Clinical features of central sensitization moderate the effect of repetitive transcranial magnetic stimulation in chronic low back pain.

This secondary analysis of a randomized controlled trial investigated whether baseline clinical characteristics moderate the efficacy of repetitive transcranial magnetic stimulation (rTMS) and motor control exercise (MCE) for chronic low back pain (CLBP). One hundred and forty participants were randomized to active or sham rTMS treatment, combined with or without MCE. Six potential effect modifiers (sex, kinesiophobia, self-efficacy, central sensitization features, lumbar instability, and expectation) were assessed at baseline. Linear mixed models tested interaction effects on pain and disabil

Association between posterior thigh tightness and lifetime low back pain in young basketball players: a cross-sectional study.

BACKGROUND: Low back pain (LBP) is common in young athletes and may restrict participation in basketball. Evidence regarding the association between lower extremity tightness and LBP in adolescent basketball players remains limited. This study aimed to examine whether anterior (quadriceps) and posterior (hamstring) thigh tightness were associated with LBP in elementary and middle school players. METHODS: In this cross-sectional study conducted in Fukushima, Japan (2013-2014), players completed questionnaires on the presence or absence of lifetime and point prevalence of LBP. Physiotherapists

Drivers of unnecessary diagnostic imaging for uncomplicated low back pain among family physicians: a theory-informed qualitative study.

BACKGROUND: Most major society guidelines recommend against diagnostic imaging for uncomplicated low back pain (LBP), referring to LBP that is not accompanied by symptoms that suggest a serious diagnosis, as it does not benefit patients. Despite this, diagnostic imaging for uncomplicated LBP remains common and contributes to overdiagnosis, overtreatment, and increased healthcare costs. AIM: The purpose of this qualitative study was to use a framework to rigorously understand the factors influencing family physicians' decisions to order diagnostic imaging for uncomplicated LBP in our context.

Vertebral endplate bone marrow lesion composition: relation to Modic change classification and associations with chronic low back pain.

BACKGROUND CONTEXT: Vertebral endplate bone marrow lesions ("Modic changes," MC) are a putative cause of chronic low back pain in some individuals. However, associations between MC subtypes and clinical symptoms are variable, which limits the utility of conventional imaging in diagnosing and treating chronic low back pain patients with MC. PURPOSE: Develop a quantitative imaging biomarker for measuring compositional abnormalities in MC-affected endplates using water-fat MRI and test for associations with patient-reported outcomes, conventional MC type classification, and longitudinal conversi

Lumbar Stabilization and Gluteal Strengthening Exercises Versus Health Education for Chronic Low Back Pain: A Retrospective Observational Study.

Background and aim Chronic low back pain represents a substantial global health burden with escalating prevalence rates and significant disability implications. Contemporary clinical practice guidelines emphasize exercise-based interventions as first-line conservative management strategies. However, optimal exercise prescription protocols remain under investigation. This study aimed to compare functional disability outcomes between patients receiving combined gluteus muscle strengthening with lumbar stabilization exercises versus those receiving spine-neutral health education alone for chronic

Comparing Movement Patterns and Physical Function Between Chronic Low Back Pain Patients With Nociplastic and Nociceptive Pain Categories.

BACKGROUND: Individuals with chronic low back pain (LBP) often present with significant physical dysfunction. The underlying cause is difficult to diagnose due to the heterogeneous nature of LBP categories. METHODS: Two hundred and fifty-six patients were assessed as having either nociceptive (NC) or nociplastic (NP) chronic low back pain using validated surveys, including the PainDETECT Questionnaire and a chronic overlapping pain condition screener. Additional covariates of anxiety, depression, and fear avoidance were evaluated using standard surveys. Physical function was judged objectivel

Occlusal byte and Mézières postural therapy improve pain and performance in elite rhythmic gymnasts with low back pain.

BACKGROUND: Low back pain (LBP) is prevalent among rhythmic gymnastics athletes due to repetitive spinal loading and postural asymmetries. The Mézières Method, a postural re-education approach, aims to restore global muscle balance, while occlusal splints (oral bytes) have been proposed to optimize posture and neuromuscular control. This study investigated the combined and independent effects of postural therapy and oral byte use on pain, function, flexibility, and quality of life in elite rhythmic gymnasts with LBP. METHODS: Eighty-nine gymnasts were screened, and 39 met inclusion criteria a

Acute Modulation of Lumbar Motor Unit Behavior During Experimentally Induced Low Back Pain: A Motor Unit Decomposition Analysis.

BACKGROUND AND PURPOSE: Acute low back pain is a prevalent musculoskeletal disorder that can disrupt motor control and compromise functional stability. Evidence suggests that pain alters motor unit discharge characteristics and the temporal organization of muscle activation; however, the specific neuromuscular adaptations associated with acute low back pain remain insufficiently understood. This study aimed to investigate the effects of experimentally induced acute low back pain on force steadiness and motor unit discharge characteristics (including recruitment threshold, motor unit firing rat

Chronic Back Pain Increases Alzheimer's Disease Risk Independent of Gross Brain Structural Alterations: Genetic Evidence from Two Populations.

BACKGROUND: Observational studies suggest chronic musculoskeletal pain (CMP) may be a risk factor for dementia, but confounding factors or reverse causation could obscure these findings. Additionally, whether brain structure imaging measurements mediate this relationship remains unclear. We aimed to investigate their causal association using Mendelian randomization (MR). METHODS: Genetic instruments derived from genome-wide association studies were employed in a two-sample MR framework. Forward MR analysis examined the genetic effect of CMP on dementia, while reverse MR analysis evaluated the

Comparative Efficacy of Physical Agent Therapies for Chronic Low Back Pain-A Systematic Review and Network Meta-Analysis.

BACKGROUND: Chronic low back pain (CLBP) involves peripheral nociception, central sensitization, and persistent disability. Transcranial magnetic stimulation (TMS) may reduce pain by modulating cortical excitability and central pain processing, while electroacupuncture (EA) combines acupuncture with electrical stimulation to influence nociceptive pathways and improve function. However, the relative efficacy of these and other physical agent therapies remains clinically uncertain in CLBP management. OBJECTIVE: To systematically compare the efficacy of different PATs in pain relief and physical

Changes in pharmacological management of low back pain: Real-world evidence from a 4-year comparison (2021-2025).

OBJECTIVE: Contemporary guidelines recommend conservative, non-pharmacological management as first-line treatment for low back pain (LBP), yet real-world implementation remains variable. Understanding temporal trends in prescribing patterns is essential for evaluating alignment with evidence-based recommendations. The purpose of this study was to evaluate changes in pharmacological and non-pharmacological treatment strategies for LBP in a tertiary physical medicine and rehabilitation (PMR) outpatient clinic by comparing 2 time periods 4 years apart. METHODS: This retrospective observational s

Machine learning models for predicting treatment outcomes in chronic non-specific back pain patients undergoing lumbar extension traction.

Conservative treatment for chronic non-specific low back pain (CLBP) includes lumbar extension traction (LET) to re-align lumbar lordosis (LL). This study explores the use of machine learning (ML) models to predict post-treatment outcomes in patients with CLBP undergoing LET and how these predictions can support clinical decision-making. We utilized a retrospective database of 431 consecutive patients with uncomplicated CLBP. Post-treatment variables predicted included LL, NRS pain score, and Oswestry Disability Index (ODI). Input model variables included pre-treatment LL, sacral base angle (S
