Reconstruction of the Femoral Diaphysis Using the Vascularized Fibula Flap: A Case Report.

The vascularized fibula flap is often utilized in various reconstructive surgeries to cover a bone or soft tissue defect due to its generous length, adequate vascularization, and the possibility of removal with minimal loss of limb function. We present a 32-year-old male patient with a 27-cm femoral diaphysis defect caused by a high-impact car accident, which led to multiple femoral fractures and was further complicated by osteomyelitis. Initial consultations with other physicians advised limb amputation as the sole approach due to the severity of the case and potential complications. Upon pre

Case Report: A severe case of traumatic ankle fracture-dislocation with medial malleolar bony defect and 5-year follow-up.

Open fracture-dislocations with significant malleolar bone and soft tissue loss, along with ligament injuries, are complex and often require extensive treatment and staged surgeries. The aim of this report is to highlight treatment strategies and key considerations, including surgical reconstruction with allografts or autografts and various flap techniques as viable options. Additionally, deltoid ligament repair or reconstruction is crucial for surgical success. We report the case of a 29-year-old woman with no significant medical history who sustained an open fracture-dislocation of the left

Identification of Postn+ periosteal progenitor cells with bone regenerative potential.

Bone contains multiple pools of skeletal stem/progenitor cells (SSPCs), and SSPCs in periosteal compartments are known to exhibit higher regenerative potential than those in BM and endosteal compartments. However, the in vivo identity and hierarchical relationships of periosteal SSPCs (P-SSPCs) remain unclear due to a lack of reliable markers to distinguish BM SSPCs and P-SSPCs. Here, we found that periosteal mesenchymal progenitor cells (P-MPs) in periosteum can be identified based on Postn-CreERT2 expression. Postn-expressing periosteal subpopulation produces osteolineage descendants that fu

Fixation with multiple K-wires and PRP injection as an effective way to manage heel pad avulsion.

The treatment of acute heel pad avulsion differs significantly from managing other soft tissue injuries due to its unique soft tissue structure. This case report outlines a scenario involving a male in his 20s who suffered heel pad avulsion without a calcaneal fracture but with an ipsilateral medial malleolus fracture after a twisting injury to the ankle caused by a road traffic accident. Immediate action was taken within 24 hours of the injury, involving thorough debridement of the wound, fixation to the calcaneum using multiple K-wires, primary suturing and internal fixation of the medial ma

Physical Therapy Approaches for Concomitant Anterior Cruciate Ligament and Posterior Cruciate Ligament Avulsion Fractures: A Case Report.

Multi-ligament knee injury, involving complex destruction of structures like the anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL), presents significant challenges in treatment as well as diagnosis. This case study focuses on a 55-year-old woman who was the victim of a severe traffic accident and had an ACL and PCL avulsion fracture, a medial condyle fracture, and a medial collateral ligament rupture. Afterward, open reduction and internal fixation of the right tibial plateau surgery was performed after a delay in order to manage the pain and swelling. Her rehabilitation w

Axial-Radial-Ulnar Carpal Dislocation with Divergent Intermetacarpal Dislocations, Peritrapezoid and Scaphotrapezial Dislocations: A Case Report.

We report a patient with a severe axial-radial-ulnar (ARU) fracture dislocation of the carpus, involving multiple intermetacarpal dislocations as well as divergent carpometacarpal dislocations involving the index, long, ring and small fingers and peritrapezoid and scaphotrapezial dislocations. She also had a degloving injury involving the dorsal hand. Emergent debridement followed by open reduction and internal fixation of all injuries was performed, followed by soft tissue management. At 6-year follow-up, the patient had adequate active range of motion with the ability to make a full fist and

Computational predictions of hydrogen-assisted fatigue crack growth.

A new model is presented to predict hydrogen-assisted fatigue. The model combines a phase field description of fracture and fatigue, stress-assisted hydrogen diffusion, and a toughness degradation formulation with cyclic and hydrogen contributions. Hydrogen-assisted fatigue crack growth predictions exhibit an excellent agreement with experiments over all the scenarios considered, spanning multiple load ratios, H 2 pressures and loading frequencies. These are obtained without any calibration with hydrogen-assisted fatigue data, taking as input only mechanical and hydrogen transport material

Atypical presentation of an inferior shoulder dislocation with an engaging Hill-Sachs lesion: A case report.

CASE: Inferior shoulder dislocations (ISD) are very rare occurrences estimated to make up only 0.5% of all shoulder dislocations and are typically associated with high-energy trauma and humerus fractures. We present an unusual case of an ISD due to the absence of the pathognomonic arm posture, mimicking an anterior shoulder dislocation. After multiple failed attempts at closed reduction in the Emergency Department, orthopaedics was consulted for further evaluation. Appropriate imaging was ordered, including an axillary radiograph and CT scan, which demonstrated an ISD with an engaging Hill-Sac

Damage Control Management for Thoracic Trauma with Cardiac Arrest Complicated by Emphysematous Gastritis and Cystitis.

A 78-year-old man was found unconscious after sliding from a rock. His history included hypertension, atrial fibrillation and cerebral infarction requiring warfarin. On arrival, he received six units of blood type O transfusion and vitamin K in an emergency room (ER) due to hemorrhagic shock. His systolic blood pressure temporarily increased to 100 mmHg, and he underwent traumatic pan scan revealing occipital fracture, cerebral contusion, and cervical and multiple left rib fractures with left-dominant bilateral hemothorax. He re-entered a shock state after the examination and underwent transfu

Recurrent acromioclavicular joint dislocation with an associated coracoid fracture following acromioclavicular joint reconstruction.

