Impact of skull base development on endonasal endoscopic surgical corridors.

OBJECT: Scarce morphometric data exist on the developing skull base as a corridor for endonasal endoscopic approaches (EEAs). Furthermore, the impact of skull base lesions on its development has not been assessed. The authors describe a novel set of anatomical parameters characterizing the developmental process as well as the utility of these parameters in preoperative planning and a feasibility assessment of EEAs for neurosurgical treatment of skull base lesions in children. METHODS: Based on specific MRI sequences in 107 pediatric patients (2-16 years of age) without skull base lesions (ref

The anatomical relations of the sphenoid sinus and their implications in sphenoid endoscopic surgery.

Alongside the past development of endoscopic sinus surgery, knowledge about the anatomy of the sinuses has become crucial for surgeons. The sphenoid sinus is one of the most variable of all sinuses. Its relations to vital vascular and nervous elements make its approach a challenge for endoscopic surgeons. These relations include the internal carotid artery, the optic nerve, the pituitary gland, the Vidian nerve, and other vascular and nervous elements depending on the size of the sinus. This paper will make a review of these anatomical relations and their importance in endoscopic sphenoid surg

Endoscopic assistance in the diagnosis and treatment of odontogenic maxillary sinus disease.

PURPOSE: Endoscopic sinus surgery has become an increasingly popular treatment for most surgical cases of chronic sinusitis. However, in some cases, a modification of the classic Caldwell-Luc operation is unavoidable. The purpose of this paper is to present the outcome of 20 cases with pathology of the maxillary sinus which was approached endoscopically on diagnostic or interventional purpose. The primary indication for endoscopy for 11 cases was acute or chronic odontogenic sinusitis. For six cases, endoscopy was performed for removing a foreign body from the sinus cavity and for the remainin

New landmark for the endoscopic endonasal transsphenoidal approach of pituitary surgery.

OBJECTIVE: To clarify the anatomical correlations of the sphenoid sinus with surrounding structures in the normal Korean population, and to identify surgical landmarks for safe sellar floor dissection in the anterior skull base by endoscopy and microscopy. METHODS: We reviewed the 196 brain magnetic resonance imaging findings showing a normal appearance, and measured the distances between anatomical landmarks. RESULTS: The mean distances from the base of the columella to the anterior wall of the sphenoid sinus and the sellar floor were 69.71±4.25 mm and 86.26±4.57 mm, respectively in the ove

Transnasal endoscopic removal of dental implants from the maxillary sinus.

PURPOSE: The penetration of dental implants into the maxillary sinus is becoming more common as a result of the use of new techniques to rehabilitate edentulous jaws. This complication requires the removal of the foreign body, even in asymptomatic patients, to prevent inflammatory disease. This article reports the authors' experience with the surgical removal of dental implants from the maxillary sinus with an endoscopic approach via middle meatotomy. MATERIALS AND METHODS: A 10-year (2001 to 2011) retrospective analysis was performed in the authors' departments to identify all patients who u

Endoscopic ultrasound-guided drainage of pancreatic fluid collections in children.

BACKGROUND AND OBJECTIVE: Although endoscopic ultrasound (EUS)-guided drainage of pancreatic fluid collections (PFCs) has evolved as the standard of care in adults, its role as a single-step drainage modality in children is unclear. The aim of the present study was to evaluate the efficacy and safety of single-step EUS-guided drainage of PFCs in children. METHODS: This is a retrospective study of all of the children who underwent single-step EUS-guided drainage of PFCs during a 4-year period at 1 institution. An endoscopic retrograde cholangiopancreatography was attempted before EUS-guided dr

Is endoscopic therapy the treatment of choice in all patients with chronic pancreatitis?

Chronic pancreatitis (CP) is a progressive inflammatory disease of the pancreas characterized by destruction of the pancreatic parenchyma with subsequent fibrosis that leads to pancreatic exocrine and endocrine insufficiency. Abdominal pain and local complications (bile duct or duodenal stenosis and pancreatic tumor) secondary to CP are indications for therapy. At the beginning, medical therapy is used. More invasive treatment is recommended for patients with pancreatic duct stones (PDS) and pancreatic obstruction in whom standard medical therapy is not sufficient. Recently, Clarke et al asses

Endoscopic retrograde cholangiopancreatography with rendezvous cannulation reduces pancreatic injury.

AIM: To examine whether rendezvous endoscopic retrograde cholangiopancreatography (ERCP) is associated with less pancreatic damage, measured as leakage of proenzymes, than conventional ERCP. METHODS: Patients (n = 122) with symptomatic gallstone disease, intact papilla and no ongoing inflammation, were prospectively enrolled in this case-control designed study. Eighty-one patients were subjected to laparoscopic cholecystectomy and if intraoperative cholangiography suggested common bile duct stones (CBDS), rendezvous ERCP was performed intraoperatively (n = 40). Patients with a negative cholan

An impacted denture in the oesophagus-an endoscopic or a surgical emergency-a case report.

Accidentally swallowed dentures can lead to severe complications in the gastrointestinal tract. Therefore, an early detection and an effective therapeutic management are important. In the era of advanced endoscopy, though the primary treatment of an impacted foreign body in the oesophagus is an endoscopic removal, a primary surgical treatment is advised for dentures with sharp hooks. Also, a prolonged impaction of dentures with sharp hooks leads to an increase in the complication rates. We are reporting a case of accidentally swallowed denture. After the diagnosis of an impacted denture was ma

Endoscopic endonasal transclival approach to a ventral pontine pediatric ependymoma.

The authors report a case of a recurrent pediatric ventral pontine ependymoma that they resected through an endonasal endoscopic transclival approach. Regarding the options for a surgical approach to ventral pontine tumors, traditional far-lateral approaches are associated with considerable morbidity due to the required muscle mobilization, brain retraction, and in-line obstruction of cranial nerves before reaching the target. The endoscopic endonasal transclival approach was made appealing by eliminating all of these concerns. The patient's fully pneumatized sphenoid sinus, laterally displace

A cadaveric study of the endoscopic endonasal transclival approach to the basilar artery.

The anterior transclival route to basilar artery aneurysms is not widely performed. The objective of this study was to carry out a feasibility assessment of the transclival approach to basilar aneurysms with advanced endonasal techniques on 11 cadaver heads. Clival dura was exposed from the sella to the foramen magnum between the paraclival segments of the internal carotid arteries (ICA) laterally. An inverted dural "U" flap was reflected inferiorly to expose the basilar artery. The maximal dimensions from operative measurements were recorded. Surgical manoeuvrability of multiple instruments a
