Predictive Models for Sustained, Uncontrolled Hypertension and Hypertensive Crisis Based on Electronic Health Record Data: Algorithm Development and Validation.

BACKGROUND: Assessing disease progression among patients with uncontrolled hypertension is important for identifying opportunities for intervention. OBJECTIVE: We aim to develop and validate 2 models, one to predict sustained, uncontrolled hypertension (≥2 blood pressure [BP] readings ≥140/90 mm Hg or ≥1 BP reading ≥180/120 mm Hg) and one to predict hypertensive crisis (≥1 BP reading ≥180/120 mm Hg) within 1 year of an index visit (outpatient or ambulatory encounter in which an uncontrolled BP reading was recorded). METHODS: Data from 142,897 patients with uncontrolled hypertension within At

Gender Bias When Using Artificial Intelligence to Assess Anorexia Nervosa on Social Media: Data-Driven Study.

BACKGROUND: Social media sites are becoming an increasingly important source of information about mental health disorders. Among them, eating disorders are complex psychological problems that involve unhealthy eating habits. In particular, there is evidence showing that signs and symptoms of anorexia nervosa can be traced in social media platforms. Knowing that input data biases tend to be amplified by artificial intelligence algorithms and, in particular, machine learning, these methods should be revised to mitigate biased discrimination in such important domains. OBJECTIVE: The main goal of

Cerebrospinal fluid cytokines and chemokines exhibit distinct profiles in bacterial meningitis and viral meningitis.

Differential diagnosis of bacterial meningitis (BM) and viral meningitis (VM) is a critical clinical challenge, as the early and accurate identification of the causative agent determines the appropriate treatment regimen and markedly improves patient outcomes. Clinical and experimental studies have demonstrated that the pathogen and the host immune response contribute to mortality and neurological sequelae. As BM is associated with the activation of an inflammatory cascade, the patterns of pro- and anti-inflammatory cytokines/chemokines (CTs/CKs) present in the cerebrospinal fluid (CSF) in res

Construction and Validation of a m7G-Related Gene-Based Prognostic Model for Gastric Cancer.

BACKGROUND: Gastric cancer (GC) is one of the most common malignant tumors of the digestive system. Chinese cases of GC account for about 40% of the global rate, with approximately 1.66 million people succumbing to the disease each year. Despite the progress made in the treatment of GC, most patients are diagnosed at an advanced stage due to the lack of obvious clinical symptoms in the early stages of GC, and their prognosis is still very poor. The m7G modification is one of the most common forms of base modification in post-transcriptional regulation, and it is widely distributed in the 5' ca

In Vivo Longitudinal Monitoring of Cardiac Remodeling in Murine Ischemia Models With Adaptive Bayesian Regularized Cardiac Strain Imaging: Validation Against Histology.

Adaptive Bayesian regularized cardiac strain imaging (ABR-CSI) uses raw radiofrequency signals to estimate myocardial wall contractility as a surrogate measure of relative tissue elasticity incorporating regularization in the Bayesian sense. We determined the feasibility of using ABR-CSI -derived strain for in vivo longitudinal monitoring of cardiac remodeling in a murine ischemic injury model (myocardial infarction [MI] and ischemia-reperfusion [IR]) and validated the findings against ground truth histology. We randomly stratified 30 BALB/CJ mice (17 females, 13 males, median age = 10 wk) int

An early model to predict the risk of gestational diabetes mellitus in the absence of blood examination indexes: application in primary health care centres.

BACKGROUND: Gestational diabetes mellitus (GDM) is one of the critical causes of adverse perinatal outcomes. A reliable estimate of GDM in early pregnancy would facilitate intervention plans for maternal and infant health care to prevent the risk of adverse perinatal outcomes. This study aims to build an early model to predict GDM in the first trimester for the primary health care centre. METHODS: Characteristics of pregnant women in the first trimester were collected from eastern China from 2017 to 2019. The univariate analysis was performed using SPSS 23.0 statistical software. Are Current Survival Prediction Tools Useful When Treating Subsequent Skeletal-related Events From Bone Metastases?

BACKGROUND: Bone metastasis in advanced cancer is challenging because of pain, functional issues, and reduced life expectancy. Treatment planning is complex, with consideration of factors such as location, symptoms, and prognosis. Prognostic models help guide treatment choices, with Skeletal Oncology Research Group machine-learning algorithms (SORG-MLAs) showing promise in predicting survival for initial spinal metastases and extremity metastases treated with surgery or radiotherapy. Improved therapies extend patient lifespans, increasing the risk of subsequent skeletal-related events (SREs).

A machine learning-derived aging index for risk stratification and mortality prediction in cardiovascular-kidney-metabolic syndrome: A retrospective cohort study.

BACKGROUND: The cardiovascular‑kidney‑metabolic (CKM) syndrome shows substantial heterogeneity in progression, yet existing biological age indices are not tailored to CKM pathways. We developed an aging index (CKMAI) using machine learning and evaluated its predictive performance for mortality and high‑risk CKM status. METHODS AND FINDINGS: We conducted a retrospective cohort study of 6,896 adults from the National Health and Nutrition Examination Survey (NHANES) 2005-2018, weighted for the US adult population. A Two-Stage Framework for Early Detection and Subtype Identification of Alzheimer's Disease Through Multimodal Biomarker Extraction and Improved GCN.