Acromioclavicular joint injuries are common and account for up to 12% of all shoulder girdle injuries and have been reported as being as high as 50% in sports-related shoulder injuries. While the majority of acromioclavicular joint dislocations can be treated non-operatively, there are certain injury configurations, which can include high-grade dislocations in overhead athletes, where surgery may be indicated. The surgical management of acromioclavicular joint instability has moved towards recreating the action of the coracoclavicular ligaments by resuspending the clavicle on to the coracoid.

Soft tissue and wound management of blast injuries.

The management of blast-related soft tissue wounds requires a comprehensive surgical approach that acknowledges extensive zones of injury and the likelihood of massive contamination. The experiences of military surgeons during the last decade of war have significantly enhanced current understandings of the optimal means of mitigating infectious complications, the timing of soft tissue coverage attempts, and the reconstructive options available for definitive wound management. Early administration of antibiotics in the setting of soft tissue wounds and associated open fractures is the single mo

Metabolic bone diseases in kidney transplant recipients.

Metabolic bone disease after kidney transplantation has a complex pathophysiology and heterogeneous histology. Pre-existing renal osteodystrophy may not resolve completely, but continue or evolve into a different osteodystrophy. Rapid bone loss immediately after transplant can persist, at a lower rate, for years to come. These greatly increase the risk of bone fracture and vertebral collapse. Each patient may have multiple risk factors of bone loss, such as steroids usage, hypogonadism, persistent hyperparathyroidism (HPT), poor allograft function, metabolic acidosis, hypophosphatemia, vitamin

Intrarenal post-traumatic pseudoaneurysm-USG colour Doppler diagnosis: a case report with review of literature.

Renal artery pseudoaneurysm, although a well-known complication in patients after penetrating trauma or iatrogenic injury, has rarely been described after blunt torso trauma. A 22-year-old man suffered an accident while on a motorcycle. Upon arrival in the Emergency Room, the patient was unconscious, hypotensive and had hematuria. Initial investigations revealed a fractured shaft of the right femur, multiple rib fractures with hemothorax, extradural haematoma on the right side and a laceration in the right kidney. After the evacuation of intracranial bleed, the patient was managed conservative

Some problems in the laboratory findings in multiple myeloma.

The clinical usefulness of alkaline phosphatase (ALP) and lactate dehydrogenase (LDH) levels in serum and pathogenetic mechanism of hypoalbuminemia and hypocholesterolemia in multiple myeloma (MM) were investigated. In cases of MM with a history of pathological fracture, the level of serum ALP was significantly higher than normal. Thus, elevated ALP in MM patients may be an indicator of the occurrence of a pathological fracture within the past 2 months. The levels of serum LDH in about 80% of the MM patients were within normal limits despite the presence of a malignant tumor. These patients sh

The orthopedic manifestation of neurofibromatosis. A clinical experience and review of the literature.

A retrospective review of 47 patients with neurofibromatosis was performed to evaluate the effects of that disease on patient function and disability. Clinical manifestations were distributed as follows: cafe-au-lait markings were the most prevalent manifestation (87%), followed by a positive family history (49%), scoliosis (53%), neurofibromata (19%), and pseudarthrosis of the tibia (19%). The classic scoliosis was resistant to brace treatment; bracing failed in 70% of patients, necessitating spinal fusion. Pseudarthrosis of the tibia can be subdivided into normal, narrow sclerotic, and cysti

A predictable cascade of junctional failure following progressive long-segment fusion: an 11-year case analysis.

Instrumented spinal fusion is a common treatment for degenerative spinal disease but may lead to progressive mechanical and biological complications, especially in long multilevel constructs. We report the case of a 62-year-old woman who developed serial adjacent segment degeneration, pseudarthrosis, implant failure, vertebral fractures, and progressive deformity over 11 years following posterior lumbar fusion. Multiple revision surgeries with stepwise cranial extension to the upper thoracic spine were required. Despite advanced fixation techniques-including multi-rod constructs, iliac fixatio

Intentional Stent Fracture to Accommodate Growth After Coarctation Stenting in an 860-g Neonate.

BACKGROUND: Congenital aortic coarctation in neonates and children can be managed with transcatheter stent implantation, but this approach raises the issue of how to accommodate the fixed stent structure within the growing vessel. CASE SUMMARY: A 15-month-old boy, born prematurely with critical aortic coarctation, was treated at 1 week of age with the implantation of a coronary stent. Subsequently, multiple stent dilatations were needed to adapt it to the growing aortic vessel. At 3 months the patient underwent balloon dilation with the simultaneous implantation of a second stent, and after 1

A Modified Direct Lateral Approach for Lateral Border and Scapular Spine Fixation in Floating Shoulder.

Floating shoulder injuries are rare but require careful management to ensure functional recovery. Traditional approaches for scapular fixation often necessitate multiple incisions. We present a modification of the direct lateral approach that allows for scapular lateral border and scapular spine fixation using a single incision with utilization of subdeltoid space. A 48-year-old male patient presented with a displaced extra-articular scapular fracture associated with a midshaft clavicle fracture following a motor vehicle accident. Subcondylar Fossa Reconstruction in Pediatric Patients Following Malunion of Proximal Phalangeal Neck Fractures: Technique and Case Series.

The subcondylar fossa is the bony depression between the articulating surface of the proximal and middle phalanges of the fingers that allows for full flexion at the proximal interphalangeal joint. A bony block may form in the subcondylar fossa secondary to the malunion of a proximal phalangeal neck fracture. If there is a substantial loss of flexion, surgical correction could be indicated. Corrective osteotomy is technically challenging in the proximal interphalangeal joint area as the phalangeal head is small, creating fixation challenges and carrying the risk of avascular necrosis and joint
