameters during Pregnancy and Gestational Weight Gain.

Gestation is a crucial life stage for both women and offspring, and outcomes are affected by many environmental factors, including diet. The Mediterranean dietary pattern (MD) is considered a healthy eating pattern that can provide the nutritional requirements of pregnancy. Meanwhile, iron deficiency anemia is one of the most frequent complications related to pregnancy. This study aimed to evaluate how the level of adherence to the MD influences maternal gestational weight gain and specific iron-related maternal biochemical parameters during the pregnancy. Accordingly, an observational, popula
