Splenorenal arterial bypass in a child with Takayasu's disease: a case report.

In adults splenorenal arterial bypass is a highly effective treatment for renovascular hypertension, but in children the procedure has been less successful because of the small size of the splenic artery. However, with the improvement in microvascular techniques the procedure is now possible in children. A 2-year-old child with Takayasu's arteritis, previous right nephrectomy and severe renovascular hypertension required revascularization to salvage his remaining left kidney. A splenorenal arterial bypass was performed through a left retroperitoneal flank incision. Postoperatively his creatini

Reconstruction of the urethral meatus with a buccal mucosa graft.

A buccal mucosa graft from the lower lip was used for meatal reconstruction in 3 patients. The buccal mucosa graft was combined with a bladder mucosa graft for urethral substitution in 1 adult and 1 child, and was anastomosed to a skin tube for creating a glanular urethra in 1 child. The buccal mucosa graft obtained was approximately twice as wide as the desired urethra. Postoperatively the buccal mucosa graft became shrunken at the proximal anastomosis and urethral stricture developed in 2 children. The graft did not shrink at the meatus, but the meatus became widened in 2 patients. These res

Cultural lag, economic scarcity, and the technological quagmire of "Infant Doe".

The major purpose of this article is to explore the substantial disparity between access to resources provided at birth to a child with spina bifida--a so-called Baby Doe--and later access to resources needed to sustain that child throughout the life course. The author's organizing principle is the allocative paradoxes created by the clash of technology, scarcity, and demography. The study is based on an eight-year inquiry into the way life-saving medical decisions are made about children with spina bifida. This work focuses on the qualitative aspects of that inquiry. There is a societal need

[Diphtheria in an infant].

A seven week old infant developed pharyngeal diphtheria. The child quickly deteriorated with severe dyspnea and bradycardia. These symptoms made endotracheal intubation necessary. The pharynx and the larynx were covered with pseudomembranes. At the fourth day of hospital stay antitoxin has been administrated. At first the condition of the child became much better. Then progredient paralysis of the diaphragm occurred and led again to intubation. The neurologic complications resolved completely. Diphtheria--above all in an infant--is a rare disease. Our report shows, that it has to be kept in mi

Transmission of 'toxic strep' syndrome from an infected child to a firefighter during CPR.

Several cases of a toxic shocklike syndrome have been reported in the United States during the past five years in association with Streptococcus pyogenes infection. We report a case of a firefighter exposed during attempted CPR to the secretions of an S pyogenes-infected child. The firefighter developed an infection of the hand and subsequent febrile illness with hypotension, erythematous rash, renal failure, and hypocalcemia. Bacterial isolates of blood and cerebrospinal fluid from the deceased child were identical in type and exotoxin production with isolates grown from the hand wound of the

Two Japanese cases of battered child syndrome with retinal hemorrhage.

We present 2 cases of battered child syndrome with retinal hemorrhage. Case 1 is a 6-year-old boy who was blinded in both eyes following abuse by his mother's common-law husband. Retinal hemorrhages and optic atrophy were found in both eyes. Case 2 is a 6-month-old boy who developed dyspnea following episodes of abuse by his mother's common-law husband. Retinal hemorrhages were visible in both fundi. We believe that retinal hemorrhage is the most frequent clinical finding in abused children and its presence should alert the ophthalmologist to suspect battered child syndrome.

Case report: chronic graphite granulomatous abscess simulating a brain tumor.

Brain abscess can develop at the site of retained intracranial foreign bodies many years after the injury. This report describes a 6 1/2-year-old child who presented with recurrent severe headaches and a focal seizure of the right upper extremity. Skull X-ray was normal. Computed tomography of the head showed a mass in the left temporoparietal region with a central area of high density and contrast enhancement, thought to be a neoplasm. The mass was seen on the radionuclide brain scan and was avascular on angiography. Craniotomy revealed a foreign body granuloma with a small abscess cavity and

Feasibility and Accuracy of Caregiver-Led Child Growth Measurements for Remote Healthcare.