BACKGROUND: Imaging-transcriptomic analysis, through the integration of multimodal magnetic resonance imaging (MRI) and transcriptomic data, provides complementary structural, functional, and molecular information that is crucial for the early detection and mechanistic exploration of Alzheimer's disease (AD). However, effectively extracting features from heterogeneous multimodal data and capturing the associations between microscopic molecular variations and macroscopic brain alterations remain key challenges. Recent advances in deep learning and multimodal integration have enhanced the abilit

Circulating serotonin is associated with calcific aortic valve stenosis: a two-phase metabolomics study.

BACKGROUND: Calcific aortic valve stenosis (CAVS) is the most prevalent valvular heart disease in the elderly, yet no pharmacological therapy can halt its progression, partly due to incomplete understanding of disease-associated metabolic alterations and the lack of circulating biomarkers for disease detection. OBJECTIVE: We aimed to characterize metabolic alterations associated with CAVS and evaluate a candidate circulating marker for CAVS detection. METHODS: In the discovery cohort, patients with severe CAVS and matched controls without aortic valve disease were enrolled. Untargeted serum

Prediction models for endometriosis-associated ovarian malignancy: A systematic review and meta-analysis.

BACKGROUND: Endometriosis is benign, but it is associated with an increased risk of clear-cell and endometrioid ovarian carcinoma. Several multivariable prediction models have been developed to identify malignant transformation or differentiate endometriosis-associated ovarian cancer (EAOC) from benign ovarian endometrioma. It remains unclear whether these models are sufficiently validated and reported for clinical use. METHODS: We searched PubMed, Embase, Web of Science, the Cochrane Library, CINAHL, Scopus, CNKI, Wanfang, VIP, and SinoMed from inception to 24 May 2025. We included reports t

A Simple Clinical Risk Score to Improve Prediction of Anthracycline-Induced Cardiotoxicity.

BACKGROUND: Anthracycline-induced cardiotoxicity is a major limitation of cancer therapy, highlighting the need for accurate and practical risk stratification. Although current guidelines recommend the Heart Failure Association-International Cardio-Oncology Society (HFA-ICOS) score for baseline risk assessment, its complexity may limit routine clinical use. OBJECTIVES: This study sought to develop and validate a simplified risk score based on readily available clinical and echocardiographic variables for predicting anthracycline-related cardiotoxicity, and to compare its performance with the

Machine learning-based prediction of ischemic cardio-cerebrovascular events after endovascular or microsurgical treatment of unruptured intracranial aneurysms and risk stratification by the early post-treatment triglyceride-glucose index.

BACKGROUND: Ischemic cardio-cerebrovascular events (ICCEs), including acute coronary syndrome and ischemic cerebral infarction, remain clinically important complications after endovascular or microsurgical treatment of unruptured intracranial aneurysms (UIAs). However, early identification of patients at high post-treatment ischemic risk remains challenging, and reliable risk-stratification tools are lacking. OBJECTIVE: To develop a machine learning-based framework for predicting ischemic cardio-cerebrovascular events (ICCEs) within 6 months after treatment in patients with unruptured intracr

A two-phase machine learning framework for coronary heart disease risk prediction: Integrating population ecology with individual pathophysiology.

OBJECTIVE: Coronary heart disease (CHD) shows significant regional variations that cannot be fully explained by individual risk factors alone, suggesting the need for an analytical framework that integrates macro-level environmental influences with individual-level pathophysiology. This study developed and validated a novel two-phase, interpretable machine learning (ML) framework for identifying and validating multi-level predictors of CHD risk. METHODS: In Phase 1 (Population-Level Discovery), we employed a consensus-driven Recursive Feature Elimination (RFE) pipeline across 12 regression al

A pyroptosis-related gene signature for the diagnosis of acute pancreatitis.

Acute pancreatitis (AP) is a severe inflammatory disorder in which pyroptosis-a pro-inflammatory form of programmed cell death-may contribute to pathogenesis. However, the complete transcriptional profile of pyroptosis-related genes (PRGs) in AP and their potential as diagnostic biomarkers remain underexplored. This study aimed to systematically characterize pyroptosis-associated transcriptional signatures and identify the reliable biomarkers for diagnostic purposes. Three transcriptomic datasets from murine AP models were integrated to identify pyroptosis-related differentially expressed gene

Quality over quantity: biopsy-anchored CT radiogenomics models outperform all-lesion training in a multi-tumour cohort despite a smaller sample size.

OBJECTIVE: Radiogenomics aims to non-invasively predict tumour genotypes from imaging, but most studies assume molecular homogeneity by assigning a single biopsy-derived label to all lesions within a patient. This approach risks substantial label noise given well-documented interlesional heterogeneity. We investigated whether anchoring training to biopsy-confirmed lesions improves radiogenomic model performance and generalisability. MATERIALS AND METHODS: We retrospectively analysed 1646 patients (11473 segmented lesions) with contrast-enhanced CT and EGFR mutation status from next-generation

Development and External Validation of an Interpretable Machine Learning-Based Prediction Model for Depressive Symptoms in Patients With Obstructive Sleep Apnea: A Multicenter Study.

INTRODUCTION: Depressive symptoms commonly co-occur with obstructive sleep apnea (OSA), increase disease burden, and may precede major depressive disorder (MDD). In routine sleep-clinic practice, mood symptoms can be overlooked during the work-up for suspected OSA and at the time of polysomnography (PSG)-confirmed diagnosis, because consultations focus primarily on sleep-breathing and cardiometabolic complaints. Guided by the biopsychosocial model, this multicenter study aimed to develop and validate an interpretable machine-learning (ML) model to predict the risk of depressive symptoms in pat