BACKGROUND AND AIMS: Remote healthcare is expanding, with accurate growth data essential for nutritional care. Guidance for routine caregiver-led measurements is limited and largely untested. This study evaluated the feasibility and accuracy of caregiver-led child growth measurements at home, compared to 'gold-standard' measurements in the hospital. METHODS: Caregivers of children attending outpatient appointments completed a survey on their access to weighing scales and tape measures at home. Those with suitable equipment were consented to participate in a home measurement study. Using provi

ManyNumbers 3: A Multi-Lab Study of Demographic Correlates of Early Number Knowledge.

Large scale studies have documented socioeconomic (SES) and racial/ethnic disparities in children's standardized math achievement at kindergarten entry. These early math skills predict future mathematics achievement and career success. However, limited research has been conducted using large sample sizes to understand how SES and race/ethnicity are related to children's numerical skills at even younger ages. The current study aims to investigate sociodemographic variability in three fundamental areas of early numeracy: nonverbal numerosity discrimination, rote counting, and cardinal number wor

Factors associated to motor development in Down syndrome patients.

From Infancy to Early School Age: Longitudinal Developmental Outcomes and Autism Diagnostic Stability in Tuberous Sclerosis Complex.

BACKGROUND: Tuberous sclerosis complex (TSC) is associated with intellectual disability (ID), autism spectrum disorder (ASD), and TSC-associated neuropsychiatric disorders. Early prospective studies have characterized neurodevelopmental outcomes in infants and toddlers with TSC. Leveraging longitudinal data from the TSC Autism Center of Excellence Research Network and the Rare Diseases Clinical Research Network, we examined longer-term developmental trajectories and ASD diagnostic stability in a cohort of children with TSC. METHODS: Participants were originally enrolled in TSC Autism Center o

Developmental literacy and epistemic dignity as primary child protection: a developmentally staged, disability-inclusive public-health framework for preventing adverse childhood experiences.

Adverse childhood experiences (ACEs), toxic stress, relational insecurity, and structural adversity are major public-health concerns for children and adolescents, but ACE evidence should not be treated as an individual diagnostic score or deterministic prognosis. This Hypothesis and Theory article proposes developmental literacy and epistemic dignity as linked upstream constructs for primary child protection. Developmental literacy is defined as developmentally staged and disability-inclusive neurobiopsychosocial knowledge and skills concerning bodies, emotions, stress, attachment, co-regulati

Estimating Mean Growth Trajectories When Measurements Are Sparse and Age Is Uncertain.

OBJECTIVES: Comparing children's growth across the world and at different moments in history can yield insight into both health challenges and healthy morphological variation in humans. A difficulty of such comparative analyses is that, in marginalized populations, there are often logistical complications to obtaining repeat measures of individual children's height and weight. The problem is even more acute for historical populations: bioarcheological datasets comprise single measures of individuals at death. Additionally, for both contemporary and historical populations, there is often non-tr

Genetic and Environmental Influences on the Development of Callous-Unemotional Behaviors in the Preschool Period.

Callous-Unemotional behaviors (CU) can be seen in early childhood and are associated with negative developmental outcomes. The present study examined the genetic and environmental influences on the development of CU in the preschool period from both rank-order stability/instability and absolute-level developmental trajectories perspectives. A community sample of 310 twin pairs (MZ = 123; DZ = 187; 51% female) was longitudinally assessed with the Child Behavior Checklist 11/2-5 reported by primary caregivers at ages 3, 4 and 5 years. Genetic and environmental contributions to rank-order stabili

Exploring the feasibility and effectiveness of a naturalistic family centered intervention to enhance early interactions in toddlers with Down syndrome.

This study analyses the feasibility and effectiveness of BabyMICARE, a manualised intervention programme aimed at improving the interactions between caregivers and infants and toddlers with Down syndrome. The programme's goal is to enhance caregivers' sensitivity and reduce directivity during early interactions, particularly during play and daily routines. A pre-test and post-test design was used with 40 dyads of infants with Down syndrome and their caregivers, who were divided into a control group (n = 20) and an intervention group (n = 20), based on baseline scores in key interaction subscal
